[Pronator teres syndrome. Clinical and neurophysiological study of 2 cases].

We report two cases of pronator teres syndrome (bilateral in one case) diagnosed in our Department by electromyographic studies during the last three years. The infrequency of this syndrome (1 case/201 cases of carpal tunnel syndrome), combined with the unspecified and inconsistent symptomatology (tenderness over the pronator teres, paresthesias and weakness of flexor pollicis longus, abductor pollicis brevis and opponens pollicis) frequently induced diagnostic mistakes. The electromyographic study is very useful showing a delay in proximal motor latency of the median nerve, diminution of moto

The neurophysiological investigation of Bell's palsy and the predictive value of the blink reflex.

Forty-nine patients with Bell's palsy were studied neurophysiologically within 10 days of the onset of symptoms and the results analysed according to the clinical outcome. Electroneuronography was not helpful in predicting outcome at this stage. The presence of a blink reflex was a good prognostic sign and all patients with an intact blink reflex recovered well.

Neurohypophysial hormones and brain function: the neurophysiological effects of oxytocin and vasopressin.

An increasing body of evidence suggests that the neurohypophysial hormones, in addition to their classical actions, may also act as neurotransmitters. They have a widespread but discontinuous distribution in the CNS; apart from their presence in the magnocellular nuclei they may be found in the hippocampus, amygdala, septum, substantia nigra, brainstem and spinal cord. They exert profound effects on behavior, particularly on memory, a function frequently ascribed to the hippocampus, amygdala and septum; on memory consolidation, internal reward and self stimulation functions frequently ascribed

The neurophysiological basis of bladder inhibition in response to intravaginal electrical stimulation.

Intravaginal electrical stimulation (IVS) induces a profound bladder inhibition and is successful in the treatment of incontinence due to detrusor instability. In this experimental study in cats, direct recordings of the efferent activity in thin hypogastric and pelvic nerve filaments to the bladder were used to analyze the neuronal mechanisms underlying this bladder inhibition. A longlasting reflex discharge, with a latency of 35 to 50 msec., was evoked in the hypogastric nerve by IVS. The reflex discharge was unaffected by imposed changes in intravesical pressure or by micturition contractio

Neurophysiological approaches to chronic pain following spinal cord injury.

Pain occurring in patients with spinal cord injury can be classified on clinical grounds into five types: peripheral, central, visceral, mechanical and psychic. An attempt has been made to correlate each type of pain with present neurophysiological knowledge. Mechanisms as to how unpleasant sensations reach the conscious level can be deduced when clinical and neurophysiological data are pooled. Eight case histories are presented which typify each class. The authors' evaluation and treatment offered is presented for each type.

[Disseminated sclerosis: diagnostic value of neurophysiological investigations (author's transl)].

Demyelinisation lesions of disseminated sclerosis actually present within the white matter are much more numerous than clinical manifestations would suggest that the majority of these lesions are clinically latent. Ophtalmologists were the first to seek a functional deficit reflecting the demyelinisation which so often affects the optic pathways. In practice, the recording of evoked visual potentials has proved to be the most reliable method. The brain stem is another region where demyelinisation lesions are frequent and quite often latent. Electronystagmography offers a highly sensitive means

Early neurophysiological assessment after insult to the central nervous system.

Cerebral insult is a generic term which includes many causes of altered cerebral function. Neurophysiological investigations, closely integrated with clinical and metabolic studies, should begin at an early stage after any such insult (namely in the first few hours) to provide information on the type, severity and evolution of latered brain function. The present contribution is based on prospective studies on 200 children with closed head injuries and 500 children who had been resuscitated following cardiocirculatory or respiratory arrest. In addition to electroencephalography, other neurophys

Intraoperative Neurophysiological Monitoring During Spinal Cord Tumor Surgery.

Intramedullary spinal cord tumor surgery has evolved through major technological advances. From early attempts in the 19th century to modern methods, innovations like the operative microscope, bipolar coagulation, and MRI have transformed practice. The introduction in the 1980s of intraoperative neurophysiological monitoring enabled real-time spinal cord functions assessment through the application of monitoring techniques. In this article, we will review the state of the art of the different techniques, the alarm criteria and related recovery strategies, current controversies and future consi

Acute response and neurophysiological mechanisms of mindfulness meditation in emotional eating.

INTRODUCTION: Emotional eating often leads to adverse health outcomes. Understanding how mindfulness influences emotional eating is crucial for developing strategies to promote healthier eating behaviors and well-being. This study aims to investigate the mechanism and acute response to a mindfulness meditation practice on emotional eating by examining immediate post-intervention effects on brain activity and neurophysiological differences between individuals with and without emotional eating. METHODS: Forty-nine adults were recruited and allocated into two groups based on the emotional eating

Neurophysiological Contributions to Dysphonia: The Role of Cortical Silent Periods in AdLD.

OBJECTIVE: Adductor laryngeal dystonia (AdLD) is a task-specific focal dystonia characterized by involuntary vocal fold hyperadduction and impaired voice quality. While reduced cortical inhibition is a hallmark of AdLD, its relationship to acoustic measures of voice dysfunction remains unclear. This study investigated whether intracortical inhibition, as measured by the cortical silent period (cSP) from the laryngeal motor cortex (LMC), is associated with cepstral peak prominence (CPP), a quantitative acoustic marker of voice harmonic periodicity. METHODS: Thirty-two participants (17 AdLD, 15

The Modulatory Effects of Anesthetics and Analgesics on Neurophysiological Monitoring and Underlying Mechanisms.

Intraoperative Neurophysiological Monitoring (IONM) is an indispensable surgical tool that offers invaluable insights into neurological function across a spectrum of anatomical areas. By comprehensively assessing the integrity of the brain, brainstem, spinal cord, cranial nerves, and peripheral nerves, IONM plays a pivotal role in guiding surgical decision-making and optimizing patient outcomes, particularly in the context of high-risk procedures. Intraoperative drugs, especially anesthetics and/or analgesics, differentially modulate neurophysiological monitoring, which remarkably affects the

Neurophysiological, imaging and neurobiological markers of central fatigue in multiple sclerosis.

Central fatigue affects 80% of patients with multiple sclerosis, with 60% of them claiming it as the most disabling symptom. Current research often independently explores neurophysiological, structural, or functional imaging and biological underpinnings of fatigue, thus lacking a multidimensional perspective. Here, we used a multidimensional approach to investigate the functional, structural and biological underpinnings of fatigue in MS and to assess the relative contribution of each factor. A cross-sectional study was conducted with 41 patients with relapsing-remitting multiple sclerosis and

Neurophysiological evidence of single-shot semantic mapping in the developing brain.

Rapid acquisition of new words and construction of large vocabularies is a unique capacity of developing human brain. This process is to a large degree mediated by a neurocognitive mechanism known as «fast mapping» (FM) which allows the child to quickly map new words onto neural representations after even a single exposure to them, using context-driven inference. However, the neurophysiological bases of this mechanism are still poorly understood. To address this open question, we used event-related potentials (ERPs) to investigate brain dynamics elicited by novel words following a single-shot

Methotrexate-induced neurotoxicity: Diagnostic challenges and the role of neurophysiological testing.

Methotrexate (MTX) is widely used in the treatment of acute lymphocytic leukemia (ALL) and other onco-hematological conditions. Although subacute MTX-related neurotoxicity is relatively rare, it can present with dramatic, fluctuating neurological deficits that mimic other serious conditions. Diagnosis may be complicated by the frequent presence of asymptomatic white matter abnormalities on MRI, commonly attributed to chronic MTX toxicity. We report an 18-year-old ALL patient who developed severe, fluctuating neurological symptoms 11 days after the third intrathecal administration of MTX. MRI s
