At the time of the American Revolution, understanding of diseases of the eye was rudimentary, treatment of ocular problems was largely ineffective, and American physicians were highly dependent on their European colleagues for training and development. Knowledge of optics, light and color was somewhat more advanced. The author reviews the state of eye care during the Revolutionary period, historically and anecdotally, with references to the literature of that time.

The results are discussed of a complex neuro-ophthalmological investigation of 439 patients with initial cerebral atherosclerosis. Their neurological characterization and the results of elecrcto-physiological, biochemical, and psychological examination methods are indicated. INTRODUCTION: The World Health Organization estimates that 2.2 billion people worldwide suffer from vision impairment, representing an annual productivity loss of US$411 billion. Meanwhile, ophthalmology advances at a fast-moving pace. As in every discipline, optimal ophthalmologic care depends on trustworthy clinical practice guidelines that promote patient-centred care and support consistent, evidence-informed decision-making.
