Satisfactory results of diaphragmatic plication for bilateral phrenic nerve paralysis.

Bilateral diaphragmatic plication was performed in a 44-year-old man who underwent complete resection of a thymoma infiltrating the right lung, bilateral brachiocephalic vein, pericardium, and bilateral phrenic nerves. The plication procedure allowed him to be weaned from the ventilator on postoperative day 4. He demonstrated no restrictive or obstructive pattern of lung function, and after respiratory rehabilitation he returned to work full time 5 weeks after the operation. The present results indicate that ventilatory movement of the thoracic cage can compensate for loss of bilateral diaphra

[Diaphragmatic paralysis after puncture of the internal jugular vein].

Two cases of paralysis of the diaphragm occurring after puncture of the internal jugular vein are reported. Both were old cancer patients, requiring an internal jugular venous catheter for intravenous feeding. Although the catheter was easily placed in both cases, the diagnosis of diaphragmatic paralysis was made postoperatively. The first patient remained asymptomatic, despite paradoxical breathing when lying supine; there was no change in her condition up to her death 4 months later. The second patient became increasingly breathless, with hypoxia and hypercapnia, during recovery from a gastr

Diaphragmatic paralysis following cervical herpes zoster.

A 74-year-old man was found to have a paralysed left hemidiaphragm within four months of the appearance of a typical herpes zoster rash involving his left shoulder and neck. Investigations, including bronchoscopy and computed tomography of the chest, failed to detect a cause for the diaphragmatic paralysis. We believe that the cervical zoster and diaphragmatic paralysis were causally related, a rare but recognised association.

Diaphragmatic paralysis following chiropractic manipulation of the cervical spine.

We present a case of phrenic nerve damage resulting in unilateral diaphragmatic paralysis following chiropractic manipulation of the neck. Related vascular and neurologic complications of spinal manipulation are reviewed.

[Favorable course of a bilateral postoperative diaphragmatic paralysis].

Writing about a case of post-operative diaphragmatic bilateral palsy (bilateral thoracotomies for oesocoloplasty and oesophagectomy necessited by a caustic oesophagitis) on a 8 years old boy, palsy which was cured 6 months after, the authors tell about causes, prognostic and treatment of diaphragmatic palsies. In that observation, treatment consisted essentially in artificial ventilation for months and a half, and on intensive kinesitherapy.

Severe stridor and marked respiratory difficulty after right-sided supraclavicular brachial plexus block.

Brachial plexus block is commonly used for upper limb surgery. Although the procedure is safe, it may be associated with some life-threatening complications. We performed right-sided supraclavicular brachial plexus block for below-elbow amputation in a 45-year-old female. At completion of the block the patient developed marked respiratory difficulty with audible inspiratory stridor. Although SpO(2) decreased to 82% initially, it was increased to 100% by continuous positive airway pressure with a face mask. On conventional direct laryngoscopy, the left vocal cord was found to be in the midline

Approach to Robotic, Transabdominal, Left Hemi-Diaphragm Plication.

Impaired contractility of the diaphragm can occur secondary to diaphragm paralysis. Signs and symptoms of diaphragm paralysis vary among patients. Some may be asymptomatic and present incidentally, while others experience dyspnea secondary to impaired ventilation. Patients who are symptomatic from diaphragm paralysis may benefit from surgical intervention with diaphragm plication. Many surgical approaches are available for diaphragm plication, including open, thoracoscopic, laparoscopic, and robotic-assisted procedures. In this paper, we discuss our step-by-step approach to a robotic, transabd

Noninvasive ventilation: An important option in the management of hemidiaphragm paralysis.

Hemidiaphragm paralysis (HP) is a potential complication of cardiac surgery. While most patients are either asymptomatic or have mild symptoms, some are at risk of developing life-threatening hypercapnia. We present a case of a patient who developed HP after tricuspid valve replacement. Diaphragm plication was deferred due to underlying comorbidities, but over time she developed severe hypercapnic respiratory failure requiring intensive care unit admission. Chronic noninvasive ventilation therapy (NIV) was initiated, which improved her symptoms and hypercapnia and prevented further hospitaliza

Management of phrenic nerve injury post-cardiac surgery in the paediatric patient.

BACKGROUND: Phrenic nerve injury is a common complication of cardiac and thoracic surgical procedures, with potentially severe effects on the health of a child. This review aims to summarise the available literature on the diagnosis and management of PNI post-cardiac surgery in paediatric patients with CHD. MAIN BODY: The presence of injury post-surgery can be difficult to detect and may present with non-specific symptoms, emphasising the importance of an effective diagnostic strategy. Chest X-ray is usually the first investigation for a suspected diagnosis of PNI, which is usually confirmed

[Two Cases of Parkinson's Disease Accompanied with Nasogastric Tube Syndrome].

We report 2 cases of patients with Parkinson's disease who exhibited bilateral vocal cord paralysis induced by an indwelling nasogastric tube (N-G tube). Both patients showed abrupt inspiratory stridor after N-G tube placement. A fiberoptic laryngeal examination revealed bilateral vocal cord abductor paralysis (VCAP). After N-G tube removal, patient symptoms improved. Nasogastric tube syndrome (NGTS) is an uncommon but life-threatening syndrome that causes sore throat and bilateral VCAP following N-G tube insertion. Throat pain is considered an important early manifestation of NGTS. However, i

Stapled Diaphragmatic Plication: Is It Better Than Suture Plication?

Diaphragm dysfunction, which can be due to eventration or an abnormally high-positioned diaphragm, can cause respiratory compromise. Eventration is most commonly due to unilateral diaphragm paralysis, which reduces ventilatory function in adults by about 25%. Since the 1920s, this condition has been treated with diaphragm plication, which itself has evolved to include the use of several different techniques and materials. This review explores the relevant literature on diaphragm plication using sutures or staples for the treatment of eventration to determine if either provides a clinical benef
