Is the convergence insufficiency symptom survey applicable to elderly individuals?

BACKGROUND: The psychometric properties of the Convergence Insufficiency Symptom Survey (CISS) have been previously determined across the younger adult population. This study investigated the psychometric properties of the CISS in presbyopic adults via classical and Rasch analysis. METHODS: A total of 100 presbyopic individuals (40-60 years) were selected with far and near acuity of 20/20 with their habitual spectacles; 50 had convergence insufficiency and 50 had normal binocular vision. Refraction, cover test, near point of convergence, and positive fusional ranges were examined. The Persian

Oculomotor Abnormalities in a Sheep (Ovis aries) Model of Huntington's Disease: Towards a Biomarker for Assessing Therapeutic Efficacy.

BACKGROUND: Huntington's disease (HD) is characterized by a loss of control of motor function that causes the presence of abnormal eye movements at early stages. OBJECTIVE: To determine if, compared to normal sheep, HD sheep have abnormal eye movements. METHODS: We measured eye movements in a transgenic sheep (Ovis aries) model of HD using a purpose-built, head-mounted sheep oculometer. This allows us to measure saccades without the need for either behavioral training or head fixation. At the age of testing (6 years old), the HD sheep were pre-manifest. We used 21 sheep (11 HD, 10 normal).

Treatment of symptomatic convergence insufficiency with home-based computerized vergence system therapy in children.

PURPOSE: To evaluate the efficacy of a home-based computer orthoptic program for symptomatic convergence insufficiency (CI) in children. METHODS: The medical records of participants aged 5 to <18 years who were diagnosed with symptomatic CI and were treated with the Computerized Vergence System (CVS) program were retrospectively reviewed. All participants were prescribed 9 or 15 minutes of daily convergence exercises with the CVS program, 5 days per week, for the initial 6 weeks. Near point of convergence (NPC) and near convergence amplitude (NCA) were measured at baseline, 6 week, and final

Near point of convergence norms measured in elementary school children.

PURPOSE: Convergence insufficiency (CI) is the most prevalent of binocular dysfunctions. A prime finding in the diagnosis of CI is the near point of convergence (NPC). METHODS: The NPC was measured six different times on the same 539 children, once in the fall and once in the spring, over the course of 3 years. At each evaluation, the NPC was performed three consecutive times. An accommodative target was used in the testing and the breaks and recoveries were carefully measured and recorded to the nearest centimeter. Normative data for the break and recovery finding were calculated. RESULTS:

[On diagnosis of internuclear ophthalmoplegia].

Optokinetic nystagmus in panoramic mono- and binocular optokinetic stimulation was studied in 24 patients with multiple sclerosis and 20 normal subjects. In control group, asymmetry coefficients and slow phase speed were less than 6% and 7.7% respectively. In the patients, these indices were 7.1 and 14% for binocular optokinetic stimulation and 14.6 and 21.7% for monocular one. Using monocular optokinetic stimulation, electronistagmographic signs of internuclear ophthalmoplegia were detected in all the patients. Therefore, panoramic monocular optokinetic stimulation facilitates determination o

Dissociated vertical divergence: a righting reflex gone wrong.

Dissociated vertical divergence (DVD) is an ocular motor disorder characterized by a slow, upward drift of 1 eye when the other eye is fixating a target. I propose that DVD is a dorsal light reflex in which asymmetrical visual input to the 2 eyes evokes a vertical divergence movement of the eyes. This primitive visuo-vestibular reflex functions as a righting response to restore vertical orientation in lower lateral-eyed animals by equalizing binocular visual input. The dorsal light reflex is suppressed in humans but can manifest as DVD when early-onset strabismus precludes normal binocular dev

Radical resection of intracavernous chondrosarcoma using an intradural keyhole approach: how I do it.

BACKGROUND: Skull base chondrosarcoma is rare, arising from the clivus or petroclival junction, and usually presents as ocular motility disorders. Endonasal transsphenoidal surgery may be performed in some cases of midline clivus chondrosarcoma. Chondrosarcoma is located within the cavernous sinus and presents a softer/gelatinous mass and can be removed with suctions and curettage. We have been using a simple intradural keyhole transcavernous approach, avoiding a complex extradural transcavernous dissection. METHODS: The intracavernous chondrosarcoma was removed via a 5 mm keyhole opening ove…

Advances in the diagnosis and treatment of acute acquired comitant esotropia.

Acute acquired comitant esotropia (AACE) is mainly characterized by sudden onset, accompanied by diplopia, without extraocular muscles paralysis or ocular motility disorders. In recent years, the incidence of AACE has been increasing, researchers have found that this phenomenon may be related to the widespread use of electronic devices and the increase in the number of people working from home during the COVID-19 pandemic. However, its neural mechanisms have not been fully elucidated. This article primarily reviews the latest developments in the diagnosis and treatment of AACE from the perspec…

Ocular manifestations of Apert and Crouzon syndromes: qualitative and quantitative findings.

There are significant differences in the ocular manifestations of Apert and Crouzon syndromes. Here, we present qualitative and quantitative data about the oculo-orbital region to demonstrate these differences. Although ocular protosis and hypertelorism characterize both disorders, the nature of the orbital dystopia differs. In Crouzon syndrome, ocular proptosis is primarily caused by retrusion of the lateral and inferior orbital margins with a very short orbital floor. In Apert syndrome, the eyeglobe actually protrudes in relation to the cranial base and to the orbit, probably resulting from

Effects of Deep Brain Stimulation on Eye Movements and Vestibular Function.

Discovery of inter-latching circuits in the basal ganglia and invention of deep brain stimulation (DBS) for their modulation is a breakthrough in basic and clinical neuroscience. The DBS not only changes the quality of life of hundreds of thousands of people with intractable movement disorders, but it also offers a unique opportunity to understand how the basal ganglia interacts with other neural structures. An attractive yet less explored area is the study of DBS on eye movements and vestibular function. From the clinical perspective such studies provide valuable guidance in efficient program

Post-concussion clinical findings of oculomotor function in paediatric patients with persisting symptoms compared to healthy controls.

OBJECTIVE: Oculomotor deficits in vergence and accommodation can arise in paediatric patients with persistent concussion symptoms, although the profile is not well established. This study aimed to describe the frequency of these deficits in persistently symptomatic concussed paediatric patients and identify effective screening tools. METHODS: This was a prospective cohort study conducted at three clinical sites across the United States. Participants aged 8-18 years with diagnosed concussion were recruited within 9 months of injury through concussion clinics or referral to a vision provider. P

Relationship between superior longitudinal fasciculus injury and visual pursuit in patients with impaired consciousness following hypoxic-ischemic brain injury.

OBJECTIVE: Visual pursuit disturbance, a common and early indicator of recovery in disorders of consciousness, may be associated with superior longitudinal fasciculus (SLF) injury. By using diffusion tensor tractography (DTT), we investigated the relationship between visual pursuit disturbance and injury of the SLF in patients with impaired consciousness following hypoxic-ischemic brain injury (HI-BI). METHODS: Twenty patients with impaired consciousness and 11 controls were recruited. All patients were divided into two groups according to visual function scale on the Coma Recovery Scale-Revi

Congenital cranial dysinnervation disorders: a concept in evolution.

PURPOSE OF REVIEW: We review the congenital and genetic diagnoses that are currently included in the congenital cranial dysinnervation disorders (CCDDs). RECENT FINDINGS: Recent literature contains new genotypic and phenotypic descriptions of Duane retraction syndrome, Moebius syndrome, and other CCDDs. New genes which when mutated can result in CCDD have been identified, permitting a better understanding of associated phenotypes. More information is available regarding neurodevelopmental and clinical effects of various gene mutations associated with individual CCDDs. For certain CCDDs, the p

Early onset horizontal gaze palsy and progressive scoliosis due to a noncanonical splicing-site variant and a missense variant in the ROBO3 gene.

BACKGROUND: Homozygous or compound heterozygous ROBO3 gene mutations cause horizontal gaze palsy with progressive scoliosis (HGPPS). This is an autosomal recessive disorder that is characterized by congenital absence or severe restriction of horizontal gaze and progressive scoliosis. To date, almost 100 patients with HGPPS have been reported and 55 ROBO3 mutations have been identified. METHODS: We described an HGPPS patient and performed whole-exome sequencing (WES) to identify the causative gene. RESULTS: We identified a missense variant and a splice-site variant in the ROBO3 gene in the pr

Bilateral Internuclear Ophthalmoplegia Caused by Dengue Fever.

A 22-year-old male was referred from the medicine department with the chief complaint of double vision for 2 months. The patient had suffered from dengue viral hemorrhagic encephalitis for 2 months for which intensive medical care was given. On ocular examination, both eyes (OU) showed limited adduction with contralateral abducting nystagmus on attempted horizontal gaze. Magnetic resonance imaging of brain showed pontine and midbrain hemorrhages which involved the region of medial longitudinal fasciculus and caused bilateral internuclear ophthalmoplegia (INO). Bilateral INO is very rare and mo
