Prolonged intrauterine transabdominal ventricular external drainage. A method to decompress dilated fetal ventricles.

Advances in diagnostic procedures to detect intrauterine hydrocephalus have compelled neurosurgeons to intervene in order to preserve precious neural tissue. We present the first case (to the best of our knowledge) of a successful decrease in intracranial pressure and head circumference by repeated transabdominal intrauterine external drainage. The mother tolerated the procedure well. No complication occurred from the procedure. This method may temporarily decrease the intracranial pressure and head circumference in utero, preserve neural tissue and facilitate normal vaginal delivery. The lite…

The management of orbital roof fractures.

The nature and aetiology of orbital roof fractures is discussed and a strategy for their successful management outlined with reference to five clinical cases. Early recognition and treatment of these fractures reduces the high incidence of intra-cranial and ocular complications and the importance of a combined approach involving maxillofacial surgeons, neurosurgeons and ophthalmologists is emphasised. A coronal flap together with frontal craniotomy is recommended for surgical access to the orbital roof and overlying dura. When reconstruction is indicated, autogenous bone harvested from the inn…

Pediatric posterior fossa tumors: hazards of the "preoperative" shunt.

Many neurosurgeons recommend a "preoperative" shunt for children with posterior fossa tumors. It has been reported that the definitive surgery is simplified and the postoperative course is more benign as a result of the cerebrospinal fluid diversion. The present report documents complications as a result of upward herniation and hemorrhage within the tumor after shunt placement. On the basis of this experience, we conclude that a shunt is potentially hazardous and should be restricted to that selected group of patients who are acutely ill from increased intracranial pressure that is refractory…

[Fracture luxation of the cervical spine. Different treatments].

There exist situations, such as cervical spinal cord injury which are treated by orthopedic specialists and/or neurosurgeons, with dissimilar methods. In is necessary to unify criteria or better still not commit mistakes which may worsen neurological déficit in these injured patients. The medical history and treatment of 16 patients with cervical spinal cord injury are discussed. The therapeutic procedures included: decompressive laminectomy, anterior fibula bone graft, Clowards' procedure and (minerva) exterior neck fixation. The evolution and complications are analyzed and in some cases conn…

Comparing the ergonomic aspects of the exoscope and microscope in neurosurgery using motion sensors.

BACKGROUND: Ergonomic improvement for neurosurgeons is essential in reducing musculoskeletal disorders, particularly during prolonged procedures. Traditional microscopes often limit the range of motion, forcing surgeons into uncomfortable positions, particularly in cases involving angulated or narrow anatomy. RESEARCH QUESTION: This study compares neurosurgeons' posture and handling of the exoscope versus the microscope evaluating its ergonomic aspects. METHODS: 26 cases were operated using a microscope (Zeiss, Jena, Germany) and 29 using an exoscope (Olympus, Tokyo, Japan). Data included ta…

The untapped potential of 3D virtualization using high resolution scanner-based and photogrammetry technologies for bone bank digital modeling.

Three-dimensional (3D) scanning technologies could transform medical practices by creating virtual tissue banks. In bone transplantation, ne … sed on CIEDE2000 was performed, and the results were compared with questionnaires distributed among neurosurgeons. The findings indicate that certain 3D scanning methods were more appropriate for specific bones. Among the technologies, SLG emerged as optimal for tissue banking, offering a superior balance of accuracy, minimal distortion, cost-efficiency, and ea …

White Cord Syndrome After Cervical or Thoracic Spinal Cord Decompression. Hemodynamic Complication or Mechanical Damage? An Underestimated Nosographic Entity.

The ischemia/reperfusion mechanism is believed to be responsible for parenchymal damage caused by temporary hypoperfusion and worsened by the subsequent attempt of reperfusion. This represents a true challenge for physicians of several fields, including neurosurgeons. A limited number of papers have shed the light on a rare pathologic condition that affects patients experiencing an unexplained neurologic deficit after spine surgery, the so-called "white cord syndrome." This entity is believed to be caused by an "ischemia/reperfusion" injury on the spinal cord, documented by a postoperative int…

Pediatric and Adult Low-Grade Gliomas: Where Do the Differences Lie?

Two thirds of pediatric gliomas are classified as low-grade (LGG), while in adults only around 20% of gliomas are low-grade. However, these … d in the infratentorial compartment. The different tumor locations require different skillsets from neurosurgeons. In adult LGG, a common practice is awake surgery, which is rarely performed on children. On the other hand, pediatric neurosurgeons are more commonly confronted with infratentorial tumors causing hydrocephalus, which more often require endoscopic o …

CSF disturbances and other neurosurgical complications after interdisciplinary reconstructions of large combined scalp and skull deficiencies.

Combined scalp and skull deficiency due to malignant scalp tumors or sequelae of intracranial surgery present challenging entities for both neurosurgeons and reconstructive treatment. In complex cases, an interdisciplinary approach is needed between neurosurgeons and cranio-maxillofacial surgeons. We present a considerably large series for which we identify typical complications and pitfalls and provide evidence for the importance of an interdisciplinary algorithm for chronic wound healing complications and malignomas of the scalp and skull. We retrospectively reviewed all patients treated by …

How to radiologically identify a spontaneous regression of sporadic vestibular schwannoma?

BACKGROUND: The natural history of sporadic vestibular schwannoma is unpredictable, with tumors growing, non-growing and even showing sponta … re associated in 64% of cases. CONCLUSION: These two newly reported radiologic features could help neurosurgeons, oto-neurosurgeons and neuroradiologists to identify a spontaneous vestibular schwannoma involution at first visit. This could allow any treatment to be postponed, monitoring to be more widely spaced, and patients to be reassured.

Association of decision-making in spinal surgery with specialty and emotional involvement-the Indications in Spinal Surgery (INDIANA) survey.

BACKGROUND: Although recent trials provided level I evidence for the most common degenerative lumbar spinal disorders, treatment still varies widely. Thus, the Indications in Spinal Surgery (INDIANA) survey explores whether decision-making is influenced by specialty or personal emotional involvement of the treating specialist. METHOD: Nationwide, neurosurgeons and orthopedic surgeons specialized in spine surgery were asked to answer an Internet-based questionnaire with typical clinical patient cases of lumbar disc herniation (DH), lumbar spinal stenosis (SS), and lumbar degenerative spondylol…

Endoscopic endonasal surgery for pituitary adenomas.

BACKGROUND: Pituitary surgery is a continuous evolving specialty of the neurosurgeons' armamentarium, which requires precise anatomic knowledge, technical skills, and integrated culture of the pituitary pathophysiology. Actually it cannot be considered only from a technical standpoint, but rather a procedure resulting from the close cooperation among different specialists (e.g., ophthalmologists, neuroradiologists, endocrinologists, neurosurgeons, otorhinolaryngologists, anesthesiologists, neurophysiologists, pathologists, instrument manufacturers). METHODS: The "pure" endoscopic endonsal sur…
