[Cavernous transformation of portal vein].

Cavernous transformation of the portal vein occurs with long-standing portal vein thrombosis (PVT) because of the development and dilatation of multiple small vessels in and around the recanalising main portal vein. Thrombosis and occlusion of the portal vein leads to portal hypertension with enlarged spleen and the development of porto-systemic collaterals. The main clinical presentation is gastroesophageal variceal bleeding and hematologic abnormalities due to splenomegaly (hyperspleenismus-pancytopenia). We described the young patient with splenomegaly and extensive cavernous transformation

Elevated portal vein drug levels of sirolimus and tacrolimus in islet transplant recipients: local immunosuppression or islet toxicity?

The recent success of islet transplantation using the Edmonton protocol involved the use of sirolimus, tacrolimus, and daclizumab for immunosuppression. Islets were infused into the portal circulation after transhepatic access. This protocol provided a unique opportunity to measure sirolimus and tacrolimus levels from the portal vein and compare them to systemic venous levels. A total of 11 portal venous samples with a corresponding peripheral venous sample were obtained from patients undergoing a first or second islet infusion and medication levels were obtained on both types of specimens. Evaluation of Risk to the Suprascapular Nerve During Arthroscopic SLAP Repair: Is a Posterior Portal Safer?

PURPOSE: The purpose of this study was to compare the risk of glenoid perforation during SLAP repair for suture anchors placed through an anterolateral portal versus a posterolateral portal of Wilmington. METHODS: Ten bilateral cadaveric shoulders were randomized to suture anchor placement through an anterolateral portal on one shoulder and a posterolateral portal on the contralateral shoulder. Anchors were placed into anterior, posterior, and far posterior positions on the glenoid rim (1 o'clock, 11 o'clock, and 10 o'clock positions for right shoulders). The shoulder was then dissected, and

Real-time Cherenkov emission portal imaging during CyberKnife® radiotherapy.

The feasibility of real-time portal imaging during radiation therapy, through the Cherenkov emission (CE) effect is investigated via a medical linear accelerator (CyberKnife(®)) irradiating a partially-filled water tank with a 60 mm circular beam. A graticule of lead/plywood and a number of tissue equivalent materials were alternatively placed at the beam entrance face while the induced CE at the exit face was imaged using a gated electron-multiplying-intensified-charged-coupled device (emICCD) for both stationary and dynamic scenarios. This was replicated on an Elekta Synergy(®) linear accele

Therapeutic approaches for portal biliopathy: A systematic review.

Portal biliopathy (PB) is defined as the presence of biliary abnormalities in patients with non-cirrhotic/non-neoplastic extrahepatic portal vein obstruction (EHPVO) and portal cavernoma (PC). The pathogenesis of PB is due to ab extrinseco compression of bile ducts by PC and/or to ischemic damage secondary to an altered biliary vascularization in EHPVO and PC. Although asymptomatic biliary abnormalities can be frequently seen by magnetic resonance cholangiopancreatography in patients with PC (77%-100%), only a part of these (5%-38%) are symptomatic. Clinical presentation includes jaundice, cho

Portal vein aneurysm, a pathological entity or an innocent bystander? A review of six cases.

Portal venous aneurysms (PVAs) are a rare venous aneurysms. The mean diameter of a healthy portal vein varies considerably, with maximum diameter of 15 mm in healthy subjects and 19 mm in cirrhotic patients. The presentation varies; they could come with abdominal pain or more often as an incident imaging finding. Although risk factors like portal hypertension and liver cirrhosis have been highlighted, the aetiology remains to be clarified. PVA may be associated with various complications: thrombosis, aneurysmal rupture, inferior vena cava obstruction or duodenal compression. A conservative tre

A case of portal venous gas caused by the use of a nasojejunal nutrition tube.

We reported a case of portal vein pneumatosis caused by a nasojejunal nutrition tube that was successfully treated conservatively. This patient was discharged after 1 week of treatment with imipenem/cilastatin sodium for anti-infection, gastrointestinal decompression, fluid replacement, and pain relief, and the patient's abdominal pain was significantly relieved. The main pathogenesis of hepatic portal venous gas is as follows: (a) increased pressure in the digestive tract leading to gas entering the portal vein circulation through the intestinal wall and finally into the liver; (b) destructio

Management of Extrahepatic Portal Vein Obstruction With Spleno-Adrenal Shunt.

Extrahepatic portal hypertension is the hypertension of the portal venous system in the absence of liver cirrhosis, and variceal bleeding is its commonly seen complication. Long-standing portal hypertension will have the risk of symptomatic hypersplenism, portal biliopathy, growth failure, and ectopic varices. Portosystemic shunt surgery can more effectively manage these complications than medical and endoscopic management. Conventional portosystemic shunts, especially the proximal splenorenal shunt (PSRS), are preferred over unconventional shunts, as the latter will have increased procedural

Meso-Rex bypass as an alternative technique for portal vein reconstruction at or after liver transplantation in children: review and perspectives.

Direct portal revascularization can be achieved by interposing a vascular graft between the SMV and the Rex recessus (left portal vein system): the MRB. To review indications and results of the procedure in the setting of pediatric liver transplantation, reports were selected from the English literature. Previously reported series were updated to analyze long-term outcome. A new series was added and analyzed as a complementary set of cases. A total of 51 cases were analyzed. With a 96% overall patient survival rate and a 100% long-term patency rate when the IJV is used for the bypass, MRB achi

Does umbilical vein catheterization to exchange transfusion lead to portal vein thrombosis?

UNLABELLED: The aetiopathology of extrahepatic portal vein obstruction is unknown. In retrospective studies, umbilical vein cannulation and sepsis have been alleged to cause portal thrombosis. This prospective study was undertaken to detect whether thrombosis and consequent obstruction of the splenoportal venous system develops after umbilical vein catheterization for exchange transfusion in newborns using Doppler ultrasound. Forty children (M = 24; F = 16) who had undergone exchange transfusion for hyperbilirubinaemia were studied at school age. Maximal duration of the venous umbilical cannul

Masked polycythaemia vera in a patient with extrahepatic portal venous obstruction.

Myeloproliferative disorder (MPD) is an important cause of thrombosis of the hepatic and portal venous system. The diagnosis in many of these patients is missed as they have an atypical clinical presentation and may have a normal haematological profile at presentation. We report a 30-year-old patient with features of portal hypertension due to extrahepatic portal venous obstruction. She had a normal haemoglobin level and haematocrit at presentation, but the red cell mass was found to be elevated even in the presence of low serum iron levels. A diagnosis of polycythaemia vera as the underlying

Portal hypertension in Felty's syndrome.

Idiopathic portal hypertension, usually with esophageal varices, minimal but definitely abnormal liver tests and subtle or no changes on percutaneous liver biopsy has been reported in only a few patients. A case associated with Felty's syndrome in a 65-year old woman with a 44-year history of rheumatoid arthritis is reported in which the liver tests and liver biopsy were normal. Portal hypertension, varices and a patent portal venous system were documented. No surgical procedure was undertaken and she has done well in the 18 months since these studies were performed. Extensive portal vein thrombosis related to abdominal trauma.

Abdominal trauma is a classic but very rare cause of portal vein thrombosis. We report the case of a patient with portal vein thrombosis and cavernoma associated with portal hypertension. Anamnesis identified a serious thoraco-abdominal trauma related to a bicycle accident 6 months before. Biological screening identified an inherited heterozygous G20210A factor II gene mutation which supports a recent notion that portal vein thrombosis most often occurs when both local and systemic aetiological factors are combined.

Sonographic diagnosis of hepatic vascular disorders.

Perhaps the most valuable application of duplex sonography in the abdomen is in the diagnosis of vascular disorders of the liver. Duplex sonography provides an accurate assessment of the portal and hepatic venous systems that is both noninvasive and convenient, because the examination can be conducted at the bedside. In this article, the normal features of portal and hepatic venous flow are considered as well as the sonographic manifestations of pathological conditions, including: (1) portal hypertension; (2) portosystemic collaterals; (3) portal vein thrombosis; and (4) hepatic vein thrombosi

Portal cytokine response and metabolic markers in the early stages of abdominal sepsis in pigs.

BACKGROUND: The portal vein could play a major role in disseminating the local inflammation of acute bacterial peritonitis since it is responsible for the venous drainage of the gastrointestinal tract. We hypothesized that after peritoneal exposure to Escherichia coli, a gradient between the portal and systemic levels of cytokines would be expected. METHODS: Acute peritonitis was induced by depositing 200 ml of broth with live E. coli in the peritoneal cavity of the animals in the B-group (n = 7). They were then observed for 4 h and compared with a control group (C-group, n = 7). Tumour necro

Endothelin-1 levels in portal venous blood in relation to hepatic tissue microcirculation disturbance and hepatic cell injury after ischemia/reperfusion.

This study was conducted to clarify the role of endothelin-1 in the portal vein after hepatic ischemia/ reperfusion and to ascertain whether it is related to hepatic microcirculation disturbance. Using a canine ischemic liver model, the portal and systemic endothelin-1 levels were measured before ischemia, then after 1 h and 2 h of reperfusion, and comparatively evaluated with the serum levels of GOT and lactic dehydrogenase (LDH). As an indicator of liver tissue microcirculation, tissue blood flow volume (TBF) was also measured in the site subjected to ischemia. The animals were divided into:
