Treatment of Glossopharyngeal Neuralgia: A Systematic Review.

INTRODUCTION: Glossopharyngeal neuralgia (GPN) is a rare neuropathy characterized by paroxysmal, unilateral, and severe pain episodes affecting the throat, tonsils, base of the tongue, and pharynx. Its management remains challenging due to the absence of standardized treatment protocols and the limited availability of high-quality comparative studies. OBJECTIVE: To review and compare the effectiveness and safety of available treatments for GPN. METHODS: A systematic review was conducted, analyzing studies published in the last decade that evaluated GPN treatments. Searches were performed in …

Pudendal Neuralgia: A Review of the Current Literature.

PURPOSE OF REVIEW: This paper aims to review pudendal neuralgia pathophysiology, risk factors, diagnosis, and treatment options. RECENT FINDINGS: Conservative and pharmacologic options are first line treatments for the treatment of pudendal neuralgia. Interventional treatment such as, pudendal nerve blocks can be tried if first line treatments feel to provide adequate analgesia. If pudendal nerve blocks provide sufficient relief but have a short duration, decompressive surgery may be considered. Neuromodulation is also a viable option. Emerging techniques such as pulsed radiofrequency ablatio…

[Localization of neurovascular conflict in trigeminal neuralgia].

BACKGROUND: The cause of trigeminal neuralgia (TN) is vascular compression of the trigeminal root. The last one is also found on intact side and in people without TN. Magnetic resonance imaging (MRI) reveals vascular compression, root deformity and atrophy, as well as localization of neurovascular conflict. To assess significance of MR signs of vascular compression, we compared appropriate data with intraoperative findings. MATERIAL AND METHODS: Preoperative MRI-based measurement of length (A), width (B) of trigeminal root and distance between brainstem and compression point (C) was performed…

[Prevalences of Trigeminal Neuralgia and Hemifacial Spasm].

Neurosurgeons must have knowledge about the epidemiology of trigeminal neuralgia and facial spasm. The annual incidence of trigeminal neuralgia is 4.3-28.9 per 100,000 persons, with a prevalence of 76.8 per 100,000 persons, increasing with age. It is more common in women and on the right side, with SCA being the most common causative vessel. The long-term efficacy of MVD for trigeminal neuralgia is 80% with complete resolution of pain and 5.2% with complications, which is safe and highly effective when performed by an expert surgeon. Hemifacial spasm has an annual incidence of 0.78/100,000 wit…

The Risk of Trigeminal Neuralgia Following Osteoporosis.

Background and objectives: Managing people with trigeminal neuralgia (TN) and osteoporosis is challenging due to their debilitating conditions. Currently, the exact association between TN and osteoporosis in patients remains unknown, although there is potential overlapping of pathophysiological mechanisms. In response, we calculated TN risk in patients who have osteoporosis. Materials and Methods: 45,393 patients aged over 50 years diagnosed with osteoporosis were matched with 45,393 non-osteoporosis patients aged over 50 years (1:1 ratio) who were used as the control group, using data from 19…

The Molecular Basis and Pathophysiology of Trigeminal Neuralgia.

Trigeminal neuralgia (TN) is a complex orofacial pain syndrome characterized by the paroxysmal onset of pain attacks in the trigeminal distribution. The underlying mechanism for this debilitating condition is still not clearly understood. Decades of basic and clinical evidence support the demyelination hypothesis, where demyelination along the trigeminal afferent pathway is a major driver for TN pathogenesis and pathophysiology. Such pathological demyelination can be triggered by physical compression of the trigeminal ganglion or another primary demyelinating disease, such as multiple sclerosi…

Trigeminal neuralgia and genetics: A systematic review.

Trigeminal neuralgia (TN) is a severe facial pain disease of unknown cause and unclear genetic background. To examine the existing knowledge about genetics in TN, we performed a systematic study asking about the prevalence of familial trigeminal neuralgia, and which genes that have been identified in human TN studies and in animal models of trigeminal pain. MedLine, Embase, Cochrane Library and Web of Science were searched from inception to January 2021. 71 studies were included in the systematic review. Currently, few studies provide information about the prevalence of familial TN; the availa…

Radiofrequency thermocoagulation for the treatment of trigeminal neuralgia.

Although microvascular decompression (MVD) should be considered as the first-line treatment for classic trigeminal neuralgia (TN) owing to neurovascular compression of the trigeminal nerve, an increasing number of surgeons prefer radiofrequency thermocoagulation (RFT). RFT is a Gasserian ganglion-level ablative intervention that may achieve immediate pain relief for TN. It is used for emergency management when MVD is not suitable for the patient. As the gold surgical standard of classic trigeminal neuralgia, MVD has the advantage of longer efficacy. However, there are currently no high-quality…

Radiosurgery for trigeminal neuralgia: the state of art.

The trigeminal neuralgia (TN) is a chronic, episodic, and disabling facial pain syndrome. It is a relatively rare disorder. Despite this, because of the intensity of the pain, TN may have a dramatic impact for many patients. Fortunately, most of the patients may obtain a good pain relief simply by taking drugs. Historically, the surgical approaches represent a true treatment paradigm for all the drug-resistant TN. In this context, radiosurgery (RS) is a relatively new treatment modality. The effectiveness and safety of radiosurgery are today widely accepted and the technique can be proposed to…
