Subperiosteal/subgaleal drainage vs. subdural drainage for chronic subdural hematoma: A meta-analysis of postoperative outcomes.

BACKGROUND: Chronic subdural hematoma (CSDH) is commonly treated via surgical removal of the hematoma, placement of a routine indwelling drainage tube, and continuous drainage to ensure that the blood does not re-aggregate following removal. However, the optimal location for placement of the drainage tube remains to be determined. OBJECTIVES: To aid in establishing a reference for selecting the optimal method, we compared the effects of different drainage tube placements on CSDH prognosis via a systematic review and meta-analysis of previous clinical studies. DATA SOURCES: PubMed, Embase, an

Alveolar bone grafting and gingivoperiosteoplasty in bilateral cleft lip and palate.

PURPOSE OF REVIEW: Gingivoperiosteoplasty (GPP) and alveolar bone grafting (ABG) procedures have a vast history. There have been many publications regarding ABG and secondary ABG, with and without presurgical infant orthopedics (PSIO), in bilateral cleft lip and palate patients. There is little long-term data available describing results of both techniques. RECENT FINDINGS: The interdisciplinary approach to primary and secondary surgical procedures has proven to be beneficial for patients with bilateral cleft lip and palate. In the neonates, naso-alveolar molding (NAM) has been found to optim

Spontaneous Regression of Diffuse Periosteal Melanotic Neuroectodermal Tumor of Infancy in the Tibia, With 13-Year Follow-up.

Melanotic neuroectodermal tumor of infancy (MNTI) is a rare entity primarily affecting the craniofacial bones during the first year of life, with only 5 reported cases involving peripheral long bones. We herein present a case of MNTI in the tibia of an infant, with a somewhat atypical presentation, and a noteworthy clinical course characterized by progressive spontaneous resolution without therapy, thus sparing the child the trauma of amputation. There is no evidence of active residual or recurrent disease with 13-year follow-up. To the best of our knowledge, essentially all reported cases of

Vascularized tibial periosteal graft in complex cases of bone nonunion in children.

Bone nonunion in the pediatric population usually occurs in the context of highly unfavorable biological conditions. Recently, the vascularized fibular periosteal flap has been reported as a very effective procedure for treating this condition. Even though a vascularized tibial periosteal graft (VTPG) was described long ago and has been successfully employed in one adult case, there has been no other report published on the use of this technique. We report on the use of VTPG, pedicled in the anterior tibial vessels, for the treatment of two complex pediatric bone nonunion case: a recalcitrant

Unusual presentation of bizarre parosteal osteochondromatous lesion of the second toe (Nora's lesion).

Bizarre parosteal osteochondromatous proliferation (BPOP) of bone is a rare benign neoplasm. This report presents the first case of BPOP associated with gangrenous changes and ulceration after remaining symptomless for 20 years. The authors hypothesize that the cause of gangrene may be attributed either to vascular occlusion due to pressure, trauma, or malignant transformation of the lesion. Although local excision of the tumor is the treatment of choice, the authors resorted to ray excision, as gangrenous changes and ulceration were present and there was the possibility of malignant change.

Establishment of a critical-sized alveolar defect in the rat: a model for human gingivoperiosteoplasty.

BACKGROUND: Despite technical advancement, treatment of congenital alveolar clefts has remained controversial. Currently, primary alveolar cleft repair (i.e., gingivoperiosteoplasty) has a 41 to 73 percent success rate. However, the remaining patients have persistent alveolar bone defects requiring secondary grafting procedures. Morbidity of secondary procedures includes pain, graft resorption, extrusion or infection, and graft or tooth loss. The authors present a novel rat alveolar defect model designed to facilitate investigation of therapeutics aimed at improving bone formation following pr

The use of free vascularized corticoperiosteal grafts from the femur in the treatment of scaphoid non-union.

Most scaphoid non-unions can be treated successfully with conventional bone graft and screw fixation. Only a few cases with established avascular necrosis of the proximal fragment and previous failed conventional bone grafting should undergo vascularized bone graft. Cases in which the fragments are too small to accommodate a vascularized bone graft are better treated by alternative procedures. This article describes the role of vascularized bone grafting for scaphoid non-union, indications, donor bone selection, the authors' operative technique for vascularized bone graft from the supracondyla

[Melorheostosis of the foot: a case report of a rare entity].

INTRODUCTION: Melorheostosis is a rare bony dysplasia and often recognised just sporadically by chance. CASE REPORT: We present a case of a 15 year old girl who presented a melorheostosis of the left foot. After birth there was recognized a shortening and deformity of the 2nd toe on the left foot. Furthermore she had an interphalangeal hallux valgus that displaced the 2nd toe increasingly. Thus in the last years there were more and more difficulties to wear normal shoes. Conservative therapy was not successful. We performed a lengthening extending osteotomy of the 2nd toe (a modified Weil ost

Endoscopic repair of a nasal septal perforation.

The repair of septal perforations represents a challenging problem, with numerous techniques existing to address the defect. The use of sinonasal endoscopy represents a novel approach to solving this problem. By using the endoscope, excellent visualization and exposure can be achieved without excessive dissection. In addition, the use of the camera and monitor allows for excellent teaching capabilities.

Anatomic study of the distal supraorbital nerve.

This investigation was designed to extend our present knowledge of the supraorbital n. (SO n.) distal to the supraorbital notch. It is based on 40 dissected hemi-faces and the position of the notch and the periosteal and frontalis cutaneous branches of the SO n. were studied. The notch was 33.05 mm from the midline on the right side and 30.70 mm on the left. The periosteal branch arises from the lateral frontalis cutaneous branch. Its ascends in an oblique direction laterally and ends in two terminal branches. The frontalis cutaneous branch, after a very short trunk, divides into two branches,

Clinical observations of cleft lip and palate.

In surgical management of cleft lip and palate, appreciation of the special anatomy and physiology of the human premaxilla is more important than the question of whether it is a separate bone. To take advantage of the potential of the premaxilla, both primary and secondary surgical procedures must strive to establish a nearly normal medial septal system and nasolabial musculature. To complement accurate muscle surgery of the lip and soft palate, the characteristics and functions of the mucoperiosteum that covers the palate must be respected. The quality of subsequent facial growth can be monit…

The surgical closure of exposed skull.

The closure of scalp defects is still surrounded by many uncertainties. Free skin graft has proved most successful. Thiersch graft, split skin graft and full thickness skin graft have their indication for use. Thiersch skin graft is used as a temporary dressing, in order to achieve aseptic wound conditions, but cannot be looked upon as a lasting solution cosmetically and functionally. Split skin is used only for the closure of smaller defects. Full thickness skin grafts are most suitable, because they offer a lasting coverage, which meets to all demands. If the outer table of the skull is expo

A Multilayered Gradient Approach to Calcium Hydroxylapatite for Enhanced Full-Face Contouring and Biostimulation.

The growing demand for noninvasive facial contouring and soft tissue regeneration has fueled interest in dermal fillers with multipurpose capabilities. Calcium hydroxylapatite combined with carboxymethylcellulose (CaHA-CMC) has gained prominence due to its dual-action properties-serving as both a volumizing filler and a biostimulatory agent. This study introduces a standardized gradient-based protocol for CaHA-CMC, employing varying dilutions to stimulate the biomechanical characteristics of different anatomical layers. Highly concentrated formulations are deposited on the periosteum to mimic-…
