ALK-negative retroperitoneal inflammatory myofibroblastic tumor treated conservatively: case report.

A 55-year-old female presented with left flank pain and ureteral obstruction. Imaging revealed a retroperitoneal mass suspicious for malignancy. Histopathology confirmed an inflammatory myofibroblastic tumor (IMT; anaplastic lymphoma kinase [ALK]-negative, mouse double minute 2 homolog-positive). Due to vascular encasement prohibiting safe resection, cystoscopic ureteral stenting was performed followed by 12 months of prednisone and celecoxib therapy. Serial imaging showed progressive regression, with near-resolution at 36 months. This demonstrates successful medical management of unresectable

Myofibroblast-Targeting Extracellular Vesicles: A Promising Platform for Cardiac Fibrosis Drug Delivery.

Current pharmacological treatments for cardiac fibrosis are often limited by their efficacy and specificity, leading to marked side effects. Fibroblast activation protein (FAP) is specifically expressed on activated myofibroblasts (myoFbs) but not on resting cardiac fibroblasts, making it a promising target for cardiac fibrosis therapy. In this study, we engineered extracellular vesicles (EVs) conjugated with an anti-FAP single-chain variable fragment, termed αFAP-EVs, which specifically target myoFbs. Our results demonstrated that αFAP-EVs successfully targeted activated myoFbs in vitro and l…

Targeting PIM1 by Bruceine D attenuates skin fibrosis via myofibroblast ferroptosis.

Skin pan-fibrosis diseases-such as hypertrophic scar (HS), keloid scar (KS), and systemic sclerosis (SSc)-pose significant threats to patients' health and quality of life. In this study, the authors conducted both in vivo and in vitro experiments and discovered that the serine/threonine kinase PIM1 is upregulated in the myofibroblasts of human HS, KS, and SSc tissues, as well as in various animal models of skin fibrosis. Overexpression of PIM1 enhanced the profibrotic phenotypes of human hypertrophic scar fibroblasts (HSFs), which serve as key effector cells in the pathogenesis of skin pan-fib…

Pulmonary Inflammatory Myofibroblastic Tumor in a Patient Treated for Pemphigus Vulgaris: A Case Report.

Inflammatory myofibroblastic tumors (IMTs) are benign tumors with diverse histological presentations. Pulmonary IMTs are particularly uncommon. Diagnosis often necessitates surgical intervention for both therapeutic and diagnostic purposes. We report the case of a 70-year-old woman with a history of uncontrolled asthma and pemphigus vulgaris treated with rituximab and corticosteroids. She presented with chest pain and a dry cough, without systemic symptoms. Imaging revealed a solitary pulmonary nodule. Thoracoscopic resection confirmed the diagnosis of an IMT, characterized histologically by s

TGFB1/SMAD3-driven macrophage-myofibroblast transition promotes fibrosis progression in endometriosis.

Endometriosis is a chronic gynecological disorder characterized by progressive fibrosis, which is closely associated with clinical symptoms such as dysmenorrhea and infertility. While myofibroblast activation is central to fibrogenesis, the cellular origins and regulatory mechanisms remain incompletely understood. This study demonstrates that the macrophage-myofibroblast transition (MMT) is a novel source of myofibroblasts in endometriosis and is regulated by the TGFB1/SMAD3 signaling pathway. Using single-cell RNA sequencing, we identified a distinct subpopulation of CD68+ macrophages co-expr…

Recurrent Inflammatory Myofibroblastic Tumor of Larynx Harboring a Novel THBS1::ALK Fusion.

Inflammatory myofibroblastic tumor (IMT) is a rare soft tissue tumor primarily occurring in the abdominopelvic region of young patients, and it is characterized by spindle-shaped myofibroblasts, or fibroblasts surrounded by inflammatory infiltrate. Herein, we report a case of a 24-year-old male with a firm submucosal mass in the anterior right vocal fold diagnosed as an IMT that recurred 14 months later. The tumor demonstrated a novel THBS1::ALK fusion containing Exons 1-7 of the thrombospondin 1 (THBS1) gene fused to Exon 19 of the anaplastic lymphoma kinase (ALK) gene via next-generation seq…

Inflammatory myofibroblastic tumor of the liver after adrenal neuroblastoma surgery: a case report.

A boy aged 55 months was diagnosed with stage IV Neuroblastoma (NB) of the right adrenal gland 2 years ago. Preoperative chemotherapy was given and he was then treated with retroperitoneal tumor resection and lymph node dissection. After surgery, the children were transferred to the Hemato-Oncology Department for chemotherapy according to the high-risk group NB, with outpatient follow-up every 6 months. In the second postoperative year, abdominal computed tomography (CT) scan revealed a rounded hypodense area in the upper part of the right posterior lobe of the liver, with marked inhomogeneous

Congenital Peribronchial Myofibroblastic Tumors Harbor a Recurrent EGFR Kinase Domain Duplication.

Congenital peribronchial myofibroblastic tumor (CPMT) is a rare benign infantile pulmonary neoplasm that presents prenatally, or early in infancy, and exhibits distinctive histologic features characterized by the presence of cartilaginous islands intermixed with bland spindle cells, not uncommonly displaying prominent mitoses. Despite its benign nature, CPMT can lead to fetal demise, postnatal respiratory distress, or complications from perinatal surgical resection. Although the morphologic and clinical features of CPMT are well described, its molecular features and oncogenesis remain elusive.

[A case of inflammatory myofibroblastic tumor with rapidly progressive growth arising from the retroperitoneum].

A woman in her 70s was admitted to our institution with complaints of right hypochondrium pain. Abdominal computed tomography revealed a 13-mm retroperitoneal tumor between the liver and right kidney. The tumor rapidly increased to 82mm within 2 months, a necrotic change was inside the tumor, and the inflammation spread to the surrounding diaphragm and the peritoneum. The patient underwent surgical resection including the affected diaphragm and the peritoneum. Histopathological examination revealed a myofibroblastic spindle-cell proliferation with prominent infiltration of inflammatory cells,

Fertility-preserving management of an inflammatory myofibroblastic tumor: A case report and review of the literature.

Molecular diagnostics have broadened the categorization of mesenchymal tumors of the uterus. Knowledge of the increasing heterogeneity of uterine neoplasms is paramount for the gynecologist as the management and prognosis of these neoplasms differ from those of typical leiomyomas. In this case, a 26-year-old nulligravid patient underwent hysteroscopic management for an enlarging submucosal neoplasm of the uterus. She was found to have an inflammatory myofibroblastic tumor (IMT) after ALK (anaplastic lymphoma kinase) immunostaining. Upon review of pathologic characteristics, she was treated exp

Low-Grade Myofibroblastic Sarcoma on Back with Repeated Localized Recurrence and Regional Metastasis.

Low-grade myofibroblastic sarcoma (LGMS) is a rare spindle cell tumor with indolent course. Due to rarity and low-grade histologic features of LGMS, accurate diagnosis is challenging. We report a 63-year-old female patient with a three-month history of a 3.1 cm×2.5 cm sized, firm, skin-colored, painless, protruding left back mass. Initial excisional biopsy was performed and the mass was diagnosed as nodular fasciitis. After 18 months after excision, the mass recurred with pain and grew larger. Considering the clinical manifestations, diagnostic impression was changed as dermatofibrosarcoma pro

Inflammatory myofibroblastic tumour of the common bile duct: a case report and literature review.

INTRODUCTION: Inflammatory myofibroblastic tumour (IMT) of the common bile duct (CBD) is an extremely rare low-grade malignancy with various biological behaviours and a lack of specific clinical and histopathological features. Preoperative and intraoperative diagnosis are challenging, and a diagnostic delay may increase surgical complexity. CASE PRESENTATION: We present the case of a 34-year-old male with no relevant medical history who presented with jaundice of 20 days of evolution. Histology and immunohistochemistry confirmed the diagnosis of an IMT with anaplastic lymphoma kinase (ALK)-1

An MRTF-A-ZEB1-IRF9 axis contributes to fibroblast-myofibroblast transition and renal fibrosis.

Myofibroblasts, characterized by the expression of the matricellular protein periostin (Postn), mediate the profibrogenic response during tissue repair and remodeling. Previous studies have demonstrated that systemic deficiency in myocardin-related transcription factor A (MRTF-A) attenuates renal fibrosis in mice. In the present study, we investigated the myofibroblast-specific role of MRTF-A in renal fibrosis and the underlying mechanism. We report that myofibroblast-specific deletion of MRTF-A, achieved through crossbreeding Mrtfa-flox mice with Postn-CreERT2 mice, led to amelioration of ren…

[A Case of Right Hemicolectomy for Inflammatory Myofibroblastic Tumor Arising in the Right Transverse Colon].

The patient was a 21-year-old male. He presented with right lower abdominal pain and showed tenderness in the same area. An abdominal contrast-enhanced CT examination revealed a 45 mm tumor that continuously stained in the right transverse colon. The patient presented with tenderness and rebound tenderness. Due to a suspected submucosal tumor, laparotomy was performed and an elastic hard tumor of 5 cm in size was found on the serous membrane side of the right transverse colon. As malignancy could not be ruled out, a right hemicolectomy with lymph node dissection was performed. The pathological

An inflammatory myofibroblastic tumor of the small intestine presenting as jejunojejunal intussusception; A case report.

INTRODUCTION AND IMPORTANCE: An inflammatory myofibroblastic tumor (IMT) is an uncommon solid neoplasm of mesenchymal origin. They are usually seen in children and adolescents and commonly affect the lung, but they can nearly arise from every organ. The prevalence of IMT in the small bowel is very rare. The tumors have generally a benign clinical course, with some risk of local recurrence or distant metastasis. PRESENTATION OF CASE: This is a 55-year-old male patient who presented with intermittent abdominal pain, vomiting of ingested matter and loss of appetite for 2 weeks duration. On physi

Inflammatory Myofibroblastic Tumor of the Sigmoid Colon in an Infant: A Case Report and Literature Review.

Inflammatory myofibroblastic tumor (IMT) infrequently involves the sigmoid colon, and has not previously been described in an infant sigmoid colon. An inflammatory myofibroblastic tumor arose from the sigmoid colon of an 11-month-old boy, confirmed by anaplastic lymphoma kinase (ALK), smooth muscle actin (SMA) and desmin immunohistochemical staining. The patient recovered well after complete resection of the tumor. Sigmoid IMT can occur in infancy. This eighth case is the youngest so far. The child did well after surgical resection.

Role of programmed cell death 4 in myofibroblast differentiation in oral submucous fibrosis.

BACKGROUND: Fibrosis is an uncontrolled healing process, led by persistent differentiation of fibroblast to alpha-smooth muscle actin (αSMA) … he normal mucosa and early OSMF, while advanced OSMF groups demonstrated a higher frequency of αSMA myofibroblasts. The PDCD4 expression was noted in the normal stromal fibroblasts. However, this expression appeared to progressively reduce with an increasing grade of OSMF. Thus, a negative correlation was noted between stromal PDCD4 and αSMA expression with prog …
