Intraluminal manometric studies of normal swallowing activity fail to distinguish between the peristaltic response of striated and smooth mu … or diminished smooth muscle peristalsis without altering striated muscle activity. The injection of neostigmine restored peristaltic amplitude toward normal in both striated and smooth muscle portions. These studies demonstrate that although the striated and smooth muscle segments are indistinguishable during normal oesophageal peristalsis, they do differ mar …

Epidural analgesia and spinal analgesia are the most effective techniques for relieving labour pain. Basically, local anaesthetics (i.e. bup … sic drugs are promising alternatives but are still at an experimental or very early clinical stage. Neostigmine and ketamine (without preservative) are not neurotoxic while midazolam neurotoxicity is still controversial. Intravenous remifentanil might prove useful when neuraxial analgesia is contraindicated.

BACKGROUND Congenital myasthenic syndrome (CMS) is a rare inherited neuromuscular disorder characterized by muscle weakness and fatigue, oft … . Initial treatment with oral pyridostigmine was ineffective, necessitating a switch to intravenous neostigmine, followed by continuous subcutaneous administration. This resulted in significant clinical improvement, including weaning off mechanical ventilation and achieving developmental milestones, with ongoing physiotherapy. CONCLUSIONS This case underscore …

OBJECTIVE: The purpose of this study was to evaluate the safety and efficacy of sugammadex for fast-track surgery in children undergoing cardiac surgery.
DESIGN: This was a prospective, randomized, controlled clinical study.
SETTING: University hospital.
PARTICIPANTS: The study comprised 60 children undergoing cardiac surgery.
INTERVENTIONS: The children in group S received sugammadex, 4 mg/kg, for reversal of neuromuscular block, and the children in group N received neostigmine, 30 µg/kg, and atropine, 15 µg/kg.
MEASUREMENTS AND MAIN RESULTS: The recovery time to a train-of-four of 0.9 a…

BACKGROUND: Approximately 50% of people with myasthenia gravis present with purely ocular symptoms, so called ocular myasthenia. Of these between 50% to 60% develop generalized disease, most within two years. Their management is controversial. This is an update of a review first published in 2006 and previously updated in 2008 and 2010.
OBJECTIVES: To assess the effects of treatments for ocular myasthenia and to answer three specific questions. Are there any treatments that impact the progression from ocular to generalized disease? Are there any treatments that improve symptoms of diplopia or

Neuromuscular junctional disorders (NMJ) in children are distinct entity. They may be acquired or hereditary. They pose problem in diagnosis because of the higher occurrence of sero negative Myasthenia Gravis (MG) cases in children. The identity of MusK antibody positivity in a good percentage of sero negative cases further adds to problems in diagnosis. The Congenital Myasthenic Syndrome (CMS) which are rare disorders of hereditary neuromuscular transmission (NMT) has to be differentiated because immunotherapy has no benefit in this group. Molecular genetic studies of these diseases helps to

A previously well infant developed severe muscle weakness and hypotonia at 6 months of age. This was reversed by anticholinesterase medication. However, she had subsequent further weakness and died at 10 months after an acute respiratory arrest. The clinical pattern was that of the 'juvenile' form of myasthenia gravis rather than the 'congenital' forms which have previously been described in early infancy.

Mary Broadfoot Walker (1888-1974) was the first to demonstrate the 'Mary Walker effect' describing the weakness of other muscle groups following release of the arteriovenous occlusion of an unrelated exercising muscle group in patients with myasthenia gravis, which led to the search for a circulating causative agent for myasthenia gravis. She was the first to clearly demonstrate that strength temporarily improved in patients with myasthenia gravis with physostigmine or Prostigmin (neostigmine). This dramatic treatment response has been erroneously termed the 'Mary Walker effect'. Further, she …

A comparative study of cholinergic and adrenergic mechanisms was performed on isolated circular muscle strips of the canine ileum, ileocolonic junction and colon. Cholinergic agonists evoked atropine sensitive contractions, which were most prominent in the colon. The presence of neostigmine to inhibit acetylcholinesterase did not alter the relative sensitivities of the tissues to acetylcholine. The different maximal contraction to cholinergic agents discriminates the colon from the ileum and the ileocolonic junction. Alpha adrenoceptor agonists produced greater contractions in the ileocolonic …

In order to study whether the antidiarrheal effects of loperamide are largely brought about by a local inhibition of the drug, we applied doses of 0.2 mg directly on the mucosa of the descending and sigmoid colon of 33 healthy volunteers. Loperamide decreased BER frequencies and reduced spike activity to 6.6 +/- 0.3% of time vs 11.3 +/- 0.7% in the control recorded from 3 cm aside and to 5.8 +/- 0.4% vs 12.7 +/- 0.9% in the descending and sigmoid colon, respectively. The same effects became apparent when spike activity was stimulated by neostigmine. Loperamide also reduced the occurrence of gr…
