Prevalence, clinical presentations, and feto-maternal outcomes of eclampsia in a teaching hospital setting in Tigray region, Ethiopia: A five-year review.

BACKGROUND: Hypertensive disorders of pregnancy remain a leading cause of preventable maternal and perinatal mortality, particularly in low‑ and middle‑income countries (LMICs). Eclampsia, the most severe manifestation, is responsible for a disproportionate share of maternal deaths in sub‑Saharan Africa, yet contemporary data from Tigray, Ethiopia are scarce. OBJECTIVE: To determine the prevalence, clinical presentation, and factors associated with adverse feto‑maternal outcomes among women managed for eclampsia at Ayder Comprehensive Specialized Hospital (ACSH), Tigray, Ethiopia, between 1 J…

The methodological challenges of attempting to compare the safety of home and hospital birth in terms of the risk of perinatal death.

This paper identifies a number of methodological difficulties associated with the comparison of home and hospital birth in terms of the risk of perinatal death, and suggests ways in which these problems can be overcome. A review of recent studies suggests that most available data sources are unable to overcome all of these challenges, which is one of the reasons why the debate about whether perinatal death is more likely if a home birth is planned or if a hospital birth is planned has not been satisfactorily resolved. We argue that the debate will be settled only if perinatal mortality data fr…

Early neonatal deaths associated with perinatal asphyxia in infants ≥2500g in Brazil.

OBJECTIVE: To assess the annual burden of early neonatal deaths associated with perinatal asphyxia in infants weighing ≥2500g in Brazil from 2005 to 2010. METHODS: The population study enrolled all live births of infants with birth weight ≥2500g and without malformations who died up to six days after birth with perinatal asphyxia, defined as intrauterine hypoxia, asphyxia at birth, or meconium aspiration syndrome. The cause of death was written in any field of the death certificate, according to International Classification of Diseases, 10th Revision (P20.0, P21.0, and P24.0). An active searc

Evaluating pregnancy reporting in Siaya Health and Demographic Surveillance System through record linkage with ANC clinics.

INTRODUCTION: Health and Demographic Surveillance Systems (HDSS) are important sources of population health data in sub-Saharan Africa, but … red to 10% of those lacking registration. Registered pregnancies had higher rates of stillbirth and perinatal mortality than those lacking registration. In 77% of cases, women accessed ANC prior to registering the pregnancy in the HDSS. Half of reported miscarriages were misclassified stillbirths. We identified 141 unreported pregnancies that likely ended in adverse …

Current approach and attitudes toward neonatal near-miss and perinatal audits: An exploratory international survey.

BACKGROUND: Combined with perinatal mortality review, neonatal near-miss (NNM) audit has the potential to inform strategies to better prevent adverse perinatal outcomes. Nonetheless, there is lack of standardised definitions of NNM and limited evidence of implementation of NNM audits. AIM: To describe definitions of NNM and assess current approaches and attitudes toward perinatal mortality and morbidity audit. MATERIALS AND METHODS: Online survey from December 2021 to February 2022, with a mix of Likert scales, polar, pool, multi-choice, and open-ended questions, disseminated through nationa…

Parents' Active Role and ENgagement in The review of their Stillbirth/perinatal death 2 (PARENTS 2) study: a mixed-methods study of implementation.

OBJECTIVE: When a formal review of care takes places after the death of a baby, parents are largely unaware it takes place and are often not meaningfully involved in the review process. Parent engagement in the process is likely to be essential for a successful review and to improve patient safety. This study aimed to evaluate an intervention process of parental engagement in perinatal mortality review (PNMR) and to identify barriers and facilitators to its implementation. DESIGN: Mixed-methods study of parents' engagement in PNMR. SETTING: Single tertiary maternity unit in the UK. PARTICIP…

Employer Groups Pushing For Improving Maternal Health.

Given the startling statistic that the rate of women dying of pregnancy-and childbirth-related complications in the United States is increasing, it seems logical that the U.S. health system would focus on improving outcomes in this area of care. But not all health plans address this disparity in health care delivery.

Perinatal outcomes in Malta between 2008 and 2022: a comparison of three 5 yearly epochs.

BACKGROUND: Perinatal and neonatal mortality rates are quality indicators of antenatal, neonatal and postnatal care. This study describes perinatal outcomes for Malta over the 15 year period 2008-2022 to inform future practice and healthcare provision. METHODOLOGY: Aggregate anonymous data on perinatal outcomes from 2008 to 2022 were obtained from the National Obstetric Information System (NOIS), Directorate for Health Information and Research (DHIR). Data for each variable was grouped into three 5-year intervals (2008-2012, 2013-2017 and 2018-2022) to investigate trends over time. RESULTS:

Perinatal death in the Nordic countries in relation to gestational age: The impact of registration practice.

INTRODUCTION: Although perinatal death rates in the Nordic countries are among the lowest in the world, the risk of perinatal death is unevenly distributed across the Nordic countries, despite similarity in health care systems and pregnancy care. Birth registration practices across countries may explain some of the differences. We investigated differences in national registration of perinatal mortality within the Nordic countries and its impact on perinatal mortality according to gestational age. MATERIAL AND METHODS: Each country provided information by answering a questionnaire about regist…

Maternal mortality, stillbirths, and neonatal mortality: a transition model based on analyses of 151 countries.

BACKGROUND: Maternal mortality, stillbirths, and neonatal mortality account for almost 5 million deaths a year and are often analysed separately, despite having overlapping causes and interventions. We propose a comprehensive five-phase mortality transition model to improve analyses of progress and inform strategic planning. METHODS: In this empirical data-driven study to develop a model transition, we used UN estimates for 151 countries to assess changes in maternal mortality, stillbirths, and neonatal deaths. On the basis of ratios of maternal to stillbirth and neonatal mortality, we identi

Changes in perinatal hospital deaths occurring outside the neonatal intensive care unit over a decade.

AIM: Perinatal deaths occurring outside the neonatal intensive care unit (NICU) are rarely recorded in outcome studies, despite having a dir … e care changed between the two study periods. Paediatricians should be aware of the epidemiology of perinatal mortality in their own practice, as it has a direct impact on the denominator in NICU outcome studies.
