Post-infection fatigue syndrome following Q fever.

In 1989, 147 individuals in the West Midlands, UK, were infected with Q fever. Five years later, following anecdotal reports of fatigue, we used a questionnaire-based case-control study to determine the prevalence of chronic fatigue syndrome symptoms in this group. Replies from 71 patients were compared with those from 142 age- and sex-matched controls. Increased sweating (52.9% vs. 31.6%, p = 0.006), breathlessness (50.7% vs. 30.6%, p = 0.006), blurred vision (34.3% vs. 17.8%, p = 0.016) and undue tiredness (68.7% vs. 51.5%, p = 0.03) were found in controls compared to cases. These findings w…

Q fever serology: cutoff determination for microimmunofluorescence.

Q fever, a worldwide zoonosis caused by Coxiella burnetii, lacks clinical specificity and may present as acute or chronic disease. Because of this polymorphism, serological confirmation is necessary to assess the diagnosis. Although microimmunofluorescence is our reference technique, the cutoff titers that are currently used to make a diagnosis of active or chronic Q fever were determined years ago with limited series of patients and sera. We determined the titers of immunoglobulin G (IgG), IgM, and IgA against both phases (I and II) of Coxiella burnetii. Rheumatoid factor was removed before t…

[Neurologic complications of Q-fever (Coxiella burnetii)].

We report on fifteen patients with a typical syndrome of Q-fever. Four patients (= 27%) showed signs of nervous system involvement. One patient developed a meningitic syndrome, another patient had a passing psychosis. In two patients, severe cerebellar deficits were predominant. It is pointed out that these two patients were under lithium therapy because of manic-depressive illness. A synergetic neurotoxic effect of cociella burneti infection and lithium is discussed as a possible cause of the severe cerebellar pattern of symptoms.

Reactivation of Q fever following cardiac surgery.

To determine whether reactivation of Q fever occurs following cardiac surgery 43 patients who had antibodies to Coxiella burnetii were identified pre-operatively. Serum samples were collected 1, 4 to 6, and 24 weeks post-operatively. One patient had active Q fever endocarditis. Seven of the remaining 42 (17%) had a > or = fourfold rise in titres for antibodies to Coxiella burnetii post-operatively. Only two of these seven had an increase in antibody titres for other agents suggesting that the fourfold titre rise for Coxiella burnetii was not part of a polyclonal response. It is concluded that …

Miller Fisher syndrome associated with Q fever.

A 51 year old woman with pneumonia developed Miller Fisher syndrome. Indirect immunofluorescence tests showed antibodies against Coxiella burnetti. Miller Fisher syndrome associated with Q fever has not been described previously.

[Q fever in Guinea-Bissau. 1 case].

Q fever is seldom reported in West Africa. The case of a man returning from Guinea Bissau who presented an acute lobar pneumonia with fever, headache, hematuria and hepatitis was confirmed by high titers of antibody in Phase II indirect immuno-fluorescence which appeared on the twelfth day of fever. Treatment with erythromycin was continued by doxycyclin, and complete resolution of all signs was promptly obtained. Coxiella Burnetii might thus be responsible of cases of unexplained fever with respiratory or hepatic manifestations in West Africa.

Progressive demyelinating disease following acute Q fever.

We describe the case of a 36-year-old farmer who presented with an acute febrile illness, diagnosed on serological testing as acute Q fever. He then developed progressive demyelinating disease. The relationship between acute Q fever and subsequent demyelinating disease is discussed.

Sheep-associated outbreak of Q fever, Idaho.

Between Feb 1 and Aug 31, 1984, an outbreak of 18 symptomatic cases of Q fever occurred in Idaho; these numbers represent an increase over the three cases reported in 1982 and the five reported in 1983. Four of the patients in the outbreak required hospitalization for two to five weeks; there were no fatalities. Eight of the cases had documented Q fever hepatitis, and one had pneumonia. All 18 of the 1984 cases for whom information was available were epidemiologically linked to visiting or working at a sheep research station and/or being exposed to animals from this research station. In this o…

[Q-Fever and its complications (author's transl)].

Q-Fever recurs as localized epidemics in the German Federal Republic. The disease has not previously been observed in North Hesse. During a familial infection with Coxiella burneti a 15-year-old boy together with his parents and an aunt became ill with atypical pneumonia. After the pulmonary symptoms had subsided a deep left leg and pelvic venous thrombosis occurred in the boy in the fifth week after the onset of the disease. The thrombosis which is very rare in childhood could be successfully removed at operation.
