We have studied the response of prolactin (PRL) secretion to the test combining i.m. sulpiride in the dose 1 mg/kg followed by 200 micrograms i.v. of TRH, in 12 normal women and 37 patients with hyperprolactinaemia. The response was expressed as a percentage rise in plasma PRL concentration (delta %) 20 minutes after the administration of sulpiride or TRH. In the controls the response in PRL secretion to sulpiride worked out at between 639 and 2,760%. We studied the influence of pregnancy on the serum prolactin levels after surgery in 35 patients with prolactinomas. Eighteen out of 22 patients with postoperative normalization of the prolactin level were able to achieve repeated spontaneous pregnancies and endocrine cure by surgery alone, but the other 4 had recurrent hyperprolactinemia. Two surges of PRL designated as nocturnal (N) and diurnal (D) are observed during early pregnancy. The present study was undertaken to determine the involvement of serotonin (5-HT) in regulating these PRL surges. The effects of progabide, a GABA receptor agonist, on haloperidol-induced and domperidone-induced hyperprolactinaemia have been studied in man. There was a moderate inhibition by progabide of the rise in prolactin after domperidone. No such effect was found after haloperidol. The results support the view that in man, as in animals, GABA agonists have an inhibitory effect on prolactin release, probably at the level of the hypothalamus and anterior pituitary.
