Preoperative Antibiotic Prophylaxis in Elective Minor Surgical Procedures among Adults in Southern Italy.

Little is known regarding the factors associated with surgical antibiotic prophylaxis (SAP) compliance in elective minor surgery. The purposes of this cross-sectional study were to identify the frequency of inappropriate SAP administration and to understand the characteristics associated with such inappropriateness in a sample of elective minor surgical procedures. The study was performed between May and July 2019 among a random sample of patients aged 18 years and older in seven public hospitals randomly selected in the Campania and Calabria Regions of Italy. Globally, only 45% of SAP approac

Gunshot Injury to Hand.

The anatomic complexity of the hand and the proximity of its structures make gunshot injuries especially prone to severe, multi-tissue damage. These injuries often involve multiple structures, including bones, tendons, nerves, and skin, simultaneously, requiring individualized and frequently staged treatment. Various reconstructive strategies are available for each affected structure. In this article, we provide an overview of treatment choices and a representative case that illustrates the clinical reasoning behind selecting among these alternatives in the context of a complex hand gunshot in

The supraclavicular artery island flap in maxillofacial reconstruction: A technical note.

INTRODUCTION: The supraclavicular artery island flap, initially described by Lamberty and Cormack in 1979 and subsequently refined by Pallua, is now a well-established option in cervicofacial reconstruction. Owing to its thin, pliable, and well-vascularized skin, it offers favorable color and texture match for facial and intraoral defects while limiting donor-site morbidity. TECHNICAL NOTE: This technical note describes the design, elevation, tunneling, and inset of the supraclavicular artery island flap for cervicofacial and intraoral reconstruction. Emphasis is placed on key anatomical land

Unilateral Cleft Lip: Primary Repair.

The unilateral cleft lip deformity is characterized by uncoupled premaxillary growth leading to distinct nasolabial anatomic changes. Careful analysis of the cleft deformity is important for surgical design. Several techniques have been developed for cleft lip repair. The surgeon should select the method that compliments their style, working consistently in their hands. The best predictor of long-term outcome is the final appearance, balance, and harmony in the operating room. Meticulous photographic and anthropometric records will aid the developing surgeon in improving their technique.

Decision Tree for Reconstructive Treatment of Peri-implantitis Defects.

Peri-implantitis is a common complication among patients receiving implant-supported restorative therapy, and it often requires surgical intervention for effective treatment. Understanding the specific configuration of the peri-implant bony defect and adjacent bone peaks is crucial for tailoring treatment strategies and improving outcomes. A decision tree for reconstructive peri-implantitis therapy has been developed based on the new classification of defect configurations (Classes I to V), guiding clinicians in selecting treatment options, including biomaterials, techniques, and healing appro

Alloplastic implants for skeletal augmentation in Parry-Romberg syndrome: A systematic review.

Parry-Romberg syndrome (PRS) is a rare disorder characterized by progressive hemifacial atrophy affecting soft tissue and, in severe cases, bone. Reconstructive surgery is commonly used to manage PRS-related facial deformities. This systematic review assesses the current literature on alloplastic implants for facial reconstruction in PRS. Following PRISMA guidelines, we conducted a systematic review across MEDLINE, Google Scholar, and Cochrane databases, including English-language studies on PRS managed with alloplastic implants. Eleven studies, including 74 PRS patients (age range 12-45, mean

Cleft Lip.

Effective management of cleft lip requires a comprehensive interdisciplinary approach that addresses the multifaceted dimensions of care. Although treatment spans infancy to adulthood and outcomes are influenced by growth and development, the critical stage of care is at primary reconstruction. This review aims to integrate recent insights from data-driven 3D analysis of cleft lip and nasal deformities into our foundational approaches for primary reconstruction of both unilateral and bilateral cleft lip. It also aims to organize the many methods of repair of both lip and nose according to prin

Unilateral cleft lip repair: Standardized key steps for Cupid's bow reconstruction.

Cleft lip repair is a complex and technically demanding surgery, performed by skilled surgeons. Improving surgical outcomes in cleft lip repair offers substantial benefits to patients, yet it remains a persistent challenge for surgical teams. To address this challenge, we developed a systematic protocol based on 40 years of refinements in 7400 cases. By deconstructing the procedure into defined steps, this framework aims to reduce procedural variability while preserving flexibility for case-specific adaptations. The stepwise approach was applied to establish a surgical procedure for Cupid's bo

[Spring-assisted cranioplasty for children with craniosynostosis].

Premature closure of the sutures, craniosynostosis, can lead to severe complications when untreated. Spring-assisted cranioplasty has some advantages compared to calvarial vault remodelling e.g. less blood loss and reduced operative time. In this review, we describe the novel treatment of nonsyndromic sagittal synostosis with spring-assisted cranioplasty and compare it to cranial vault remodelling, which has been the standard treatment for many years.

Surgical Management of Unilateral Cleft Lip.

Unilateral cleft lip is a common congenital defect of the head and neck, which ranges from incomplete to complete forms, with or without cleft palate. Early intervention is crucial to address feeding difficulties and unwanted sequalae. Advanced ultrasonography enables prenatal diagnosis and coordinated planning with multidisciplinary cleft repair teams. Postnatal care includes feeding strategies, growth monitoring, and early interventions like nasoalveolar molding. In this article, surgical techniques, such as Millard, modified Millard or Mohler, and Fisher, are highlighted. Emphasis is placed

Preaxial Polydactyly Reconstruction in Adults: A Report of 3 Cases.

CASE: Preaxial polydactyly is typically addressed early in childhood when treated operatively. We present 3 cases of surgical treatment of preaxial polydactyly in adulthood (Wassel/Flatt Types I, III and IV). CONCLUSION: Similar technical principles may be used for management of preaxial polydactyly in pediatric and adult patients. However, as physeally mature individuals have decreased osseous remodeling and decreased cortical plasticity in comparison with children, adult patients should be counseled preoperatively regarding the potential differences in outcomes in comparison with similar te

Conjoined Twins.

Conjoined twins have long been of interest to the medical community. Evaluation is complex and tailored to the unique variant. A thorough multidisciplinary work-up, including surgical specialties, nursing, anesthesia, and critical care, must be performed prior to proceeding with surgical separation. Outcomes have become more predictable with certain advances such as tissue expansion and three-dimensional modeling. A number of important lessons learned from previous cases can be used to improve future outcomes.

Tessier Facial Clefts.

Less-common congenital facial clefts were originally described by Tessier and bear his name and classification system. These clefts are limited to the primary and secondary palate and involve other regions of the face other than simply the lips, palate, and nose. They are also among the more complex of facial anomalies, affecting both soft tissues and skeletal structures, which presents difficulties for surgical correction.

[Expert consensus on skull defect reconstruction after decompressive craniectomy in children (2025 edition)].

Children with skull defects after decompressive craniectomy face issues such as lack of brain protection and aesthetics during the recovery period, which also affect the recovery of brain structure and function and hinder subsequent rehabilitation. The skull of pediatric patients is still in the growth and development stage, with head circumference changing with age, and there are characteristics such as easy bone hyperplasia and absorption. Therefore, the timing, materials, methods and other problems of skull reconstruction for pediatric patients have always been puzzling clinical practice. T

Masquelet's induced membrane technique in the upper limb: a systematic review of the current outcomes.

BACKGROUND: The Masquelet induced membrane technique is a surgical procedure that allows the reconstruction of segmental bone defects using a relatively simple approach that requires minimal resources from both the healthcare facility and the patient. Historically applied to the lower limb, this technique is gaining increasing attention in the literature for its use in the upper limb. METHODS: A systematic review of the literature was conducted using the PubMed and Google Scholar databases to identify all studies reporting the outcomes of the Masquelet induced membrane technique in the long b

CONTEMPORARY CONCEPTS AND SURGICAL TECHNIQUES IN FACIAL REANIMATION: A SCOPING REVIEW.

Paralysis of the facial nerve represents an exceptional functional, social and cos- metic problem for the patient. Loss of function directly leads to the inability to protect the eyes, as well as oral competence, while indirectly affecting breathing and verbal communication. Patients often encounter social stigmatization and isolation, which is direct consequence of the loss of mimic function and disfiguration that is necessary for everyday nonverbal communication. Thus, the quality of life is strongly impaired, and patients are prone to developing depression. The primary goal of reanimation i
