A case is presented of priapism resulting from disseminated intravascular coagulopathy (DIC), which was diagnosed by pathological studies of the amputated penis and skin biopsy. To our knowledge, this is the first case reported in Japan. Presented is a case of idiopathic priapism that was successfully treated with inhalation of amyl nitrite. Routine treatment measures had failed. The same patient presented on several other occasions in a similar fashion. Each time amyl nitrite was successful. An investigation for diseases known to be associated with priapism proved negative. Due to the frequency of recurrences, definitive surgical management was offered. The patient refused and has been lost to follow up.

Caverno-glandular shunt was performed in 8 patients with priapism during the past 3 years. Complete and sustained detumescence was achieved immediately in all patients. Only one case developed a complication, viz. cavernositis which responded to antibiotic administration systemically as well as locally. This procedure was performed under local anaesthesia in 5 patients. The technical details and advantages of this procedure over conventional operations for priapism are discussed.

Priapism is a pathological and mostly painful permanent penile erection. Typically only both corpora cavernosa and not the corpus spongiosum urethrae and the glans penis are involved. The vascular and nervous preconditions for a normal erection are discussed and a new concept presented. On this basis the conditions for a priapism are enumerated and the principles of therapy are discussed.

A questionnaire study of Jamaican patients with homozygous sickle cell (SS) disease indicated a 42% prevalence of priapism, with a median age at onset of 21 years. Two predominantly different patterns of priapism were recorded: short "stuttering" episodes lasting less than three hours, with normal consequent sexual function, and severe prolonged attacks (generally more than 24 hours) commonly followed by impotence. The paper reports 6 cases of priapism with subsequent protrusion and paresis after neuroleptanalgesia and anaesthesia in horses. Five were ultimately treated by amputation of the penis but the sixth horse is responding satisfactorily to conservative treatment at the time of writing. Causative factors, preventive measures and possible methods of treatment are discussed.

Priapism was associated with multiple myeloma and hyperviscosity in a sixty-year-old black male. Plasmapheresis treatment of the hyperviscosity corrected his priapism, and chemotherapy for the underlying multiple myeloma prevented the recurrence of priapism after a chronic intermittent history of seventeen years.

A 10-year-old white boy is described in whom priapism developed as a complication of parotitis without orchitis. Medical study failed to reveal any other etiology. The syndrome was assumed to be related to a mumps virus. Treatment by corpora cavernosa-spongiosum shunt relieved the acute symptoms as well as preserved the patient's ability to have an erection.
