Peripheral nerve regeneration using a keratin-based scaffold: long-term functional and histological outcomes in a mouse model.

PURPOSE: The management of peripheral nerve injuries with segmental defects is a challenge to both patient and surgeon. Repairs under tension have a poor prognosis; sensory nerve allografts have donor site morbidity and suboptimal motor recovery, but remain the gold standard. The development of conduit-based repair strategies has evolved and these are promising for sensory nerves and short defects; however, no conduit filler is clinically available that improves motor recovery equivalent to sensory autografts. In this study, motor recovery using keratin-based hydrogel filler was compared with

Differences in relative capacities of oxidative phosphorylation pathways may explain sex- and tissue-specific susceptibility to vision defects due to mitochondrial dysfunction.

Mitochondrial dysfunction is a major cause and/or contributor to the development and progression of vision defects in many ophthalmologic and mitochondrial diseases. Despite their mechanistic commonality, these diseases exhibit an impressive variety in sex- and tissue-specific penetrance, incidence, and severity. Currently, there is no functional explanation for these differences. We measured the function, relative capacities, and patterns of control of various oxidative phosphorylation pathways in the retina, the eyecup, the extraocular muscles, the optic nerve, and the sciatic nerve of adult

Glioneuronal Heterotopia Presenting as Cerebellopontine Angle Tumor of Cranial Nerve VIII.

BACKGROUND: Vestibular schwannomas and meningiomas account for the great majority of lesions arising in the cerebellopontine angle. In this report, we present a case of glioneuronal heterotopia, also known as glioneuronal hamartoma, arising from cranial nerve VIII, which is an extremely uncommon lesion. Important radiologic and surgical aspects are reviewed, which may help in early recognition and intraoperative decision making when these lesions are encountered. CASE DESCRIPTION: A healthy 29-year-old female presented with intermittent right facial numbness. Magnetic resonance imaging showed

Pinealectomy exaggerates and melatonin treatment suppresses neuroma formation of transected sciatic nerve in rats: gross morphological, histological and stereological analysis.

At present, an intensive effort for prevention of neuroma formation following peripheral nerve section continues. It has been recently suggested that surgical pinealectomy (Px) induces elevation of the collagen content in the granulation tissue of a wound, while melatonin application after Px suppresses elevation of the collagen accumulation in the tissue. The aim of the present study was to assess whether melatonin had the ability to suppress collagen production and neuroma formation following peripheral nerve transection. A total of 40 male rats (four groups of 10) were left intact (intact c

Pathogenesis and treatment of cubital tunnel syndrome caused by osteoarthrosis of the elbow joint.

We consider the primary cause of cubital tunnel syndrome caused by osteoarthrosis of the elbow joint to be degenerative osteophytes originating in the humeroulnar joint underneath the ulnar nerve. In the operative procedure the ulnar nerve in the cubital tunnel is fully exposed by cutting the soft tissues covering the nerve, because some of the soft tissues often seem to compress the nerve. Internal neurolysis is performed with microscopy in cases where the constriction is severe. Osteophytes in the humeroulnar joint underneath the nerve are removed. The osteophytes and loose bodies in the ant

Chemoreceptor properties of glomus tissue found in the carotid region of the cat.

1. ;Miniglomera' appearing as small masses of tissue with ample vascularization were found around the common carotid artery of the cat. Physiological, gross anatomical and electron microscopic studies were conducted on these tissues.2. The chemosensory function of each ;miniglomus' was evident from the behaviour of the afferent nerve fibres supplying the tissue: afferent responses became more active during asphyxia, when the blood flow through the tissue was reduced or blocked and when cyanide or ACh were applied. The afferent impulses became more infrequent during hyperventilation.3. Sensory

Isolated traumatic optic nerve avulsion in a boy who suffered a horse kick.

We report a case with traumatic avulsion of optic nerve caused by a horse kick in a child that caused loss of vision. Optic nerve avulsion is a rare injury usually associated with maxillofacial structures. Our patient had no light perception in the left eye. The left pupil was dilated and unreactive to light. The left globe was intact. A non-contrast axial computed tomography scan of the orbit demonstrated a widened and altered nerve globe junction with periorbital soft tissue edema. At six month following the injury, subsequent magnetic resonance imaging of the orbit showed that the preseptal

Use of cold-preserved allografts seeded with autologous Schwann cells in the treatment of a long-gap peripheral nerve injury.

BACKGROUND: Limitations in autogenous tissue have inspired the study of alternative materials for repair of complex peripheral nerve injurie … ted significant regeneration across 6-cm peripheral nerve defects. Use of cold-preserved allogeneic nerve tissue supplemented with autogenous Schwann cells poses a potentially safe and effective alternative to the use of autologous tissue in the reconstruction of extensive nerve injuries.

Clinical significance of imaging and histological characteristics of filum terminale in tethered cord syndrome.

OBJECT: The pathophysiology of tethered cord syndrome (TCS) is uncertain; however, it has been suggested that fibrous and fatty elements within the filum terminale (FT) play a role. The objective of this study was to describe the radiological and histological features of the FT in TCS and determine if there are associations between those features and clinical outcomes, complications, and urodynamics. METHODS: In this retrospective study, histological, MRI, and clinical data obtained in 293 patients with TCS who underwent FT transection were reviewed and analyzed in a multivariate analysis. R

Electrospinning porcine decellularized nerve matrix scaffold for peripheral nerve regeneration.

The composition and spatial structure of bioscaffold materials are essential for constructing tissue regeneration microenvironments. In this study, by using an electrospinning technique without any other additives, we successfully developed pure porcine decellularized nerve matrix (xDNME) conduits. The developed xDNME was composed of an obvious decellularized matrix fiber structure and effectively retained the natural components in the decellularized matrix of the nerve tissue. The xDNME conduit exhibited superior biocompatibility and the ability to overcome inter-species barriers. In vivo, af…

Menopause-Associated Anterior Cutaneous Nerve Entrapment Syndrome: A Case Highlighting Perineural Microenvironmental Pathology.

Anterior cutaneous nerve entrapment syndrome (ACNES) is an underrecognized cause of abdominal wall pain and is commonly diagnosed based on localized physical findings and response to local anesthetic nerve block. While this approach is clinically practical, it provides limited insight into the underlying pathophysiology and may contribute to diagnostic delay and repeated unnecessary investigations. ACNES represents a form of localized chronic compressive neuropathy characterized by perineural fibrosis and vascular involvement; however, little attention has been paid to systemic conditions that

Rapid continuous 3D printed multi-channel poly(ethylene glycol) diacrylate/chitosan nerve guidance conduit: In vivo study.

Digital light processing (DLP) 3D printing is a rapidly expanding technique in nerve tissue engineering, advancing towards fabricating nerve guidance conduits (NGCs). However, lack of bioink with printability and admirable biological features is the challenging engineering issue in this method. In this study, DLP based printing of multi-channel NGCs for peripheral nerve rehabilitation using polyethylene glycol diacrylate (PEGDA) hydrogel containing chitosan (0, 12.5, 25 and 50 wt%) biological cue is presented. Multi-channel NGCs with desired patterns are successfully printed and characterized.…

Developing Extracellular Matrix Technology to Treat Retinal or Optic Nerve Injury(1,2,3).

Adult mammalian CNS neurons often degenerate after injury, leading to lost neurologic functions. In the visual system, retinal or optic nerve injury often leads to retinal ganglion cell axon degeneration and irreversible vision loss. CNS axon degeneration is increasingly linked to the innate immune response to injury, which leads to tissue-destructive inflammation and scarring. Extracellular matrix (ECM) technology can reduce inflammation, while increasing functional tissue remodeling, over scarring, in various tissues and organs, including the peripheral nervous system. However, applying ECM

Mast cells alter granular properties and spatial relation to nerve fibres in spondylitis of adjuvant-treated rats.

It has been long implicated that mast cells (MCs) have a close spatial relationship to the peripheral nerve fibres. In the present study, which used spondylitis of adjuvant-treated rats, we investigated the behaviour of MCs in relation to peripheral nerve fibres and other inflammatory cells. Rat MCs with staining properties like connective tissue MCs decreased in number as inflammation progressed. With additional electron microscopic studies it was possible to observe the sequence of changes in their granular ultrastructure during active inflammation. Thus, the decrement of MCs with staining p

Fluorescence Imaging of Peripheral Nerves by a Nav1.7-Targeted Inhibitor Cystine Knot Peptide.

Twenty million Americans suffer from peripheral nerve injury caused by trauma and medical disorders, resulting in a broad spectrum of potentially debilitating side effects. In one out of four cases, patients identify surgery as the root cause of their nerve injury. Particularly during tumor resections or after traumatic injuries, tissue distortion and poor visibility can challenge a surgeon's ability to precisely locate and preserve peripheral nerves. Intuitively, surgical outcomes would improve tremendously if nerves could be highlighted using an exogeneous contrast agent. In clinical practic

Acute nerve grafting in traumatic injuries: two case studies.

Primary nerve grafting in traumatic injuries is rarely performed because of the uncertainty of the extent of injury, the limited availability of nerve grafts, and the damage to adjacent soft tissue. In this report the authors present two cases of acute nerve grafting after trauma-the first of the common peroneal nerve and the second of the ulnar nerve above the elbow-with sensory and motor recovery. Although compelling general arguments against primary posttraumatic nerve grafting exist, these cases illustrate that, in certain favorable and critical clinical situations, acute nerve grafting ma

Malignant Peripheral Nerve Sheath Tumor.

Malignant peripheral nerve sheath tumor (MPNST) is the sixth most common type of soft tissue sarcoma. Most MPNSTs arise in association with a peripheral nerve or preexisting neurofibroma. Neurofibromatosis type is the most important risk factor for MPNST. Tumor size and fludeoxyglucose F 18 avidity are among the most helpful parameters to distinguish MPNST from a benign peripheral nerve sheath tumor. The histopathologic diagnosis is predominantly a diagnosis of light microscopy. Immunohistochemical stains are most helpful to distinguish high-grade MPNST from its histologic mimics. Current surg

Neural outcomes after plasma knife dissection: a pathologic study and clinical correlation.

BACKGROUND: The initial aim was to determine the rate of pathologic tissue damage when dissecting the rat sciatic nerve with either bipolar forceps or low-temperature tripolar plasma knife. The second aim was to determine the safety and effectiveness of the plasma knife during parotid surgery. METHODS: A prospective, randomized, single-blind study was performed on 40 rat sciatic nerves dissected by either bipolar electrocautery with a cold knife or a plasma knife. Dissected nerves were excised and submitted blindly to pathology for analysis of nerve injury. The degree of nerve trauma was grad
