A low risk of nosocomial transmission of subclinical tuberculosis to neonates in a postpartum care center under COVID-19 control measures.

We report the results of investigating and managing a tuberculosis (TB) exposure in apostpartum care center. Among the contacts exposed to a nursing assistant with subclinical TB,5 of 44 neonates (11.4%) had positive tuberculin skin tests (TSTs) at 3 months of age, and all theTST-positive neonates received the Bacille Calmette-Guérin vaccination. Seven of 28 healthcareworkers (25.0%) and 1 of 3 household contacts (33.3%) were positive in the initial or repeatedinterferon-gamma release assay. None of the contacts developed TB disease during the studyperiod. Annual TB examinations of healthcare

Pregnancy and the Opioid Epidemic.

Opioid use disorder (OUD) in pregnancy is increasing, which often results in poor maternal and neonatal outcomes including neonatal abstinence syndrome (NAS) as a result of lack of prenatal care and inadequate substance use disorder management. Practice guidelines have been developed to manage OUD during and after pregnancy for mother and baby, but barriers exist, limiting comprehensive implementation. To reduce the impact of OUD in pregnancy and associated maternal and neonatal sequela, implementing compassionate evidence-based care and a non-punitive response is needed. A stigma-free approac

Water and heat--the priority for the newborn infant.

Maintaining fluid and heat balance is of vital importance to the newborn infant. At birth, the infant is exposed to a cold and dry environment, and preterm neonates in particular, are then at risk of dehydration and hypothermia. These conditions may have serious consequences and significantly influence mortality and morbidity. Preterm neonates have a high rate of water and heat loss mainly as a consequence of their immature skin. The care environment influences the magnitude of water and heat exchange and needs to be individually tailored on the basis of the infant's clinical status, maturity

The Use of Whole Body Cooling in the Treatment of Hypoxic-Ischemic Encephalopathy.

Hypoxic-ischemic encephalopathy (HIE) is a major cause of morbidity and mortality in neonates. Hypoxic-ischemic encephalopathy occurs as a result of a perinatal hypoxic-ischemic event just prior to or during delivery. Therapeutic hypothermia using whole body cooling is the current treatment of choice to reduce brain injury and improve long-term neurodevelopmental outcomes for neonates with HIE. All English language articles published since 2005 in PubMed and the Cumulative Index to Nursing and Allied Health Literature (CINAHL) were analyzed for existing evidence-based methods for whole body co

Occurrence and prediction of clinical interventions during transfer of near-term and term infants with respiratory distress on CPAP. An observational study.

BACKGROUND: Neonatal respiratory distress (NRD) requiring continuous positive airway pressure (CPAP) is a common indication for inter-hospital transfer of late preterm and term neonates. The optimal composition of the inter-hospital team transfer - involving an advanced neonatal nurse and an ambulance driver or a complete team, which also includes a paediatrician - remains uncertain. Particularly when clinical interventions are required. OBJECTIVES: To assess the occurrence of clinical interventions necessitating a complete transport team during the inter-hospital transfer of neonates with NR

Pharmaceutical preparations. A review of drugs commonly used during the neonatal period.

Certified nurse-midwives, whose responsibility includes care of the newborn in the first days of life, should be well versed in the commonly used pharmaceutical preparations in the neonatal period. This article reviews therapeutic uses and the pharmacodynamics of vitamin K, as well as the neonatal eye preparations for prophylaxis of infections (silver nitrate, tetracycline, and erythromycin ophthalmic ointments). Preparations used in caring for the umbilical cord, as well as the commonly prescribed antibiotics ampicillin and gentamicin, are discussed. The narcotic antagonist naloxone is also r

Surgical ligation of patent ductus arteriosus in a neonatal intensive care setting is safe and cost effective.

The physiological and biochemical status of two groups of neonates with patent ductus arteriosus (PDA) requiring surgical ligation were compared. One group (n = 14) had ductus ligation in the operating room (OR) and the other group (n = 14) had the same operation in an isolation room in the Neonatal Intensive Care Unit (NICU). The groups were closely matched in terms of gestational age and weight. Nursing time and disposable equipment savings were significantly different. We have confirmed that PDA ligation can be done safely in the NICU and is more cost efficient than ligation in the OR.

Induction of transient hyperprolactinaemia in neonatal rats by direct or maternal treatment with the dopamine receptor blocker, sulpiride.

Prolactin was measured in the plasma of neonatal rats after iv and ip injection of the dopamine receptor blocking drug sulpiride, and after its ip injection to neonatal rats' nursing mothers. The sulpiride dose PRL response relationship in 10-25 day old neonatal rats was similar to that found in lactating rats, with a threshold sensitivity around 29 nmol sulpiride/kg body weight and a maximal response at about 2.9 mumol/kg. Absolute levels of PRL in the neonate (both peak and increment-over-basal) were, however, 90% lower than in adults. Treatment of lactating mothers with a maximally stimulat

Alagille Syndrome: An Overview.

Alagille syndrome (AGS) is a highly complex, multisystem, autosomal dominant disorder that is caused by a defect in the Notch signaling pathway. This syndrome mainly affects the liver, causing significant cholestasis, which is caused by a paucity of intrahepatic bile ducts. There can be cardiac involvement, including, but not limited to, pulmonary stenosis and tetralogy of Fallot. Patients can also present with butterfly vertebra, ocular issues, and vascular events. Because this syndrome follows an autosomal dominant inheritance, it can have variable expression even in the same family line. Fo

Infant crying: nature, physiologic consequences, and select interventions.

This article describes the nature of infant crying, the physiologic events and changes associated with it, and appropriate nursing interventions for infant crying. A cry is a series of four movements that basically resembles a Valsalva maneuver. Documented immediate and long-term sequelae of crying include increased heart rate and blood pressure, reduced oxygen level, elevated cerebral blood pressure, initiation of the stress response, depleted energy reserves and oxygen, interrupted mother-infant interaction, brain injury, and cardiac dysfunction. Caregivers are encouraged to answer infant cr

Acyanotic lesions with increased pulmonary blood flow.

The neonatal nurse is a vital link in the recognition and management of infants born with congenital heart disease. This article examines the acyanotic cardiac defects with increased pulmonary blood flow: patent ductus arteriosus, atrial septal defect, ventricular septal defect, and endocardial cushion defects. The hemodynamics, clinical presentation, differential diagnosis, surgical repair, and medical management of each defect are presented. Parental teaching needs are identified and discussed.

Challenges in nursing: infants with congenital diaphragmatic hernia.

Despite advances in the care of infants born with congenital diaphragmatic hernia, mortality rate continues to be high. Immediate survival is directly related to the degree of pulmonary hypoplasia present. The spectrum is wide, from minimal pulmonary hypoplasia, in which neonates do well, to severe pulmonary hypoplasia, which is incompatible with life. Between these two extremes lie infants with compromised pulmonary function whose long-term survival depends on the clinical strengths of their multidisciplinary team. Over the past year, 23 infants with congenital diaphragmatic hernia were cared

Recognition, understanding, and current management of cardiac lesions with decreased pulmonary blood flow.

Congenital heart lesions can be classified according to hemodynamic characteristics related to pulmonary blood flow patterns: increased, normal, or decreased, lesions with decreased pulmonary blood flow characteristically present with cyanosis. This article briefly reviews differential diagnoses for the presenting sign of cyanosis. Additionally, this article provides a comprehensive overview of cardiac physiology of lesions with decreased pulmonary blood flow. This information provides a foundation of knowledge for the recognition of acute symptoms in the newborn period and an understanding of

Enhancing perinatal care in Rwanda: building quality improvement into system strengthening.

Rwanda has achieved substantial gains in maternal, newborn and child health over the last two decades. But reduction in neonatal mortality has slowed, with most deaths now occurring in hospitals and health centres. Mortality is mainly caused by prematurity, perinatal asphyxia, infection and gaps in quality of care. From 2017 to 2023, the Rwanda Pediatric Association and the Royal College of Paediatrics and Child Health Global, in partnership with UNICEF, the Ministry of Health and Rwanda Biomedical Center, implemented a multiphase programme across 21 hospitals designed to enhance perinatal car
