Scaphoid Fractures in Adults Aged 50 Years or Older: Epidemiology and Association With Osteopenia and Nonunion.

BACKGROUND: Scaphoid fractures are less commonly reported in adults older than 50 years. The association between bone density and outcomes following scaphoid fractures has not been explored in this patient population. The second metacarpal cortical percentage (2MCP) has been shown to predict low bone density. The purpose of this study is to describe the epidemiology and radiographic characteristics associated with scaphoid fractures in adults older than 50 years, determine the prevalence of osteopenia defined by 2MCP, and evaluate the characteristics associated with scaphoid nonunion in this p

Fractures and pseudarthrosis of the carpal scaphoid.

The ultimate outcome of fractures of the scaphoid bone is determined by the choice of initial treatment. The special characteristics of the blood supply to the bone, which often increase the risk of non-union, have prompted us to propose the indications for primary surgical treatment based on the radiographic appearances. The authors propose a classification of the radiographic characteristics of fractures of the scaphoid for determining at the outset the treatment most likely to achieve consolidation and hence a more rapid resumption of occupational and sports activities. They present the res…

Interfragmentary compression across a simulated scaphoid fracture--analysis of 3 screws.

PURPOSE: To measure the interfragmentary compression generated across a simulated fracture in cadaveric scaphoids by 3 different headless co … ression, which was recorded during screw insertion and for the following 5 minutes. As a measure of scaphoid bone quality the modulus of elasticity of the trabecular bone of each specimen was then calculated from uniaxial compression tests. RESULTS: The mean interfragmentary compression generated by the Acutrak Standard screw was significantly greater and more …

Regional scintimetry in scaphoid fractures.

A quantitative scintigraphic method was introduced to examine the proximal, middle and distal third of the carpal scaphoid bone. 99mTc-Sn-pyrophosphate scintimetry was obtained by the use of a gamma camera equipped with a pinhole collimator. Of six patients in whom a unilateral fresh fracture of the scaphoid was clinically suspected, radiology showed a fracture of the scaphoid in four and no fractures in two. The method may be suitable in early diagnosis and further localization of clinically suspected fractures with initially non-diagnostic radiographs.

Systematic Review of Diagnosis of Clinically Suspected Scaphoid Fractures.

Background  Scaphoid fracture accounts for approximately 15% of acute wrist fractures. Clinical examination and plain X-rays are commonly used to diagnose the fracture, but this approach may miss up to 16% of fractures in the absence of clear-cut lucent lines on plain radiographs. As such, additional imaging may be required. It is not clear which imaging modality is the best. The goal of this study is to summarize the current literature on scaphoid fractures to evaluate the sensitivity, specificity, and accuracy of four different imaging modalities. Case Description  A systematic-review and me

Extensor tendon rupture of the index finger secondary to avascular necrosis of the scaphoid: a case report.

We report on a 74-year-old man with extensor indicis proprius (EIP) tendon rupture secondary to avascular necrosis and collapse of the scaphoid. The collapsed scaphoid was removed, followed by 4-corner fusion and tendon transfer of the extensor digiti minimi to the EIP. At the 3-year follow-up, the patient was painfree and had full extension of his left index finger, with proper alignment of the carpal bones and union of the 4 bones.

Vascularized Bone Flaps for Carpal Reconstruction.

Within the field of hand and upper extremity surgery, reconstruction of the bony carpus remains a perplexing task and is a field undergoing rapid evolution. Among the eight bones of the carpus, the scaphoid and lunate are most frequently affected by traumatic and avascular processes which render their articular surfaces degenerated and painful. These conditions include scaphoid waist fracture, scaphoid proximal pole fracture, and Kienböck's disease of the lunate. While traditional salvage operations with limited functional outcomes have historically been employed for management of these unsolv

MRI in the management of scaphoid fractures in skeletally immature patients.

BACKGROUND: The scaphoid is the commonest fractured carpal bone, but excluding a scaphoid fracture with plain radiographs is difficult. Other imaging modalities are being increasingly evaluated in the management of scaphoid injuries. MRI has been shown to be of considerable value in the adult population but there have been limited studies of its use in children. PURPOSE: To evaluate the role of MRI in the acute management of suspected scaphoid injuries in children. METHODS AND MATERIALS: Fifty-six children (57 injuries) who had a suspected scaphoid injury underwent MRI within 10 days of thei

Idiopathic osteonecrosis of the scaphoid: a series of 3 surgical cases.

Osteonecrosis of the scaphoid is usually a posttraumatic condition, but idiopathic cases are well known to occur. Magnetic resonance imaging is useful in distinguishing the types of osteonecrosis. In posttraumatic osteonecrosis, MRI abnormalities are in the proximal scaphoid that are consistent with interosseous vascular disruption, whereas in idiopathic osteonecrosis, the entire scaphoid is affected. The MRI findings correlate with histologic findings of osteonecrosis, and the etiology may be an extraosseous insult to the scaphoid blood supply. We presented a series of 3 cases of idiopathic g

Replacement of a destroyed scaphoimodelled prosthesis.

Preiser disease is a rare condition where ischaemia and necrosis of the scaphoid bone occurs without previous fracture. A female patient in her 30s presented with severe right wrist pain caused by Preiser disease. Initially, she was treated by a pedicled vascularised bone graft, but unsuccessfully. Four years after the initial diagnosis, the affected scaphoid was replaced by a 3D modelled prosthesis. Three years after the operation, the patient had significant pain reduction, good wrist function and grip strength. Final X-rays demonstrated no signs of dislocation of the implant or signs of per…

Computer-Assisted 3-Dimensional Reconstructions of Scaphoid Fractures and Nonunions With and Without the Use of Patient-Specific Guides: Early Clinical Outcomes and Postoperative Assessments of Reconstruction Accuracy.

PURPOSE: To present results regarding the accuracy of the reduction of surgically reconstructed scaphoid nonunions or fractures using 3-dimensional computer-based planning with and without patient-specific guides. METHODS: Computer-based surgical planning was performed with computed tomography (CT) data on 22 patients comparing models of the pathological and the opposite uninjured scaphoid in 3 dimensions. For group 1 (9 patients), patient-specific guides were designed and manufactured using additive manufacturing technology. During surgery, the guides were used to define the orientation of t

Osteosarcoma of the scaphoid. A case report and review of the literature.

We describe a case of osteosarcoma of the scaphoid bone, which to our knowledge is only the second reported case of osteosarcoma in the carpus. A 38-year-old man complained of intense pain in the right wrist and had curettage and a bone graft for a lesion in the scaphoid. Histological examination showed this to be an osteosarcoma. Below-elbow amputation was performed and adjuvant chemotherapy given. There has been no evidence of recurrence or metastases at 33 months after amputation.

Dorsal approach to scaphoid nonunion.

The surgical approach for treatment of scaphoid nonunion usually involves various combinations of screws and Kirschner wires, with or without bone grafts. This article reports our results with 36 dorsal-approach bone grafting procedures for treatment of scaphoid nonunion with Kirschner wires. Union was achieved in 89%. The average follow-up was 5 years. Flexion/extension averaged 76% of the opposite wrist, and grip strength was 88% of the opposite hand. Ninety-one percent of the employed patients returned to their original jobs. The dorsal approach provides satisfactory exposure for fragment r

How to manage scaphoid waist fractures? Introduction of pragmatic imaging efficient pathway and literature review of the recent evidence.

INTRODUCTION AND OBJECTIVE: The scaphoid bone sustains about 90 % of carpal bone fractures and is the second-highest bone at risk of post-traumatic osteonecrosis. Delayed diagnosis and treatment could lead to non-union and advanced carpal bones collapse. This study aimed to introduce an imaging efficient and practical scaphoid waist fracture management pathway (SWFMP) and measure its efficacy in clinical practice. MATERIALS AND METHODS: The SWFMP was introduced in January 2020. Suspected occult fractures were approached by early orthopaedic clinical assessment and subsequent urgent MRI scan w…

High-resolution CT of the wrist: initial experience with scaphoid disorders and surgical fusions.

We performed high-resolution CT scans on 30 wrists in 27 patients with either previous surgical intercarpal fusion or known or suspected scaphoid abnormalities. Most examinations used the same protocol: contiguous 1.5-mm axial sections parallel to the long axis of the body of the scaphoid, small reconstruction circle, and bone reconstruction algorithm. Fifteen patients were evaluated for union of previous surgical carpal fusions; eight patients were evaluated for healing of scaphoid fractures; and the other four patients had miscellaneous disorders, including a midcarpal dislocation with a sca

Occult fractures of the waist of the scaphoid: early diagnosis by high-spatial-resolution sonography.

OBJECTIVE: This study evaluated the diagnostic accuracy of high-spatial-resolution sonography in the diagnosis of occult fractures of the waist of the scaphoid. SUBJECTS AND METHODS: Sonography of the scaphoid bone with a 12-MHz transducer was performed in 54 patients with clinically suspected scaphoid fracture and normal findings on initial radiographs, including specific scaphoid images. Three levels of clinical suspicion were considered: high (20%), moderate (30%), and low (50%). Attention was paid to the continuity of the scaphoid cortex and to the surrounding soft tissues (i.e., hemarthr…

[Scaphoid fractures--operative or conservative treatment? A CT-based classification].

Traditionally acute scaphoid fractures were treated by immobilization. As a consequence we have to deal with a high number of scaphoid non-unions or SNAC wrists. A study of 30 patients with scaphoid non-union showed that only 30% (9 patients) have not seen a doctor, while the majority of the patients (70%, 21 patients) were treated by a physician after trauma. In 15 (71.4%) of these 21 patients a missed diagnosis and in 6 (28.6%) a failed conservative treatment of the scaphoid fracture were the reasons for scaphoid non-union. Therefore, improvements in the diagnosis and therapy of scaphoid fra

Rethinking Scaphoid Fixation.

Scaphoid fixation, whether for acute injuries or nonunion, is made challenging by the small and intra-articular nature of the most commonly fractured carpal bone. The purpose of this article is to review the techniques to simplify scaphoid fixation and to optimize healing and early return to activity.

Carpal instability, the missed diagnosis in patients with clinically suspected scaphoid fracture.

To determine the incidence of carpal instability and its relation to clinical findings in patients with suspected scaphoid fracture, we performed a long-term follow-up investigation in a consecutive series of 160 patients who were treated in our department of traumatology for suspected scaphoid fracture after a fall on the outstretched hand. Radiography of the carpus was obtained. Bone scintigraphy was performed in all patients with negative initial radiographs. Follow-up investigation was performed in 100 patients and consisted of history, clinical examination, including measurement of grip s

Scaphoid Plate Osteosynthesis in Complex Fractures and Wrist Trauma: A Case Series.

Introduction Treatment of scaphoid fracture is challenging due to its unique blood supply and geometry. Traditionally, a headless compression screw is the standard treatment for unstable scaphoid fracture. Some fractures are complex, for example, comminution with bone loss. A scaphoid plate is an option in these difficult fractures providing adequate rotational stability. Aim To share our experience in using scaphoid plates in complex wrist trauma and comminuted fractures. Method and material Complex wrist trauma involving scaphoid fractures that were comminuted and multi-fragmentary fractures
