Retinal arteriovenous malformation and cerebral cavernous malformation in a 6-year-old child.

Retinal arteriovenous malformations (AVMs) are rare congenital, nonhereditary vascular anomalies of the retina. We report the case of a 6-year-old child presenting with recurrent frontal headaches. Funduscopy examination revealed an AVM in the right eye, inferior to the optic nerve head. Optical coherence tomography and retinal imaging confirmed the presence of an AVM measuring 3 disk diameters. Magnetic resonance imaging of the head revealed a cavernous malformation in the right frontal subcortical region.

Intracranial pressure and veins.

This review summarizes the relationship between intracranial pressure and the venous system as far as ultrasound methods are concerned. Although the pressure in cortical veins is tightly and linearly coupled to the intracranial pressure, venous flow velocities in the basal veins are not. However, venous flow velocities reflect better the local cerebral blood flow than measurements in arteries. Therefore, they correlate better with clinical outcome in head trauma and subarachnoid hemorrhage. Transorbital sonography with measurement of the optic nerve sheath diameter and optic disk elevation has…

Pigmentary macular degeneration with multifocal necrotizing encephalopathy.

A previously healthy 10-year-old girl suffered sudden, binocular visual deterioration. During the next few years her neurologic and visual condition progressively worsened and she developed hypertension, seizures, ataxia, and lactic acidemia, leading to death at the age of 16 years. Bilateral optic disk pallor was followed by the loss of the foveal reflex and pigmentary maculopathy, manifested as disorganization of the retinal layers, loss of ganglion cells, degeneration of the photoreceptors and nuclei, and irregular infiltration of the retina by pigment epithelial cells. The optic nerves and…

Imaging the optic nerve with optical coherence tomography.

Optical coherence tomography (OCT) is a non-invasive imaging technology, which may be used to generate in vivo quantitative and qualitative measures of retinal structure. In terms of quantitative metrics, peripapillary retinal nerve fiber layer (pRNFL) thickness provides an indirect evaluation of axonal integrity within the optic nerve. Ganglion layer measures derived from macular scans indirectly reflect retinal ganglion cell status. Notably, ganglion layer indices are platform dependent and may include macular ganglion cell inner plexiform layer (mGCIPL), ganglion cell layer (GCL), and gangl

Tumors and pseudotumors of the optic disc.

There are many lesions that can present as an elevation of the optic disc. Since each of them has different ophthalmic and systemic significance, it is important for the ophthalmologist to recognize them and to plan strategies of management when necessary. Important inflammatory and benign congenital conditions should be differentiated from malignant neoplasms. This review covers the differential diagnosis of lesions that can cause an elevation of the optic disc. It includes selected congenital lesions, inflammatory conditions, non-inflammatory non-neoplastic disorders, benign hamartomas, and

Correlation of Structure With Function: Future Utilities for Optical Coherence Tomography Angiography in Neuro-Ophthalmology.

Optical coherence tomography angiography is a burgeoning imaging modality in Ophthalmology. In this review, we outline the breadth of potential utility for optical coherence tomography angiography for diagnosis and prognostication in neuro-ophthalmology. Further mapping of the characteristics and natural history of the optic disc vascular network in neuro-ophthalmic conditions is necessary to increase the utility of this imaging modality for neuro-ophthalmic conditions, as the specificity and sensitivity of optical coherence tomography angiography are currently limited by the small pool of ava

Autoimmune Optic Neuropathy: Pathogenesis, Diagnosis, and Therapeutic Advances.

Autoimmune optic neuropathy (AON) encompasses a heterogeneous group of immune-mediated optic nerve inflammatory disorders, characterized by progressive or recurrent visual loss with or without optic disc edema, and distinct autoantibody profiles and neuroimaging features. This review synthesizes current knowledge on AON, highlighting key advances in the discovery of disease-specific biomarkers [eg, aquaporin-4 (AQP4)-IgG for NMOSD, myelin oligodendrocyte glycoprotein (MOG)-IgG for MOG-associated disease (MOGAD), and glial fibrillary acidic protein (GFAP)-IgG for astrocytopathy], while neuroima
