Dacron ingrowth during penile prosthesis revision: presentation, outcomes, and surgical management.

The Boston Scientific American Medical Systems 700™ (AMS 700™, Boston Scientific Corporation, United States of America) inflatable penile prostheses feature an inner Dacron layer that facilitates controlled expansion. Over time, outer silicone rupture exposes the Dacron, allowing for tissue ingrowth and complicating explantation. We herein describe our experience managing this situation in a retrospective, single-surgeon case series. Operative reports of all AMS 700™ inflatable penile prostheses explants from 2011 to 2025 (CPT 54406, 54410, 54411, 54416, 54417) of a single surgeon at one insti

Biomechanical Evaluation of Different Implant-Abutment Connections, Retention Systems, and Restorative Materials in the Implant-Supported Single Crowns Using 3D Finite Element Analysis.

This is an in silico study aimed to evaluate the biomechanical influence of different implant-abutment interfaces (external hexagon and Morse taper implants), retention systems (cement and screw retained), and restorative crowns (metal-ceramic and monolithic) using 3-dimensional finite element analysis (3D-FEA). Eight 3D models were simulated for the maxillary first molar area using InVesalius, Rhinoceros, and SolidWorks and processed using Femap and NEi Nastran software. Axial and oblique forces of 200 and 100 N, respectively, were applied on the occlusal surface of the prostheses. Microstrai

Biofilm in implant infections: its production and regulation.

A significant proportion of medical implants become the focus of a device-related infection, difficult to eradicate because bacteria that cause these infections live in well-developed biofilms. Biofilm is a microbial derived sessile community characterized by cells that are irreversibly attached to a substratum or interface to each other, embedded in a matrix of extracellular polymeric substances that they have produced. Bacterial adherence and biofilm production proceed in two steps: first, an attachment to a surface and, second, a cell-to-cell adhesion, with pluristratification of bacteria o

Osteomyelitis Risk in Patients With Transfemoral Amputations Treated With Osseointegration Prostheses.

BACKGROUND: Percutaneous anchoring of femoral amputation prostheses using osseointegrating titanium implants has been in use for more than 25 years. The method offers considerable advantages in daily life compared with conventional socket prostheses, however long-term success might be jeopardized by implant-associated infection, especially osteomyelitis, but the long-term risk of this complication is unknown. QUESTIONS/PURPOSES: (1) To quantify the risk of osteomyelitis, (2) to characterize the clinical effect of osteomyelitis (including risk of implant extraction and impairments to function)

Long-term outcomes of proximal interphalangeal joint surface replacement arthroplasty.

BACKGROUND: Surface replacement arthroplasty is a reconstructive alternative for the treatment of pain and deformity due to osteoarthritis and rheumatoid arthritis of the proximal interphalangeal joint of the finger. This retrospective study was performed to examine long-term outcomes of proximal interphalangeal joint prosthetic surface replacement with a proximal cobalt-chromium (CoCr) and distal ultra-high molecular-weight polyethylene component over thirty years at a single institution. METHODS: Sixty-seven prostheses were implanted in forty-seven patients between 1974 and 2007. The mean d

Ten-year follow-up of a total knee prosthesis combining multi-radius, ultra-congruency, posterior-stabilization and mobile-bearing insert shows long-lasting clinically relevant improvements in pain, stiffness, function and stability.

PURPOSE: There is a paucity of data available on total knee prostheses combining dual-radius, ultra-congruency, posterior-stabilization and mobile-bearing insert. This prospective cohort study aimed to assess the clinical evolution of the FIRST® prosthesis (Symbios Orthopédie, Yverdon, CH), the earliest prosthesis with this particular design. It was hypothesized that the primary outcomes, evaluating pain, stiffness, function and stability, would improve following arthroplasty and remain improved during the follow-up period of 10 years. METHODS: All patients programmed for a total knee arthrop

Results of the Volar Approach in Proximal Interphalangeal Joint Arthroplasty.

BACKGROUND: Most hand surgeons use a dorsal approach for proximal interphalangeal (PIP) joint implant arthroplasty. However, a volar approach offers the advantage of no disturbance to the extensor mechanism, thus allowing early initiation of active range of motion. We examined our results in patients who underwent PIP joint arthroplasty via a volar approach. METHODS: Using a retrospective chart review, we evaluated the outcomes of patients undergoing PIP joint arthroplasty through a volar approach between 2001 and 2005 by 3 fellowship-trained hand surgeons at our institution. The indication f

Clinical, immunological and bacteriological evaluation of adverse reactions to skin-penetrating titanium implants in the head and neck region.

Between 1977 and October 1989, 445 patients have been treated with bone-anchored skin-penetrating titanium implants for anchorage of facial prostheses or bone-conducting hearing aids, at the Ear, Nose and Throat Department at Sahlgren's Hospital in Gothenburg. The majority of patients had no adverse skin reactions, while a few patients were responsible for the majority of the adverse reactions. The aim of our study was to analyse differences between these groups. We started a clinical study on 9 patients with a clinical history of adverse skin reactions around the titanium implants and 9 patie

MRI of Penile Prostheses: The Challenge of Diagnosing Postsurgical Complications.

Penile implants are surgically inserted devices used for the treatment of erectile disfunction. Improvements in surgical techniques and in the manufacturing of these implants have reduced the risk of intra- and postoperative complications and increased the popularity and use of this therapeutic option. Routine imaging is not recommended before penile prosthesis surgery. Malleable penile prostheses use noninflatable shafts inserted into each corpus cavernosum, which appear hypointense on T1- and T2-weighted MR images. Inflatable penile prostheses (IPPs) may consist of two or three parts and are

Clinical outcomes in high flexion total knee arthroplasty were not superior to standard posterior stabilized total knee arthroplasty. A multicenter, prospective, randomized study.

High flexion prostheses have been introduced to achieve high flexion and improve clinical outcomes. Controversy exists in the literature regarding outcomes of high flexion vs. standard implants. This multicenter study compares outcomes in patients receiving a high flexion prosthesis vs. standard prosthesis. 278 high flexion and standard knee prostheses were used. Patients were followed for two years and evaluated prospectively. The mean HSS was 87.3 for the standard group and 88.9 for the flexion group. At two-year follow up the standard prosthesis group had mean flexion of 121° and the high f

Total knee arthroplasty in valgus knees: predictive preoperative parameters influencing a constrained design selection.

INTRODUCTION: In valgus knees, ligament balance might remain a challenge at total knee prostheses implantation; this leads some authors to systematically propose the use of constrained devices (constrained condylar knee or hinge types...). It is possible to adapt the selected level of constraints, by reserving higher constraints to cases where it is not possible to obtain final satisfactory balance: less than 5 degrees of residual frontal laxity in extension in each compartment, and a tibiofemoral gap difference not in excess to 3 mm between flexion and extension. HYPOTHESIS: It is possible t

Silicone radial head prostheses revisited: do they have a role in today's practice? A systematic review of literature on clinical outcomes.

BACKGROUND: Silicone radial head prostheses (SRHP) are considered obsolete due to reports of frequent failure and destructive silicone-induced synovitis. Considering the good outcomes of modern non-radial silicone joint implants, the extent of scientific evidence for this negative view is unclear. The aim of this research was to systematically analyze the clinical evidence on complications and outcomes of SRHP and how SRHP compare to both non-SRHP and silicone prostheses of other joints. METHODS: A systematic literature review was conducted through the Cochrane, PubMed, and Embase databases.

Role of polyethylene particles in peri-prosthetic osteolysis: A review.

There is convincing evidence that particles produced by the wear of joint prostheses are causal in the peri-prosthetic loss of bone, or osteolysis, which, if it progresses, leads to the phenomenon of aseptic loosening. It is important to fully understand the biology of this bone loss because it threatens prosthesis survival, and loosened implants can result in peri-prosthetic fracture, which is disastrous for the patient and presents a difficult surgical scenario. The focus of this review is the bioactivity of polyethylene (PE) particles, since there is evidence that these are major players in

[Progress on the relationship between wear debris-induced apoptosis and aseptic loosening of prosthesis].

Aseptic looseningis is one of the most frequent long-term complications after joint replacement, which limits the service lire of prosthesis. A lot of studies have been focused on macrophage, osteoblast, osteoclast and fibroblast in interface membranes around prostheses recently. Aseptic loosening of orthopedic implants used in joint replacement results from bone loss that occurs through the resorptive activity of inflammatory cells activated by the presence of wear particles. Apoptosis has been observed in the periprosthetic site and it has been interpreted as a sign of resolution of inflamma

Target Organs of Metals Released from Metal-on-Polyethylene Knee and Hip Arthroplasty Implants: Implications for Tissue Metal Profiles.

Metals are used in orthopedic implants. The wear of arthroplasty implant can lead to the release of arthroprosthetic metals, both locally and systemically, after migration into the organs. While the toxicity of metal-on-metal arthroplasty implants is well-known and monitored, the toxicity associated with metal-on-polyethylene (MoP) ones is not as comprehensively understood. This study aimed to investigate the release of metals from MoP arthroplasty implants and their impact on the tissue metal profile in autopsied individuals, comparing them to deceased controls without prostheses. Retinal Prostheses: Engineering and Clinical Perspectives for Vision Restoration.

A retinal prosthesis, also known as a bionic eye, is a device that can be implanted to partially restore vision in patients with retinal diseases that have resulted in the loss of photoreceptors (e.g., age-related macular degeneration and retinitis pigmentosa). Recently, there have been major breakthroughs in retinal prosthesis technology, with the creation of numerous types of implants, including epiretinal, subretinal, and suprachoroidal sensors. These devices can stimulate the remaining cells in the retina with electric signals to create a visual sensation. A literature review of the pre-cl
