Hiatal hernia mimicking ST elevation myocardial infarction.

A 72-year-old man with a history of hypertension, hiatal hernia, prostate cancer and lung cancer was admitted with complaints of abdominal pain, sweating and rigors. An electrocardiogram showed ST elevation in multiple leads. Noninvasive and invasive cardiovascular work-up was performed that was negative for occlusive coronary artery disease. Further studies demonstrated a large hiatal hernia; this was found to be the culprit causing his symptoms. Hiatal hernia is a very rare cause of ST segment elevation and should be considered in the differential diagnosis of disorders that can cause ST ele

Reactive thrombocytosis leading to acute myocardial infarction.

Thrombocytosis is defined as platelet count of > 600000. An elevated platelet count may be primarily (essential) or secondary (reactive). Acute myocardial infarction and other vaso-occlusive phenomenon are seen in less than 5% of the patients of reactive thrombocytosis. Here we report such a case. A 49 yr old lady presented with chest pain and had isolated right ventricular infarction that progressed to anteroseptal wall STEMI. Her platelet count was 11 lac; the triggering factor in this case was reactive thrombocytosis secondary to lower respiratory tract infection and iron-deficiency anaemia…

Cardiomyocyte mineralocorticoid receptor function post myocardial infarction.

Clinical trials have clearly demonstrated that mineralocorticoid receptor (MR) blockade improves outcome in patients with chronic systolic heart failure and left ventricular dysfunction after myocardial infarction; however, the underlying mechanisms as well as the cell-specific functional role of MR activation are still under investigation. Extrarenal effects of MR blockade on cardiovascular extracellular matrix turnover and oxidative stress, on myocardial structural and electrical remodeling, and on sympathoadrenergic stimulation, platelet activation, endothelial dysfunction, and macrophage p…

The snow-shoveler's ST elevation myocardial infarction.

Heavy snowfall, cold temperatures, and low atmospheric pressure during the winter months have been associated with increased adverse cardiovascular events. However, only a few cases of the "snow shoveler's infarction" have been reported. The investigators describe their experience with 6 patients presenting with ST elevation myocardial infarctions, all within a 24-hour period during an unprecedented snowfall (4 of whom were shoveling snow), and provide a detailed review of previously reported cases of snow shoveler's infarction. Consistent with other reports, most patients reported here had th…

Acute inferior myocardial infarction with nephrotic syndrome.

Although thrombotic complications in the venous system are common in patients with nephrotic syndrome, arterial thromboses associated with nephrotic syndrome are much less common. However, coronary thromboses are extremely rarely observed. Whereas venous thromboses predominate in adults, thromboembolic complications of the arterial system are mainly seen in children. We presented a case of acute inferior myocardial infarction secondary to a thrombotic occlusion of the right proximal coronary artery in a 21-year-old man with nephrotic syndrome due to renal amyloidosis. It is interesting that th…

Traffic-generated air pollution and myocardial infarction.

BACKGROUND: Although long-term air pollution exposure has been linked to cardiovascular mortality, data on incidence and nonfatal coronary disease are limited and inconclusive. The aim of this study was to investigate the association between long-term residential exposure to air pollution from traffic and the risk of nonfatal and fatal myocardial infarction. METHODS: The records of all individuals aged 15 to 79 with a first event of myocardial infarction in Stockholm County during 1985 to 1996 were retrieved from a registry. Population controls were randomly selected from the study base strat…

Impaired renal function in acute myocardial infarction.

Impaired renal function is a risk factor for cardiovascular disease and an adverse prognostic factor in patients with established cardiovascular disease. In addition, with current widespread use of invasive procedures in the treatment of acute myocardial infarction, contrast-induced nephropathy is a growing problem in this patient population. In acute myocardial infarction, impaired renal function may result from underlying kidney disease, acute renal failure, and the effect of drugs and contrast agents used during diagnostic procedures or treatment. These various causes may coexist, resulting…

Surgical therapies for post-myocardial infarction patients.

Occasionally, high-risk patients in the post-myocardial infarction (MI) period require surgical intervention for stabilization and/or revascularization. In a meta-analysis involving 3,088 patients with ischemic heart disease, revascularization was associated with nearly an 80% reduction in the risk of death. Coronary artery bypass graft (CABG) surgery is commonly performed in post-MI patients and is associated with more favorable outcomes than medical therapy. However, several factors have to be considered in proper patient selection for CABG, such as the left ventricular ejection fraction (LV…

Isolated right ventricular dysfunction without myocardial infarction.

Right ventricular (RV) infarction or dysfunction is commonly complicated with acute inferior myocardial infarction. In this report, we describe a patient with unstable angina pectoris who showed a temporary isolated RV dysfunction. The patient had total occlusion of proximal right coronary artery, including RV branch, with good collateral circulation from left coronary artery. Recanalization was obtained by percutaneous coronary intervention (PCI). Echocardiography at admission showed RV dilatation and severe hypokinesis of RV free wall but no left ventricular wall motion abnormalities. Hemody…

Myocardial infarction in pregnant women--case reports.

Five cases of women with MI in pregnancy are presented. Their past and present heart condition, therapy and cardiovascular disease risk factors were analyzed. The mean follow-up period was 14.6 years. The observed women had an atherosclerotic etiology of MI with multiple risk factors of ischaemic heart disease. Pulmonary oedema complicating MI in presented cases had no influence on long-term prognosis.

Acute myocardial infarction following scrub typhus infection.

Up to recently, a few cases of myocarditis in the cardiac manifestations of scrub typhus have been reported. However, acute myocardial infarction (AMI) associated with scrub typhus has not been previously reported. We presented a case of AMI ensued on scrub typhus, which were successfully treated by percutaneous coronary intervention (PCI).

Acute reperfusion therapy in acute myocardial infarction.

The ECG classification of acute myocardial infarctions has had a profound influence on the treatment of patients with AMI. Deciding whether a patient has ST-segment elevations or a new left bundle branch block or neither of these findings on ECG launches the treating physician down two different treatment pathways: patients with ST-elevation MI need to be assessed for immediate re-perfusion therapy, whereas patients with non-ST-elevation MI are best treated with aggressive medical management without acute reperfusion.

Myocardial infarction associated with intravenous immune globulin.

OBJECTIVE: To report a case of acute myocardial infarction (MI) experienced by a patient receiving intravenous immune globulin (IVIG) and review other published cases of MI associated with IVIG. CASE SUMMARY: An 81-year-old Vietnamese man was prescribed IVIG for treatment of toxic epidermal necrolysis secondary to allopurinol. Thirty minutes following the start of the IVIG infusion, the patient developed crushing retrosternal chest pain and shortness of breath. The pain improved upon discontinuation of IVIG infusion but recurred when IVIG was restarted. The troponin level reached 140 microg/L…
