[New recommendations on palliative sedation].

Palliative sedation aims to relieve refractory suffering in patients with life-limiting disease. The 2009 framework on palliative sedation of the European Association for Palliative Care (EAPC) has recently been updated. Recommendations have also been formulated by the research group SedPall in Germany. This article describes the social and ethical complexity of decision-making and summarises the recommendations. Patient autonomy is emphasised. Refractoriness of the suffering should be determined jointly by physician and patient. Sedation should be proportional, that is to say, its form and du

Propofol and non-propofol based sedation for outpatient colonoscopy-prospective comparison of depth of sedation using an EEG based SEDLine monitor.

Propofol is a popular anesthetic sedative employed in colonoscopy. It is known to increase the patient satisfaction and improve throughput. However, there are concerns among the clinicians with regard to the depth of sedation, as a deeper degree of sedation is known to increase the incidence of aspiration and other adverse events. So we planned to compare the depth of sedation between propofol and non-propofol based sedation in patients undergoing outpatient colonoscopy, as measured by an electroencephalogram (EEG) based monitor SEDLine monitor (SedlineInc., San Diego, CA). The non-randomized

Hemodynamic Safety of Dexmedetomidine-Fentanyl Sedation During TAVI.

BACKGROUND: Transcatheter aortic valve implantation (TAVI) is an established alternative for patients with severe aortic stenosis who are unsuitable for surgical valve replace-ment. Conscious sedation is preferred to preserve spontaneous respiration and patient cooperation. Dexmedetomidine, a selective α2-adrenergic agonist, provides sedation, analgesia, and sympatholysis with minimal respiratory depression, making it suitable for high-risk TAVI patients. METHODS: We retrospectively analyzed 53 patients who underwent TAVI under dexme-detomidine-based sedation at a single center between Januar

Baseline Cannabinoid Use Is Associated with Increased Sedation Requirements for Outpatient Endoscopy.

Background and Aims: Given the underlying properties of cannabinoids, we aimed to assess associations between cannabinoid use and sedation requirements for esophagogastroduodenoscopy (EGD) and colonoscopy. Methods: A prospective cohort study was conducted at three endoscopy units. Adult outpatients undergoing EGD or colonoscopy with endoscopist-directed conscious sedation (EDCS) were given questionnaires on cannabinoid use and relevant parameters. Outcomes included intraprocedural midazolam, fentanyl, and diphenhydramine use, procedural tolerability, and adverse events. Multivariable logistic

Early Cognitive Function after Deep Sedation Using Different Anesthetic Agents in Pediatric Patients: A Prospective, Randomized Controlled Trial.

Background and Objectives: The impact of anesthetic agents on memory and cognitive function following general anesthesia is of great interest, particularly regarding their effects on the developing pediatric brain. While numerous studies have examined the relationship between anesthetic drugs and brain function, research focusing on early cognitive function following sedation remains limited. Materials and Methods: This study was a prospective, randomized controlled trial involving 148 pediatric patients scheduled for hematological procedures, specifically bone marrow aspiration (BMA) and intr

Safely Sedating Children and Adolescents Who Are Neurodivergent.

Children who are neurodivergent have higher rates of injury, number of health care visits, and perceived need for sedation compared with their peers who are not neurodivergent, resulting in unique medical care challenges for this patient population. Sedation of children who are neurodivergent is challenging due to increased risk of adverse events, medication interactions, and behavioral complexities. Safe and effective sedation requires individualized planning, careful agent selection, and incorporation of nonpharmacologic strategies. Significant variability exists in sedation practices for ch

Capnography Should Be Mandatory Monitoring For Moderate Sedation.

Background There has been increasing concern by the American Dental Association, state dental boards, regional legislators, and specialty groups about the current state of dental anesthesia. Specific interest has surrounded methods to improve patient safety during parental sedation and anesthesia. Many times in the history of dental anesthesia, monitoring advances begin in the hospital for general anesthesia, then downscale, become smaller, and find utility in outpatient anesthesia. Monitoring advances ultimately have been shown to improve patient safety and are subsequently universally adopte

In response to the letter by Cabadas and Álvarez-Escudero about the editorial: "Will societies of anesthesiologists partake in the take-off of non-anesthesiologist administration of propofol?".

I thank R. Cabadas and J. Álvarez-Escudero (1). 1. Apologies accepted. 2. The statistical significance of the difference in proportions seems doubtful. 3. Although these small studies do not allow us to confirm the safety of endoscopist-directed propofol sedation, they deserve a short comment as they represent the best available level of evidence (RCTs) and are also informative. 4. Hypoxemia, hypotension and bradycardia are frequent and have no clinical consequences in most cases. 5. The authors started this chapter by stating "scientific opinions must be carefully based" (3); 6. As quotation

Factors associated with satisfaction with oral sedation during ophthalmic surgeries.

PURPOSE: To investigate patient and surgical characteristics associated with higher patient, surgeon, or anesthesia provider satisfaction with oral sedation in ophthalmic procedures. DESIGN: This was a secondary analysis of a prospective, randomized, double-masked, non-inferiority clinical trial measuring patient satisfaction comparing oral versus intravenous (IV) sedation for ophthalmic surgeries. METHODS: Data was collected from a validated 6-point satisfaction survey from patients, surgeons, and anesthesia providers. We focused on the oral sedation arm and used multivariate regression ana

Ultrasound-Guided Retroclavicular Approach to the Infraclavicular Region (RAPTIR) Brachial Plexus Block for Anterior Shoulder Reduction.

BACKGROUND: Shoulder dislocations are a common presenting injury to the emergency department (ED), with anterior dislocations comprising the majority of these cases. Some patients may tolerate gentle manipulation and reduction, but many require analgesia of some type. Oral or parenteral pain medication is often used alone or in combination with procedural sedation if gentle manipulation fails to achieve reduction. Recently, this treatment algorithm has grown to include regional anesthesia as a mode of analgesia for reduction of shoulder dislocations in the form of brachial plexus blocks. It ha…

Preventing Hypoxia in Pediatric MRI Sedation: A Randomized Controlled Trial of Prophylactic Nasopharyngeal Airway Placement.

BACKGROUND: Deep procedural sedation for magnetic resonance imaging (MRI) is associated with airway collapse, causing hypoxia. The nasopharyngeal airway (NPA) is a simple rescue device, but there is limited literature on its prophylactic use to prevent hypoxia. This study evaluates the incidence of hypoxic episodes with and without prophylactic NPA placement in children undergoing MRI under sedation. METHODS: Two hundred and sixty-two American Society of Anesthesiologists (ASA) I and II children aged 1-10 years scheduled for MRI under sedation were randomized into the NPA and control groups. …

Outcomes of Non-Anesthetist Led Conscious Sedation for 2000 Transcatheter Aortic Valve Implantations.

BACKGROUND: A minimalist approach to transcatheter aortic valve replacement (TAVR) has accompanied the increase in TAVR procedures worldwide. Comprehensive evidence regarding the safety of non-anesthetist-led conscious sedation for TAVR is lacking. AIMS: We aimed to evaluate the outcomes of patients undergoing TAVR with non-anesthetist-led conscious sedation. METHODS: This retrospective analysis included consecutive patients who underwent percutaneous transfemoral TAVR with non-anesthetist-led conscious sedation, from March 2018 to 2025, in a single high-volume center. Thirty-day outcomes we

The Safety and Efficacy of Remimazolam Compared to Dexmedetomidine for Awake Tracheal Intubation by Flexible Bronchoscopy: A Randomized, Double-Blind, Controlled Trial.

BACKGROUND: Remimazolam is a novel ultra-short-acting benzodiazepine sedative that has the potential to be an alternative for procedural sedation due to its rapid sedation and recovery, no accumulation effect, stable hemodynamics, minimal respiratory depression, anterograde amnesia effect, and specific antagonist. Here, we aimed to compare the safety and efficacy of remimazolam with dexmedetomidine for awake tracheal intubation by flexible bronchoscopy (ATI-FB). METHODS: Ninety patients scheduled for ATI-FB were randomly divided into three groups, each consisting of 30 cases: dexmedetomidine …

Feasibility of a cardiologist-only approach to sedation for electrical cardioversion of atrial fibrillation: a randomized, open-blinded, prospective study.

BACKGROUND/OBJECTIVES: Sedation with propofol should be administered by personnel trained in advanced airway management. To overcome this limitation, the use of short acting benzodiazepines by cardiologists spread widely, causing concerns about the safety of this procedure in the absence of anesthesiology assistance. The aim of the study was to compare feasibility of a cardiologist-only approach with an anesthesiologist-assisted sedation protocol during elective direct-current cardioversion (DCC) of persistent atrial fibrillation (AF). METHODS: This prospective, open-blinded, randomized study

Imaging and Interventional Radiology Capabilities in Extreme Cold Environments: Operational Implications for Military Medicine.

INTRODUCTION: Extreme cold environments impose unique constraints on the delivery of image-guided and interventional care due to hypothermia-associated coagulopathy, altered drug metabolism, impaired operator performance, and temperature-sensitive imaging and device systems. These challenges are particularly relevant to military operations in polar and austere settings, where evacuation may be delayed, and medical capability must be delivered with limited infrastructure. MATERIALS AND METHODS: This work presents an operationally focused review of the physiologic effects of cold exposure, imag

Nitrous oxide analgesia for rehabilitation after anterior cruciate ligament reconstruction: A randomized controlled trial.

BACKGROUND: Although there is increasing emphasis on rehabilitation training after ligament reconstruction, little is known about the pain induced by the procedure itself. Procedural success may be limited by pain and anxiety. Nitrous oxide is widely used to alleviate procedural pain. However, few studies have been conducted to show the efficacy and safety of nitrous oxide for rehabilitation training. OBJECTIVES: To explore the short-term efficacy and safety of nitrous oxide for acute pain elicited by rehabilitation training. METHODS: A double-blinded randomized placebo-controlled trial was
