Oxygen saturation was measured during anaesthesia and electroconvulsive therapy (ECT) using ear oximetry. Significant hypoxia occurred during some treatments; desaturation related to the number of ventilations performed after muscle relaxation and prior to the shock. Desaturation did not correlate significantly with seizure duration. These findings highlight the need for adequate oxygenation during ECT.

A diffusion barrier to oxygen caused by fibrin deposition around dilated, proliferating capillaries in patients with venous hypertension may contribute to the development of venous ulceration. This diffusion barrier was studied in 18 patients with venous ulcers using the transcutaneous oxygen (TcPO2) monitor (TCM204 Radiometer, America). TcPO2 sensors were placed adjacent to venous ulcers on lower limbs and on the chest and foot of each patient. Readings were taken after a sensor temperature of 44 degrees C was reached (10-15 minutes). TcPO2 values were markedly decreased in skin adjacent to t

The clinical or physiological signs of hypoxaemia have limited value during anaesthesia. In the absence of surgical bleeding, the best signs are probably cyanosis and/or bradycardia-although neither is sensitive or specific. A moderate degree of hypoxaemia can be present without clinical signs! In the presence of surgical bleeding, the best sign may be darkening of fresh blood in the surgical field. The only reliable indicators of arterial oxygenation available for routine anaesthesia are the laboratory measurements of PaO2 and SaO2. Unfortunately, these measurements are intermittent and delay

Hyperbaric oxygen administered systemically has been advocated in the treatment of pyoderma gangrenosum. Owing to the serious potential risks of oxygen toxicity with systemic administration, we have devised a simplified technique for the administration of hyperbaric oxygen topically, and have used this method successfully to treat two patients with pyoderma gangrenosum, in whom the underlying etiology was not apparent, and specific therapy, therefore, not possible. The abundant formation of granulation tissue and the ability of the hyperbaric oxygen to arrest further extension of the ulceratio

The effects of 6 h of continuous low flow, nasally administered oxygen were compared with the effects of breathing air in 10 men and 2 women with obstructive sleep apnea and daytime hypersomnolence. The overall quality of sleep, sleep fragmentation, the pattern of breathing, nocturnal oxygenation, and the clinical effects on daytime hypersomnolence determined by multiple sleep latency testing were evaluated. We found that in non-REM sleep, breathing 3 L/min of oxygen increased baseline percent arterial oxyhemoglobin saturation and decreased both the rate of sleep-disordered breathing from 69 +

A critical review of the literature of retrolental fibroplasia indicates that the cause of this disease is not yet known. Oxygen is certainly a critical factor but it is still not possible to make precise recommendations as to the amount or the duration of therapy that is safe. We have overemphasized the role of oxygen in the past, and as a result of this the false impression has been created that RLF is a disease that can be prevented. This gross oversimplification of a complex disease with multiple causes has resulted in many unjustified malpractice claims. A study of the present epidemic in

The fact that as many as 60 new hyperbaric oxygen chambers are being established each year in the United States indicates the extent of interest in this relatively new therapy. In 1983 the Hyperbaric Medicine Committee of the Undersea Medical Society revised its classification of conditions being treated by hyperbaric oxygen (HBO), dividing them into two categories: category 1, conditions for which HBO treatment is known to be effective and is third-party reimbursable, and category 2, conditions for which such treatment is unproved experimentally or clinically and is not reimbursable. In the f

Thoracic physicians in New South Wales, Australia, and conservative in their administration of long-term oxygen therapy. Relatively few patients are being treated with it at present. Those who are use oxygen cylinders and concentrators in their homes. Use of long-term therapy is restricted to two groups: first, well-motivated patients who, after investigation and treatment, continue to have PaO2 values below 60 mm Hg and evidence of complications arising from hypoxia; and second, a few patients who have central apnea and severe desaturation of hemoglobin during sleep.

A system of transtracheal oxygen administration has been developed which is more effective for rehabilitating chronic obstructive pulmonary disease (COPD) patients than traditional systems for providing continuous oxygen therapy. The procedure involves administering oxygen continuously through a No. 16 intravenous catheter inserted transtracheally. Therapeutic PaO2 levels are attained with an oxygen flow of 0.25 to 1 liter per minute. Transtracheal oxygen administration has numerous advantages over nasal cannula or Venturi mask devices. With this system, the patient requires 3 to 4 times less

Hyperbaric oxygen therapy was utilized in a case of fulminant mucormycosis of the maxilla, orbit and temporal bone. The patient had refused radical surgery and death seemed iminent in spite of aggressive medical management of diabetic ketoacidosis, amphotericin B and wide surgical drainage of the maxillary and ethmoid sinuses with orbital decompression. Hyperbaric oxygen was instituted on the following theoretical premises: 1. It would provide oxygenation of tissues distal to occluded arteries, thereby increasing local survival and decreasing acidosis, 2. resultant lessening of acidosis would
