Haemophobia.

I recently finished an introductory session with a group of new start undergraduate nursing students about their responsibilities in clinical practice and was finishing up the session by checking if anyone had any questions. The usual questions were asked such as "why is it called off duty and not on duty?" Good question but once you have worked in clinical practice for a while the question of when you are off duty becomes very important more so than on duty! However, I was not expecting the following question which was, "I faint when I see blood do I need to tell the mentor when I am out in p

Psychotherapy mediated by remote communication technologies: a meta-analytic review.

BACKGROUND: Access to psychotherapy is limited by psychopathology (e.g. agoraphobia), physical disability, occupational or social constraints and/or residency in under-served areas. For these populations, interventions delivered via remote communication technologies (e.g. telephone, internet) may be more appropriate. However, there are concerns that such delivery may influence the therapeutic relationship and thus reduce therapy effectiveness. This review aimed to determine the clinical effectiveness of remotely communicated, therapist-delivered psychotherapy. METHODS: Systematic review (incl

Unexplored issues and future directions in social phobia research.

There have been significant advances in recent years in the understanding and treatment of social phobia, which are summarised in this Special Issue. This paper outlines and describes some further priority areas for future research. It is suggested that social phobia should be investigated as a developmental condition, with special attention to the first two years of life; and subtypes of social phobia be further explored and their implication for treatment. Social phobia is often comorbid with other disorders and its relationship with these disorders warrants further investigation with respec

Self-focused attention in social phobia and social anxiety.

Self-focused attention is an awareness of self-referent information and is present in many emotional disorders. This review concentrates on the role of self-focused attention in social anxiety with particular reference to the Clark and Wells [Clark, D. M., & Wells, A. (1995). A cognitive model of social phobia. In R. R. G. Heimberg, M. Liebowitz, D. A. Hope, & S. Scheier (Eds.), Social phobia: diagnosis, assessment and treatment. New York: Guilford.] model of social phobia. According to Clark and Wells, self-focused attention is an important maintaining factor in the disorder because it increa

[Social phobia: east and west].

The condition corresponding to social phobia, named taijin-kyofu (taijin: in relation to people, kyofu: phobia), has been widely known since the early 1930s in Japan. As there had been few reports on social phobia in Western countries until DSM-III was introduced in 1980, taijin-kyofu had long been considered as a unique condition existing only in Japan. After DSM-III, it was realized that taijin-kyofu included two types of social phobia (SP). The first one meets the diagnostic criteria for social phobia described in DSM-IV and ICD-10 (type 1 SP). On the other hand, the clinical manifestations

Virtual reality exposure therapy.

It has been proposed that virtual reality (VR) exposure may be an alternative to standard in vivo exposure. Virtual reality integrates real-time computer graphics, body tracking devices, visual displays, and other sensory input devices to immerse a participant in a computer-generated virtual environment. Virtual reality exposure is potentially an efficient and cost-effective treatment of anxiety disorders. VR exposure therapy reduced the fear of heights in the first controlled study of virtual reality in treatment of a psychiatric disorder. A case study supported the efficacy of VR exposure th

Panic disorder and agoraphobia: fear of fear or fear of the symptoms produced by hyperventilation?

Two versions of the fear-of-fear hypothesis of panic disorder are discussed. The fear-of-the-somatic-effects-of-fear version, which is distinguished from the classical conditioning version, is compared with the hyperventilation theory of panic disorder and agoraphobia. The fear-of-the-somatic-effects-of-fear hypothesis is criticized on the basis of its inability to explain adequately (a) the initiation of panic attacks, (b) the growth in intensity of panic attacks, and (c) the termination of panic attacks. The tenability of the hyperventilation theory is supported by evidence from programs of
