We report a case of Aeromonas osteomyelitis due to injury in a sewage worker. He presented with cellulitis of the left foot. Radiographs showed evidence of osteomyelitis involving the head and neck of the fifth metatarsal. Aeromonas species was isolated from the tissue and swab from the foot. The head and neck of the fifth metatarsal were excised and the patient improved on 4 weeks of intravenous meropenem followed by 4 weeks of oral clindamycin and ciprofloxacin.

Panniculitis is an inflammatory process of the subcutaneous adipose layer (panniculos adiposus). It is frequently a sign of systemic disease with a wide range of clinical presentations. The histopathological findings are diverse, making diagnoses difficult. We present a case of heel panniculitis, initially misdiagnosed as calcaneal osteomyelitis. Our intention is to make physicians, treating these type of patients, aware of the possibility of this rare disease, especially in situations when conventional therapy is not successful. A 66-year-old woman was admitted to the hospital with the initia…

Cutaneous fistulas and sinuses in the maxillofacial region secondary to osteomyelitis rarely appear in clinical practice. The most frequent cause of mandibular osteomyelitis is a dental infection, but it may also be hematogenic in origin. The diagnostic criteria for bacterial osteomyelitis are suppuration and osteolytic changes in the radiological study. The differential diagnosis of an ulcerative lesion in the mandibular area includes several pathologies, such as a fistula of dental origin, a reaction to a foreign body, a deep mycotic infection, a pyogenic granuloma or a congenital malformati…

OBJECTIVE: To analyze the clinical, microbiological, and radiologic features of patients without drug addiction suffering from spontaneous pyogenic vertebral osteomyelitis.
METHODS: We collected all microbiologically proved cases of pyogenic vertebral osteomyelitis seen between January 1980 and December 1999 in a teaching hospital. Patients with prior spinal instrumentation or surgery and injection drug users were excluded.
RESULTS: Sixty-four patients, with a mean age of 59 +/- 17 years, were identified. In 29 (45%) patients, 1 or more underlying medical illnesses were found. The mean durat…

A case is reported of an 85-year-old woman with mandibular osteomyelitis secondary to a submental pressure sore. The main aetiological factors in the development of the pressure sore were dementia and severe senile kyphosis. Pressure sores are rare in the head and neck region and, though osteomyelitis is a common complication at other anatomical sites, it has not been previously reported in the mandible.

Ulnar clubhand is a rare deformity. In a patient with postnatal osteomyelitis of the ulna, the wrist had an ulnar deviation with marked ulnar slope of the radius. Good hand function precluded surgical correction.

INTRODUCTION: Osteomyelitis is a common inflammatory process caused by an infection which is usually from gram-positive germs. Acute, subacute and chronic forms can be distinguished both clinically and radiographically, each of them presenting different patterns but which are not always easy to recognize. Besides clinical-laboratory data, imaging methods are also useful to make the diagnosis; in chronic forms, MRI has higher sensitivity in recognizing the active foci and the presence of sinus tracts than the other techniques.
MATERIAL AND METHODS: We examined nine patients with suspected chro…

Plantar puncture wounds to the foot are a common injury. A small number (1.8%) of these puncture wounds become infected and progress to osteomyelitis. The purpose of this article is to report the cases of six patients who developed osteomyelitis of the calcaneus after a puncture wound to the heel caused by a nail. The characteristics of the patients, the pathogenic organism, and the outcome were studied. Patients who were healthy and had no systemic illness (N = 4) had only one pathogenic organism cultured, whereas patients who had systemic illness (diabetes mellitus, N = 2) had more than one …

In acute osteomyelitis of childhood a rapid diagnosis and initiation of antibiotic therapy is necessary in order to prevent late sequelae. Thus, diagnostic imaging plays a crucial role. If acute osteomyelitis is suspected in a child, imaging starts with conventional radiography in order to exclude other differential diagnoses. This is followed by sonography for the purpose of diagnosing a subperiosteal abscess or joint fluid from which the causative organism could be isolated. If the diagnosis is unclear, the next step should be either MRI or 99mTc-MDP bone scan, depending on the possibility o…

Infections in systemic lupus erythematosus are frequent. However, osteoarticular Salmonella infections are rarely reported. We report a case of systemic lupus erythematosus diagnosed in a 15 year-old girl. Seven months later, she presented with fever and a localized collection of the upper extremity of the left tibia related to a Salmonella enteritidis acute osteomyelitis (sub periosteal abscess). The out-come was chronic and led to death. The authors emphasize the severity of non typhoidal salmonellosis in systemic lupus erythematosus.

We reviewed nine cases of acquired radial clubhand due to osteomyelitis. Goals were cosmetic improvement, maintenance of any wrist mobility, and a stable, painless wrist and forearm. All patients achieved these goals, although residual forearm shortening ranged from 4-8 cm. Treatment of acquired radial clubhand is difficult. The options and guidelines for treating the congenital variety may not be applicable in the treatment of the acquired type. Surgical options include interposition bone grafting, centralization, and radioulnar transposition. Recently the circular external fixator has been u…

Osteomyelitis of the spine is a well-recognized delayed manifestation of septicemia but has not been recognized as a complication of total parenteral nutrition. We report five cases of spinal osteomyelitis that were clinically recognized 1 to 13 months after total parenteral nutrition catheter-induced septicemia. Radiographic evidence of osteomyelitis was seen in all five patients. In three patients, culture of bony aspirates was positive for the same organism as from the blood. In one case, the diagnosis was established by histology, and in one the diagnosis was based on radiographic and radi…

A nine-month-old girl with osteomyelitis related to BCG vaccination is presented. The patient did not have any clinical evidence of immunodeficiency. Her X-ray examination showed a lytic lesion of the right tibia and of the left ulna. Histopathological examination of biopsy material revealed a chronic granulomatous inflammatory reaction. She improved with intermittent short course antituberculosis therapy.

A systematic and easily reproduced bone protocol was used over a 14-month period to evaluate bone histologically and by mycobacterial culture in patients with chronic osteomyelitis. On examination of 140 bone specimens, we found four patients with unsuspected tuberculous osteomyelitis whose diagnosis was obscured by a concomitant staphylococcal osteomyelitis. Three of the patients had axial skeleton involvement, and one had disease of the femur. No patient had a history of a positive skin test or of tuberculous disease, and none had coexistent pulmonary disease. The underlying granulomatous in…
