[Invasive aspergillosis of the skull base with orbit infiltration].

BACKGROUND: [corrected] Aspergillosis of the paranasal sinuses is subdivided into noninvasive and invasive types, depending on invasion of the tissue. The invasive form often occurs in immunodeficient patients and can be divided further into granulomatous, chronic invasive, and acute fulminating forms. CASE REPORT: We report the clinical course of an immunosuppressed 64-year-old male with invasive aspergillosis originating from the sphenoid sinus with infiltration of the orbit and intracranial extension into the cavernous sinus. The patient was referred to our hospital with loss of vision, pt…

Medial microsurgical approach to the orbit: an anatomic study.

The authors have identified and described three medial approaches to the intraorbital portion of the optic nerve in cadavers. The structures exposed via the superior, inferior or central approaches were approached through the medial orbital wall. The superior approach in which the medial rectus muscle is retracted inferiorly exposes the superomedial aspect of the orbit. The inferior approach in which the medial rectus muscle is retracted superiorly, provided the inferomedial aspect of the optic nerve from the globe to the muscle cone. The central approach in which the ethmoidectomy was perform…

Hepatocellular carcinoma metastatic to the orbit: a case report.

Hepatocellular carcinoma rarely metastasizes to the orbit. We report on a 72-year-old man with a past history of resection for hepatocellular carcinoma and recurrent HCC, who presented with diplopia and left painful proptosis. Head scans revealed a large and irregular mass in the left orbit that caused destruction of the orbital bone superiorly and posterolaterally. A biopsy specimens of the orbital tumor showed features of metastatic foci of hepatocellular carcinoma. The tumor was surgically excised and histological findings revealed moderately differentiated hepatocellular carcinoma, resembl…

Unusual presentation of gastric adenocarcinoma metastatic to the orbit.

A 69-year-old woman with no history of malignant disease presented with complaints of ptosis, diplopia, and left upper eyelid fullness. Computed tomography showed soft tissue infiltration of the left superior orbit. Biopsy was performed through an anterior orbitotomy. Histopathology revealed a mucinous adenocarcinoma. Subsequent systemic evaluation included esophagogastroduodenoscopy, which revealed a primary gastric malignancy.

Solitary fibrous tumor of the orbit with epithelioid features.

Extrapleural solitary fibrous tumors have often been confused with other mesenchymal tumors, such as hemangiopericytoma, fibrous histiocytoma, fibrous meningioma, and leiomyoma, because of morphologic similarity and underrecognition, especially if some unusual features are present. Recently, epithelioid solitary fibrous tumor has been reported in the mediastinum. We report a case of solitary fibrous tumor of the orbit with biphasic architecture, including spindle cell and epithelioid components. Both components demonstrated immunohistochemical features of a solitary fibrous tumor. A background…

Compressed helium injury to the orbit resulting in pneumocephalus.

A 33-year-old woman received a conjunctival laceration after accidental contact with the flexible outlet tip of a tank of compressed helium while filling balloons. The gas discharged during the contact, blowing compressed helium into her right orbit, with intracranial extension. The patient was asymptomatic, except for a transient headache. She was treated with prophylactic antibiotics and observed overnight, then discharged without complication. A literature search reveals that the usual outcome for this mechanism of injury is favorable.

Volume fusion for two-circular-orbit cone-beam tomography.

By using the Feldkamp-Davis-Kress (FDK) algorithm, we can efficiently produce a digital volume, called the FDK volume, from cone-beam data acquired along a circular scan orbit. Due to the insufficiency of the cone-beam data set, the FDK volume suffers from nonuniform reproduction exactness. Specifically, the midplane (on the scan-orbit plane) can be exactly reproduced, and the reproduction exactness of off-midplanes decreases as the distance from the midplane increases. We describe the longitudinal falling-off degradation by a hatlike function and the spatial distribution over the object domai…

Impacted knife injury of the orbit, maxilla and oropharynx.

Injuries in the maxillofacial region with knife in situ are not common. We report a rare case with knife impacted in the orbit, maxilla and soft palate. Removal was possible only by surgical intervention. Despite the deep penetration, the knife had not injured major structures and hence there was no untoward complication in the outcome.

Mortality following exenteration for malignant tumours of the orbit.

BACKGROUND: Orbital exenteration is a rare, but disfiguring procedure reserved for the treatment of locally invasive malignancy or potentially life threatening orbital neoplasms, when less destructive techniques are inadequate. The authors report their experience and analyses of 64 cases of orbital exenteration performed over a 13 year period, looking specifically at key factors affecting mortality associated with such a destructive surgical procedure. METHODS: Records were reviewed retrospectively of all patients who had undergone exenteration of the orbit from 1 January 1991 to 1 April 2004…

Concomitant cavernous hemangioma and venous angioma of the orbit.

BACKGROUND: An unusual case of cavernous hemangioma coexisting with venous angioma in the ipsilateral orbit is described. CASE: A 67-year-old woman had a mass in the lower eyelid of her right eye and proptosis. Imaging examinations showed two masses connected to each other in the extraconal space of the right orbit. OBSERVATIONS: Anterior orbitotomy was performed to remove the tumors. Histopathological evaluation of the anteriorly located tumor revealed cavernous hemangioma. The posteriorly located tumor was found to be a venous angioma. Endothelial cells in both tumors showed positive immun…

[Ophthalmologic aspects in the treatment of traumatic orbit deformity].

Results of ophthalmologic examinations of a total of 65 patients with traumatic orbit deformations and a pronounced dislocation of the eyeball (a mean time elapsed from trauma to surgery is 19 months) are presented in the article. Enophthalmos was diagnosed in 89% of patients, ptosis of the eyeball--in 69% of patients and ophthalmoplegia--in 77% of patients. Pathology of the posterior eye segment was recorded in a majority of cases, while pathology of the optic nerve was recorded in more than 43% of cases. Inferior orbital wall plasty was performed in all patients. A reduced optic neuropathy (…

Surgical management of the anophthalmic orbit, part 1: congenital.

Congenital microphthalmos and anophthalmos are rare conditions in which orbital growth is deficient. Hypoplasia of the globe affects the bony orbit (micro-orbitism), the conjunctival sac, and eyelids (microblepharism), and it may be associated with abnormalities of the entire hemifacial skeleton (hemifacial microsomia). In the present article, the authors review a series of 19 patients with microphthalmos (nine had right-sided, one had bilateral, and nine had left-sided microphthalmos) who were treated in the Orbitopalpebral Unit at Hospital Foch over a period of 15 years (follow-up, 5 months …

Fractures about the orbit in professional American football players.

A retrospective review was performed of records for players who suffered fractures about the orbit during participation in officially sponsored activities of the National Football League from 1980 to 1997. Clinical information was obtained on 19 of 29 players who sustained orbital fractures. The most common signs and symptoms included decreased visual acuity (74%, 14), decreased eye movement (42%, 8), hyphema (37%, 7), and infraorbital numbness (21%, 4). The mechanisms of injury were a digital poke (74%, 14) and blunt facial trauma (26%, 5). There were significantly more orbital fractures than…

Pseudotumor of the orbit. Clinical, pathologic, and radiologic evaluation.

Pseudotumor, Graves' disease, and lymphoproliferative disease are the most common ophthalmologic disease entities requiring evaluation by CT … evaluation of the Tolosa-Hunt syndrome, which is characterized by an inflammatory infiltrate in the orbital apex and cavernous sinus leading to cranial nerve involvement with ophthalmoplegia. The clinical and radiologic constellation of findings allows a definitive diagnosis in most cases.

Extension to the orbit from paraorbital disease. The sinuses.

Disease of the sinuses frequently involves the orbit. Infection, mucocele, and tumor of the sinuses originally may present with orbital findings. Imaging attempts to evaluate the interface between sinus and orbit. Imaging findings reflect the integrity of the sinus mucosa, the thin bony wall, and the more resistant fibrous periorbita. Both CT and MR imaging play a role in this evaluation.

Unilateral dermis-fat graft implantation in the pediatric orbit.

The purpose of this study was to evaluate the role of the dermis-fat graft (DFG) as an orbital implant in the pediatric age group. A retrospective study was made of a series of 16 patients who had undergone unilateral orbital implantation of a DFG. The ages of the patients at the time of surgery ranged from 2 months to 17 years, with followup ranging from 2 to 15 years. Growth of the graft was clinically apparent in the younger children. Increasing proptosis required surgical debulking of the graft in six of eight children who were 4 years old or younger at the time of DFG implantation. None o…
