Fungi are schematically responsible for three distinct kinds of infections: superficial mycoses, subcutaneous and deep ones. The current socio-epidemiological transition observed in sub-Saharan Africa does not actually lead to similar consequences regarding these three categories of fungal entities. For instance, it has long been known that superficial mycoses are very prevalent in tropical areas, since they are partly due to the warm climate and the promiscuity. They are mostly caused by dermatophytic fungi or Malassezia sp. (Pityriasis versicolor). Subcutaneous mycoses are rarer, and usually…

Antibodies against a 33-kDa antigen from Coccidioides immitis were detected by ELISA in patients' cerebrospinal fluid (CSF). Anti-33-kDa antibodies were detected at dilutions > 1:80 in only 1 (1.4%) of 73 patients without coccidioidal meningitis but in 74 (71.8%) of 103 with meningitis. Anti-33-kDa antibodies were detected in 53 (91.4%) of 58 patients whose anti-coccidioidal complement-fixing (CF) antibodies were detectable and in 21 (46.7%) of 45 patients whose CSF was negative by CF test (positive predictive value, 99%; negative predictive value, 71%; sensitivity, 72%; specificity, 99%). A variety of superficial and deep mycoses may affect the immunocompromised patient. Among the superficial mycoses, candidal infections are common in all groups, but dermatophyte and pityrosporum infections may also be found. Although not primarily dangerous, they may lead to secondary bacterial infections and morbidity. Of the systemic mycoses, candidiasis, aspergillosis, and mucor-mycosis are frequently lethal and require early diagnosis and aggressive antifungal treatment. Endemic mycoses, such as histoplasmosis and coccidioidomycosis, may result in severe and often fatal infections in those…

In order to clarify the present state of opportunistic fungal infections increasing in incidence in autopsy cases, all autopsy cases from 1966 to 1975 reported in the Annual of Pathological Autopsy Cases in Japan were reviewed. Of the total 233,130 autopsy cases, mycoses were present in 4,340 (1.86%). The incidence of mycoses has strikingly increased during the recent five-year period. In Japan, the mycoses most frequently occurring in autopsy cases were candidiasis (32.28%), aspergillosis (23.08%), cryptococcosis (9.63%), and mucormycosis (2.90%). These occurred more frequently in younger per…

Because deep opportunistic mycoses are relatively rare, clinicians frequently lack routine in the application of antimycotics. It is the purpose of this review to summarize, from a practical point of view, rationale, indications, applications and toxicity of antimycotic therapy for the mycoses autochthonous to Europe. Amphotericin B remains the standard therapy for most deep opportunistic mycoses, while the newer azoles are the first line agents to be used for superficial forms of candidiasis.

In order to provide insight into the role of accessory cells in lymphoproliferative neoplasms, 7 cases of mycosis fungoides at various clinical stages--patches, plaques and nodules--were studied ultrastructurally and immunohistochemically. The aim was to establish whether interdigitating reticulum cells are a constant finding in the dermal infiltrate. Their possible relationships with mycosic cells were also investigated. This study revealed that interdigitating reticulum cells were present in all the skin lesions examined, were present in considerable number in the patches and plaques and bec

After some general epidemiological data concerning mycoses occurring during the acquired immunodeficiency syndrome (AIDS), the role of the pathologist in the diagnosis of mycotic diseases is reviewed. Histological methods and criteria allowing fungal identification in tissues are described. Changes in the inflammatory reaction in contact with mycoses arising in these immunodeficient hosts are reported. The various mycoses observed in HIV positive patients are then reviewed as well as their morphological features and their various sites.

Invasive fungal infections have become a major cause of morbimortality in intensive care patients, persons suffering from cancer or immune deficiencies, and other diseases with impaired immunity. Candida albicans remains the most frequent fungal pathogen, but advances in the diagnosis, prevention and treatment of invasive candidiasis are leading to important etiological changes. Among the emerging invasive mycoses, are those caused by filamentous fungi, such as Aspergillus, Lomentospora/Scedosporium, Fusarium or the Mucorales. Invasive aspergillosis is difficult to diagnose, and although there…

Fungal infections are one of the most common diseases. Superficial mycoses affect the skin and visible mucous membranes. Deep mycoses include organ and systemic mycoses. The incidence of these diseases is increasing due to the use of broad-spectrum antibiotics, corticosteroids and cytostatics. Mycoses (yeasts and filamentous fungi) occur more often in patients with burns than in other patients; especially in patients with large and deep extent burns. In this case report, we are presenting a case of a 62-year-old patient hospitalized at the Department of Burns and Plastic Surgery, University Ho…

Among diseases of horses caused by fungi (=mycoses), dermatophytosis, cryptococcosis and aspergillosis are of particular concern, due their worldwide diffusion and, for some of them, zoonotic potential. Conversely, other mycoses such as subcutaneous (i.e., pythiosis and mycetoma) or deep mycoses (i.e., blastomycosis and coccidioidomycosis) are rare, and/or limited to restricted geographical areas. Generally, subcutaneous and deep mycoses are chronic and progressive diseases; clinical signs include extensive, painful lesions (not pathognomonic), which resemble to other microbial infections. Untreated dermatological mycoses are easy to diagnose. Once treatment with corticosteroids has been initiated the diagnosis of a mycosis can be difficult. In the immunosuppressed patient (AIDS patients) typical dermatological manifestations can be lacking. Systemic antimycotic therapy requires precise detection of the pathogen concerned. Mistakes still made in surgical practice are incision of a tumour in the case of tinea profunda and the extraction of nails affected by fungi. In the healthy person yeasts are transient organisms present in the mouth and intestinal tract in contrast, the mouth…

An overview of important pulmonary mycoses is presented. Four endemic mycoses occur in restricted geographic areas: histoplasmosis, blastomycosis, coccidioidomycosis, and paracoccidioidomycosis. Three opportunistic mycoses have causative agents that are ubiquitous in nature but cause infection only in compromised hosts: aspergillosis, mucormycosis, and candidiasis. Cryptococcosis is acquired by inhalation but usually presents as a chronic meningitis. Pulmonary sporotrichosis is rare; the usual subcutaneous form of the disease is acquired by inoculation. The clinical features of these infection…

Although endemic mycoses are a frequent health problem in Latin American countries, clinical and epidemiological data remain scarce and fragmentary. These mycoses have a significant impact on public health, and early diagnosis and appropriate treatment remain important. The target population for endemic disease in Latin America is mostly represented by low-income rural workers with limited access to a public or private health system. Unfortunately, diagnostic tools are not widely available in medical centers in Latin America; consequently, by the time patients are diagnosed with fungal infecti…

Endemic mycoses are important fungal infections in their respective habitats. In the Asia-Pacific region, an accurate epidemiological picture of endemic mycoses is elusive; few epidemiological surveys have been performed, and limited laboratory facilities and experience with fungal infections have further hampered recognition of infection. However, pockets of endemicity do indeed exist, and endemic fungal infections can have a significant impact on public health. This article reviews the most common endemic mycoses in the Asia-Pacific region: histoplasmosis, penicilliosis, and sporotrichosis. …

Report of three cases. Newly appeared immunosuppression related conditions have led to an increase in morbidity of systemic opportunistic mycoses. Rhinosinusal infection has also increased in frequency. Three invasive nasosinusal mycoses are presented: two cases of mucormycosis and a simultaneous infection by mucormycosis and candida albicans. In all cases, and despite treatment, progression of the disease could not be stopped and the patients died. The main characteristics of invasive nasosinusal mycoses are reviewed, to obtain early diagnosis. Early therapy improves the prognosis, and thus c…

UNLABELLED: Our work aimed to generate a preliminary estimation of severe and chronic fungal diseases in the Republic of Kazakhstan with a model proposed by LIFE (Leading International Fungal Education). Calculations were carried out on data from 2015. Published results of studies of mycoses in Kazakhstan were identified; in the absence of national data from the scientific literature, the frequency of life-threatening and serious mycoses in defined groups of patients at risk from other countries were taken into account. We also used analogous estimations of mycoses in the Russian Federation. W…

Subcutaneous mycoses are caused by a variety of mostly tropical organisms, usually when they are implanted into the dermis or the subcutaneous tissue. They rarely disseminate or become systemic. Sporotrichosis, mycetoma, and chromoblastomycosis are more common subcutaneous mycoses than are rhinosporidiosis, zygomycosis, pheohyphomycosis, and lobomycosis.

Fungal infections present with a broad spectrum of diseases in humans (from relatively mild superficial infections of the skin and mucous membranes to the invasive or chronic infections of internal organs, which have a high mortality rate). Globally, up to 1.6 million people die each year as a result of various types of mycoses. Currently, many scientific studies focus on the best possible understanding of the aspects of the epidemiology and pathogenesis of invasive mycoses and effective methods to combat them. However, mycoses of the skin and its appendages remain a relatively less explored a…

Comparable human postmortem surveys in central Kentucky and southern Florida have demonstrated an altered pattern of cerebral mycoses due primarily to therapeutic manipulations. From both states 8,975 complete autopsies yielded 39 patients with histologically verified cerebral mycoses. The most common infection was candidosis (49%), characterized by intraparenchymal microabscesses without significant leptomeningitis in hospitalized patients compromised by antibiotic therapy for infection with gram-negative organisms. The remaining 20 patients with noncandidal cerebral mycoses included 9 (23%) …
