Long-term outcomes of early childhood wheezing.

PURPOSE OF REVIEW: To summarize the currently available data on the long-term outcome until young adulthood after early childhood wheezing. Asthma and lung function abnormalities were evaluated, with special attention to early-life risk factors. RECENT FINDINGS: In postbronchiolitis studies, 30-40% of the patients hospitalized for wheezing at less than 24 months of age have had asthma at 17-20 years of age. In the Tucson birth cohort study, the respective figure after wheezing at less than 36 months of age was 30% at 22 years of age. Relapses after many nonsymptomatic years have been common.

Developmental origins of inflammatory and immune diseases.

Epidemiological and experimental animal studies show that suboptimal environments in fetal and neonatal life exert a profound influence on physiological function and risk of diseases in adult life. The concepts of the 'developmental programming' and Developmental Origins of Health and Diseases (DOHaD) have become well accepted and have been applied across almost all fields of medicine. Adverse intrauterine environments may have programming effects on the crucial functions of the immune system during critical periods of fetal development, which can permanently alter the immune function of offsp

Impaired renal graft perfusion due to malignant hyperthermia in kidney transplantation: early reduced graft perfusion by Doppler ultrasonography.

Malignant hyperthermia (MH) is a potentially life-threatening clinical reaction originating from uncontrolled calcium release from skeletal muscle cells triggered by volatile anaesthetic agents or succinylcholine (MH-trigger drugs). MH in a kidney transplant recipient jeopardises both graft and recipient survival.

Translation and Cultural Adaptation of the German Mastocytosis Quality of Life Questionnaire (MC-QoL) into Spanish.

BACKGROUND: Mastocytosis encompasses a spectrum of disorders in which different organs and tissues are affected by the clonal expansion of mast cells. The skin is one of the most frequently affected organs. The clinical manifestations of mastocytosis are linked to the release of proinflammatory mediators, and the impact of this disorder on patient quality of life has been described in various studies. The Mastocytosis Quality of Life Questionnaire (MC-QoL), which was recently developed in Germany and now also exists in English, is an important tool for evaluating the psychosocial impact of thi

Validation of a French version of the quality of life "Celiac Disease Questionnaire".

BACKGROUND AND OBJECTIVE: Celiac disease (CD) is a common chronic autoimmune disorder. Both the manifestations of the disease and the burden of the compulsory life-long gluten-free diet (GFD) have been shown to be associated with impairment of health-related quality of life. The objectives of this study were to provide a cross-cultural adaptation of the specific quality of life "Celiac Disease Questionnaire" (CDQ) and to analyze its psychometric properties. MATERIALS AND METHODS: A cross-cultural French adaptation of the CDQ (F-CDQ) was obtained according to the revised international guidelin

The origins of parasitism in the platyhelminthes.

Symbiotic associations have arisen independently in several groups of the largely free-living turbellarians. Morphological adaptations of turbellarians to a symbiotic way of life include suckers and adhesive glands for attachment, elaborate systems of microvilli and other epidermal structures for absorption of food, glands for the formation of cysts, cocoons and cement material, and lack of a pharynx and intestine in some species. However, many species closely resemble their free-living relatives. Egg production is greatly increased at least in some species, and life cycles are always direct.

Life-threatening hemoptysis due to left inferior phrenic artery to pulmonary artery fistula rescued by extracorporeal membrane oxygenation therapy.

Life-threatening hemoptysis due to bronchiectasis is an emergency and needs immediate bronchial angiographic embolization or surgical intervention. Most of the bleeding events arise from the bronchial artery. We report a very rare case of a patient who had bronchiectasis, but with bleeding from the left phrenic artery to pulmonary artery fistula, an artery originating from abdominal aorta. In this case, we also demonstrate that extracorporeal membrane oxygenation without heparinization as a bridge tool for life-threatening hemoptysis could allow the patient to undergo definite therapy more suc

[The aging changes of biological variability (author's transl)].

1. Examples are given to justify the concept that biological variability is dependent on age. 2. It appears possible that the variability of fundamental parameters of those with a strong genetic fixation increases toward the middle of the life span and decreases again in the senium. 3. The variability of most variables increases with age. The more complex the variables, the greater the increase. 4. Originally unitary statistical populations may split up into several populations in the course of life. These will then require separate statistical treatment. 5. Our considerations seem to have som

Prenatal Growth and CKD in Older Adults: Longitudinal Findings From the Helsinki Birth Cohort Study, 1924-1944.

BACKGROUND: According to the Developmental Origins of Health and Disease (DOHaD) hypothesis, several noncommunicable diseases, including hypertension, type 2 diabetes, and coronary heart disease, have their origins in early life. Chronic kidney disease (CKD) has traditionally been assumed to develop as the result of an interaction between genetic and environmental factors, although more recently, the importance of factors present early in life has been recognized. STUDY DESIGN: Longitudinal birth cohort study. SETTING & PARTICIPANTS: 20,431 people born in 1924 to 1944 in Helsinki, Finland, w

Switching from adalimumab originator to biosimilars in hidradenitis suppurativa: What's beyond cost-effectiveness?

In the last years, adalimumab biosimilars have represented a commonly used alternative to the originator agent in the treatment of moderate to severe hidradenitis suppurativa. As of today, studies investigating the switch from adalimumab originator to biosimilar, following pharmacoeconomic policies, are lacking. Herein we present a real-life setting retrospective study aimed at assessing the safety and efficacy of this switch in 37 patients, evaluated for 12 months in terms of IHS4 (International Hidradenitis Suppurativa Severity Score System) and HiSCR (Hidradenitis Suppurativa Clinical Respo

The EORTC QLQ-CR29 quality of life questionnaire for colorectal cancer: validation of the Dutch version.

PURPOSE: To validate the Dutch version of the EORTC QLQ-CR29 quality of life questionnaire for colorectal cancer. METHODS: We translated and pilot-tested the original questionnaire in the Netherlands, following EORTC guidelines. We assessed factor structure, reliability and construct validity in different samples of patients from four hospitals. RESULTS: Of 296 patients, 236 (80 %) returned the questionnaire, and 27 out of 48 patients returned the retest questionnaire. In addition to the original three scales, we found a reliable bowel functioning scale (α = 0.80), reducing the number of ind

A Prebiotic Synthesis of Pterins.

The genesis of life on Earth is a hypothesis of evolutionary science that can be, at least partially, tested experimentally. The prebiotic synthesis of cofactors or coenzymes is a poorly explored issue, likely because their formation under plausible prebiotic conditions is not clear. In this sense, it has been proposed that the cofactors are "molecular fossils" of an early phase of life. In contrast, Eschenmoser and Loewenthal suggested a prebiotic hydrocyanic origin of cofactor building blocks. In the present paper, the formation of a set of pterins from cyanide polymerizations is demonstrate

Segmental arterial mediolysis: a rare cause of an acute abdomen.

Segmental arterial mediolysis (SAM) is a nonatherosclerotic, noninflammatory and nonimmune arteriopathy of unknown aetiology. We present the case of a 43-year-old male who presented to the emergency department with abdominal pain. A computed tomography of abdomen and pelvis showed a narrow, hypodense superior mesenteric artery after the origin, raising the possibility of thrombus or vasculitis. He was commenced on rivaroxaban and steroids. He subsequently presented with an acute abdomen in a collapsed state. Repeat imaging of his abdomen and pelvis revealed an ischaemic ileal segment and caecu

Dominant gingers - Discovery and inheritance of a new shell polymorphism in the great pond snail Lymnaea stagnalis.

Color polymorphism is a classic study system for evolutionary genetics. One of the most color-polymorphic animal taxa is mollusks, but the investigation of the genetic basis of color determination is often hindered by their life history and the limited availability of genetic resources. Here, we report on the discovery of shell color polymorphism in a much-used model species, the great pond snail Lymnaea stagnalis. While their shell is usually beige, some individuals from a Greek population show a distinct red shell color, which we nicknamed Ginger. Moreover, we found that the inheritance fits

Understanding hippocampal sclerosis in the elderly: epidemiology, characterization, and diagnostic issues.

Hippocampal sclerosis (HS) is a pathologic term used to describe severe loss of neurons and reactive gliosis without cystic cavitation in the CA1 sector of the hippocampus. In late life, HS is associated with hippocampal atrophy, severe amnesia, and slowly progressive dementia without clinical seizure activity. HS is difficult to distinguish clinically from Alzheimer's disease and is often diagnosed postmortem. In autopsy series, HS may be found without significant other pathology (2%-4% of cases), but it occurs frequently in combination with other vascular and neurodegenerative disorders (12%

[Malnutrition in the aged--effect of physical, mental, psychological and social factors].

The elderly are considered a risk group for malnutrition, due not only to poor nutrition, but also to other aspects of daily life which affect food intake and nutritional status. In this study 11 factors of physical, mental, psychical, and social origin have been defined as risk-factors leading to malnutrition. Their appearance in a group of 100 geriatric patients above the age of 75 years has been examined. Immobility and difficulties with chewing were found in nearly 50% of the patients. Social problems were observed in 22%-31%. In contrast, there was usually a daily hot meal available to th

Neonatal septicaemia.

A total of 410 proved cases of neonatal septicaemia from seven Finnish hospitals seen between 1976 and 1980 were reviewed. The annual incidence of neonatal septicaemia was 3 per 1000 births, and overall mortality was 23%. Onset was early in most patients. Symptoms of septicaemia occurred within the first 24 hours of life in 44% and within the first week of life in 90%. In the very early onset disease (within 24 hours) mortality was 30%, compared with 17% in all other cases. Group B streptococcus was the leading cause in very early onset disease (52%) but mortality from infection with this orga

Diagnosis, Pathophysiology and Management of Hypercalcemia in Malignancy: A Review of the Literature.

Hypercalcemia of malignancy is the most common life-threatening metabolic disorder in patients with advanced stage cancers and is a sign of poor prognosis. It usually presents with markedly elevated calcium level and is severely symptomatic. It is associated with hematological malignancies, such as multiple myeloma, non-Hodgkin lymphoma, leukemias and solid cancers, particularly renal and breast carcinomas as well as squamous cell carcinomas of any organ. Several mechanisms have been implicated in the development of hypercalcemia of malignancy amongst them the osteolytic related hypercalcemia,

Perinatal nicotine exposure disrupts hematopoietic stem cell development and elevates influenza severity in adulthood.

Developmental exposures profoundly shape lifelong health by altering disease resilience later in life. One potential cause of increased disease susceptibility is an altered immune system, originating in epigenetically maladaptive hematopoietic stem cells (HSCs). Here, we establish perinatal nicotine exposure (PNE) as a mouse model of developmental maladaptive immunity, wherein early-life environmental insults permanently alter HSCs. We show that PNE selectively disrupts the establishment of HSCs and tissue-resident, but not circulating, immune cells. Remarkably, the specific decrease of HSCs a

Development of fetal and neonatal sleep and circadian rhythms.

The origin of sleep and circadian rhythms development is found during the fetal period. Both quiet (NREM) and active (REM) sleep are distinguishable during the last 10 weeks of gestation. Comparable to fetuses, low risk preterm infants recorded at 30-40 weeks postconceptional age, had a similar development of sleep i.e. an increase in quiet sleep and a decrease in indeterminate sleep. A further development in sleep organization characterized by increased slow wave and spindle activity during quiet sleep and coupling with circadian rhythm takes place during the first 6 months of life in both te
