The authors report on a case of Paget's disease with hydrocephalus in an elderly female with a favourable result after ventriculoperitoneal shunt. The neurological examinations at the time of admission revealed mild dementia, ataxic gait and urinary incontinence. Skull X-ray, CT-scan and MRI showed hydrocephalus and basilar impression. The pathophysiological hypothesis is discussed and the technical difficulties during the shunt surgery are described.

Analyses of the urinary concentration relative to creatinine of the collagen crosslinks, pyridinoline (Pyd) and deoxy-pyridinoline (Dpd) were made in 47 patients with metabolic bone diseases to assess the validity of these assays as indicators of bone resorption. The mean values for patients with Paget's disease of bone, primary hyperparathyroidism and osteomalacia were significantly higher (P less than 0.001) than those for age-matched healthy individuals. During treatment of Paget's disease with bisphosphonates, there was a steady decline in the urinary concentration of the crosslinks to the

A 48-year-old man with a ten-year history of corticosteroid-dependent asthma and a previous fracture of the right humerus after minimal trauma was referred for a dual photon absorptiometry (DPA) study to assess the degree of osteoporosis. The DPA study revealed a large discrepancy in bone mineral density (BMD) between the right and left hips. A Tc-99m MDP bone scan and an anteroposterior frog-leg radiograph demonstrated changes typical of Paget's disease in the sacrum, pelvis, and femurs. This case is an example of how underlying bone disease may artificially elevate bone mineral density, and

Fifty patients with Paget's disease of bone were reviewed with regard to the basis of their symptoms and the long-term results of treatment. Twenty-four patients (48%) presented with pain localised within bone, while 17 (34%) presented with symptoms of degenerative joint disease. Three patients presented with bone pain and arthritis and the remaining six with fractures, ataxia, or painless deformity. Symptomatic osteoarthritis of the hip (OA) developed in 25 patients (50%) with approximately half developing radiological changes identical to those of idiopathic OA. Among the other patients thos

Fifty-one patients with Paget's disease were treated during 6 months with ethane-01 hydroxy-1, 1 diphosphonate (EHDP, etidronate) in doses of 5 mg/kg/day. The drug was most effective in relieving pain, at the same time diminishing the uptake of pagetic lesions on quantitative bone scans and lowering by almost 70% the abnormally high serum alkaline phosphatase levels and 24-hour hydroxyproline urinary levels. Histologically, the resorption surfaces and the number of osteoclasts per mm2 of bone tissue were significantly reduced. On follow-up, the improvement persisted for at least one year after

Nine patients with symptomatic Paget's disease were treated with mithramycin in doses of 15 mug/kg/day for 10 days. All but one had considerable symptomatic improvement and, in some, pain relief was dramatic after only a few days. A progressive decrease in serum alkaline phosphatase levels and urinary hydroxyproline excretion was seen at the same time. Bone scans with technetium-99m stannous polyphosphate were performed in six patients and were grossly abnormal in all, showing increased uptake in areas approximately co-extensive with the radiological changes. In five cases improvement in the s

Although Paget's disease of bone (PDB) is the second most common metabolic bone disease, there is only limited information about the microarchitecture of affected bones. Therefore, the aim of this study was to determine cortical and trabecular bone properties in clinically relevant locations by microcomputed tomography (µCT). Ten femora and ten tibiae affected by Paget's disease taken from the Natural History Museum Vienna were compared to 13 femora and 10 tibiae of non-affected body donors. Digitization of the cortical and trabecular bone microarchitecture was performed with an X-ray-based µC

Paget's disease is a chronic focal high turnover bone disorder that is primarily present in middle-aged or older adults. It seems to be restricted to humans and has no clear parallels with other diseases. Although much has been learnt about its pathology and epidemiology, and treatment is now highly effective we still lack a complete understanding of its etiology and biology. This review focusses on the natural history of the disorder, in particular its changing epidemiology, recent discoveries about its genetic basis and current approaches to diagnosis and treatment. While there is strong evi

Paget's disease of bone (PDB) is associated with a germline mutation in Sequestosome1/p62 (SQSTM1) found in ≤16 % of sporadic cases worldwide, and in 19-46 % of those studied with familial PDB. The P392L is the most prevalent mutation identified to date. This mutation by itself does not confer PDB or define the phenotype of PDB in a given person. Environmental determinants remain elusive, although increasing age of the individual, other gene polymorphisms in the context of SQSTM1 mutations, and measles virus have been implicated. Measles exposure has been unexamined in this context. The goal o

Epidemiological studies of Paget's disease of bone (PDB) have shown a remarkable geographical variation in the prevalence rates of the disease. In South America, the frequency is considered to be low, despite scant epidemiological data on this continent regarding the disease. The aim of this study was to evaluate the prevalence and incidence of PDB in one institution in the city of Recife, Northeast Brazil. All patients aged 45 years and over attending the Pernambuco Osteoporosis Centre at the Endocrine and Diabetes Department between January 2006 and December 2009 were assessed. Period preval
