The sympathetic nervous system and the kidney: its importance in renal diseases.

There is a two-way relation between the sympathetic nerve system and the kidney. On the one hand the sympathetic nerve system affects renal … bolished by bilateral nephrectomy. Experimentally, interruption of afferent nerve traffic by dorsal rhizotomy partially prevents hypertension in renal damage models. Imidazoline-preferring binding sites are found in the kidney. In the rat, natriuresis is elicited by injection of moxonidine into the renal artery. In humans, there is no definite evidence for …

Both motor and sensory abnormalities contribute to changes in foot posture in an experimental rat neuropathic model.

This study was undertaken to find out whether pain contributes to abnormal posture of the foot in rats displaying neuropathic pain behaviors, and if so, whether there is a specific abnormality which reflects the level of pain. We used a rat model of neuropathic pain which is produced by tightly ligating the L5 and L6 spinal nerves just distal to the dorsal root ganglion (DRG). The operation produced neuropathic pain behaviors along with abnormal foot posture as has been shown previously. On the other hand, injury to the same somatic sensory and motor fibers at a more proximal site by sectionin

Neural stimulation for control of voiding dysfunction: a preliminary report in 22 patients with serious neuropathic voiding disorders.

Our experience with electrode implantation has demonstrated that the most successful combination to achieve continence and promote bladder evacuation is implantation on the ventral component of S3 or S4 and extensive dorsal rhizotomy with selective peripheral neurotomy. Of 22 patients with serious neuropathic voiding disorders treated during the last 6 years results were available for evaluation in 19 (2 were lost to followup and 1 was withdrawn from the protocol because of an infection at the receiver site). In 8 patients (42 per cent) complete success was achieved. These patients have regain…

Tinnitus: diagnosis and treatment.

Few conditions are seen as commonly by the otologist and are more poorly understood than subjective tinnitus. Tinnitus has been reported in … cranial fossa section of the cochlear nerve. Tinnitus from cervical nerve lesions can be treated by rhizotomy. The use of a hearing aid or introduction of a sound with a tinnitus masker has been found to be 82% effective in suppressing tinnitus. Maskers can be combined with a hearing aid in some cases. The pathogenesis of tinnitus is discussed, but the meth …

Trigeminal root compression for trigeminal neuralgia in patients with no vascular compression.

BACKGROUND: Trigeminal neuralgia (TN) may be present in the absence of vascular compressiom, and surgical treatment in these cases is controversial. Our objective is to evaluate the efficacy and safety of trigeminal root compression in this situation. METHODS: A prospective collection of clinical data on all patients diagnosed with idiopathic TN in our institution. Cases with no visible offending vessel intraoperatively were included in this study. These patients underwent trigeminal root compression and long-term follow-up. The basic characteristics and outcomes were analyzed. RESULTS: From

Outcomes after percutaneous surgery for patients with multiple sclerosis-related trigeminal neuralgia.

BACKGROUND: Approximately 1% to 2% of patients with multiple sclerosis (MS) develop trigeminal neuralgia (TN). Percutaneous surgery is commonly performed in medically refractory cases. OBJECTIVE: To analyze the pain outcomes and complications of patients with MS-related trigeminal neuralgia (MS-TN) having percutaneous surgery. METHODS: Patients having balloon microcompression (BMC; n = 69) or glycerol rhizotomy (PRGR; n = 67) from 1997 to 2010 were reviewed retrospectively. Patients in the 2 groups were similar with regard to age, sex, pain location, and pain quality. Mean pain duration was …

MEG-BMI to Control Phantom Limb Pain.

A brachial plexus root avulsion (BPRA) causes intractable pain in the insensible affected hands. Such pain is partly due to phantom limb pain, which is neuropathic pain occurring after the amputation of a limb and partial or complete deafferentation. Previous studies suggested that the pain was attributable to maladaptive plasticity of the sensorimotor cortex. However, there is little evidence to demonstrate the causal links between the pain and the cortical representation, and how much cortical factors affect the pain. Here, we applied lesioning of the dorsal root entry zone (DREZotomy) and t

Neurosurgical treatment of short-lasting, unilateral, neuralgiform hemicrania with conjunctival injection and tearing.

Trigeminal neuralgia solely involving the upper trigeminal nerve branch is rare. The SUNCT syndrome (short-lasting, unilateral, neuralgiform hemicrania with conjunctival injection and tearing) in which the periorbital pain lasts for 60-120 s, and is accompanied by conjunctival injection and tearing is even less common. Unlike trigeminal neuralgia, SUNCT is usually not relieved by medication. Three patients with SUNCT were treated with retrogasserian glycerol rhizolysis, two of them twice. All five treatments provided complete pain relief and the duration of the effects was 2 to more than 4 yea

Neurophysiological navigation in the trigeminal nerve: use of masticatory responses and facial motor responses evoked by electrical stimulationof the trigeminal rootlets for RF-thermorhizotomy guidance.

RF-thermorhizotomy of the trigeminal nerve is an effective and safe treatment of trigeminal neuralgia, provided lesioning of the sensory fibers is performed precisely. To control the accurate placement of the electrode tip, electrical stimulation testing, prior to thermal lesioning, is of prime importance. The clinical observation of direct masticatory responses (DMR) and facial evoked motor responses (EMR) produced by stimulation of the trigeminal rootlets (at 5 Hz) helps to place the electrode tip in the optimal location. The best location is the one where the threshold for eliciting DMR is …

An electron-microscopic analysis of biopsy samples of the trigeminal root taken during microvascular decompressive surgery.

During surgical exposure of the trigeminal root for the treatment of trigeminal neuralgia, biopsy specimens of the nerve root were taken in 12 consecutive patients. The biopsies were taken from areas as close as possible to the site of vascular compression, when present. The tissue was prepared for electron-microscopic evaluation. Except for 1 case, massive disruption of tissue ultrastructure was observed. The major findings included zones of de- and dysmyelination, juxtaposition of denuded axons, apparent axon loss and degeneration, and collagen deposition. Adjacent areas of normal root struc

Microvascular decompression for glossopharyngeal neuralgia using intraoperative neurophysiological monitoring: Technical case report.

BACKGROUND: Glossopharyngeal neuralgia (GN) is a rare functional disorder representing around 1% of cases of trigeminal neuralgia. Lancinating throat and ear pain while swallowing are the typical manifestations, and are initially treated using anticonvulsants such as carbamazepine. Medically refractory GN is treated surgically. Microvascular decompression (MVD) is reportedly effective against GN, superseding rhizotomy and tractotomy. METHODS: We encountered three patients with medically refractory GN who underwent MVD using intraoperative neurophysiological monitoring (IONM). The offending ve…
