Robot-assisted partial and total splenectomy.

BACKGROUND: The interest of robotics in performing partial and total splenectomy is poorly reported so far. We report herein our experience. METHODS: From November 2001 to November 2009, 24 consecutive robotic splenectomies were performed by the same surgeon. All data were prospectively collected and reviewed retrospectively. RESULTS: Twelve men and 12 women with a median age of 48 years underwent a robotic splenectomy, three of which were partial splenectomies. The indications were: ABO incompatibility for kidney transplantation (n = 7), haematological disease (n = 7) and miscellaneous path…

Robot-assisted resection of pulmonary sequestrations.

Pulmonary sequestration is a rare congenital malformation and may cause recurrent infections and hemoptysis. Although video-assisted thoracic surgery (VATS) is feasible, some drawbacks remain, mainly dealing the managing of anomalous vessels. We describe the use of a robotic system (da Vinci Robotic System, Surgical Intuitive, Mountain View, CA, USA) in the treatment of four consecutive cases of pulmonary sequestration.

Transoral robotic approach for schwannoma of the larynx.

We present a rare case of schwannoma of the supraglottic larynx in a 30-year-old woman with a 5-year history of dysphonia. Excision of the mass was performed by transoral robotic surgery (TORS) without complication using the robotic da Vinci Surgical System, 0-degree three-dimensional endoscope, 5-mm microinstruments compatible with the da Vinci robot, and an FK retractor. Transoral robotic surgery rendered good exposure that allowed complete tumor resection. An external approach and tracheotomy were not necessary in this case. We found that TORS was practicable, efficient, and nonhazardous fo

Robotic enterocystoplasty: technique and early outcomes.

OBJECTIVE: Enterocystoplasty is an established treatment for patients with refractory neurogenic bladder symptoms. We assessed the feasibility, safety, and efficacy of a robot-assisted enterocystoplasty in this population. MATERIALS AND METHODS: Five neurogenic bladder patients, median age of 43.8 years, underwent the procedure. Using a five-port technique, intraperitoneal robotic enterocystoplasty was performed through the following steps: (1) creation of a U-shaped full-thickness detrusor cystotomy, (2) intracorporeal harvesting of 30 cm of ileum, (3) intracorporeal construction of a detubu

PARAMIS parallel robot for laparoscopic surgery.

The paper presents the parallel robot, which has been developed in Romania and it is used for laparoscope camera positioning. Based on its mathematical modeling, the first low-cost experimental model of the PARAMIS surgical robot has been built. The system has been built in such a way that it has the possibility to transform it in a multiarm robot controlled from the console. The control input allows the user to give commands in a large area for the positioning of the laparoscope using different interfaces: joystick, microphone, keyboard & mouse and haptic device. The first results have been o

Robotic hypospadias surgery: a new evolution.

The dictum that "there is nothing new in surgery not previously described," is quoted regularly and is particularly true of hypospadias. There is an ongoing search for solutions to many troublesome issues concerning surgical treatment of hypospadias, such as what age is the most appropriate to apply surgery, or in how many stages surgery should be performed. We present a case report of the first robotic hypospadias surgery to propose a departure from the standard practice, in the hope of expanding medical expertise and teaching globally. The use of a robot for reconstructive surgery is not nov

Robotically assisted laparoscopic transhiatal esophagectomy.

BACKGROUND: Esophagectomy is a technically demanding operation with high procedure-related morbidity and mortality rates. Minimally invasive techniques were introduced in the late 1980s in an effort to decrease the invasiveness of the procedure. Data concerning the use of robotic systems for esophageal cancer are scarce in the literature. The goal of this report is to describe the authors' early experience using robotically assisted technology to perform transhiatal esophagectomy (RATE). METHODS: Between September 2001 and May 2004, 18 patients underwent RATE at the authors' institution. A re

Cystoscope- and robot-assisted bladder diverticulectomy.

BACKGROUND AND PURPOSE: Minimally invasive approaches to manage bladder diverticula have become increasingly popular; however, intracorporeal identification of bladder diverticula may be challenging. We report a novel technique for diverticular illumination using flexible cystoscopy. PATIENTS AND METHODS: After management of bladder neck obstruction, two patients with bladder diverticula and persistent lower urinary tract symptoms underwent robot-assisted bladder diverticulectomy. Cystoscopic illumination was used in both cases to aid diverticular identification and dissection. RESULTS: Our

Robotic colposuspension: two case reports.

BACKGROUND AND PURPOSE: Until now, open colposuspension has been the gold standard in the surgical treatment of stress urinary incontinence. However, the open procedure is associated with more complications and longer hospital stays and is hard to justify given the availability of minimally invasive alternatives. We report two cases of robotic colposuspension performed for genuine stress urinary incontinence at our institution. PATIENTS AND METHODS: Two multiparous women age 42 years and 53 years with urodynamically proved genuine stress urinary incontinence underwent robotic colposuspension.

Histopathologic outcomes of robotic radical prostatectomy.

Robotically assisted laparoscopic radical prostatectomy is a minimally invasive alternative for the treatment of prostate cancer. We report the histopathologic and short-term PSA outcomes of 500 robotic radical prostatectomies. Five hundred patients underwent robotic radical prostatectomy. The procedure was performed via a six trocar transperitoneal technique. Prostatectomy specimens were analyzed for TNM stage, Gleason's grade, tumor location, volume, specimen weight, seminal vesicle involvement, and margin status. A positive margin was reported if cancer cells were found at the inked specime

Solo surgery--with the aid of a robotic assistant.

In the interests of cost containment this study was performed to compare the cost-effectiveness of utilizing a robotic assistant instead of a human assistant. Twelve patients undergoing laparoscopic bladder neck suspension were studied retrospectively. In six of these cases a human assistant was utilized and in another six a robot arm was utilized to hold and manoeuvre the laparoscope and camera. Evaluations have previously been made showing the improved steadiness and clarity of picture when utilizing the robot rather than a human and in this case the length of surgery was compared and the co
