Severe erosive lesion of the glenoid in gouty shoulder arthritis: a case report and review of the literature.

BACKGROUND: Gout is a metabolic disease characterized by recurrent episodes of acute arthritis. Gout has been reported in many locations but is rarely localized in the shoulder joint. We describe a rare case of gouty arthritis involving bilateral shoulder joints and leading to severe destructive changes in the right shoulder glenoid. CASE PRESENTATION: A 62-year-old male was referred for pain and weakness in the right shoulder joint for two years, and the pain had increased in severity over the course of approximately nine months. A clinical examination revealed gout nodules on both feet and …

A modified internal rotation stretching technique for overhand and throwing athletes.

Strength, stretching, and rehabilitation methods for the shoulder have been previously described and have been universally applied; nevertheless, many throwing athletes continue to develop overuse injuries. The pitching process tends to increase external rotation and limit internal rotation of the shoulder joint. Our technique involves a modified stretching method of the posterior shoulder musculature. The athlete lies prone with the elbow flexed 90 degrees. With the shoulder abducted 90 degrees, in neutral flexion/extension, and 90 degrees or more of internal rotation, the scapula protrudes p…

Editorial Commentary: Immediate Surgical Stabilization Following a First-Time Traumatic Anterior Shoulder Dislocation Is Still the Best Evidence-Based Approach.

Traumatic anterior shoulder dislocations are unfortunately common. The initial dislocation is enough to cause permanent anatomic and biomechanical alterations to the glenohumeral joint, which increases the likelihood of further events. When a patient crosses into the multiple dislocator category, further soft-tissue and bony injury occurs. This is almost certainly the reason that the number of preoperative dislocations significantly diminishes the success of an arthroscopic stabilization and increases the need for a more aggressive approach that may carry greater risks of complications. When i

Creating a Three-Dimensional Reconstruction of the Glenohumeral Joint From Magnetic Resonance Imaging to Assist in Surgical Decision-Making.

Understanding the anatomical structure of a patient's shoulder joint is essential in surgical decision-making, especially regarding glenohumeral bone loss. The use of various imaging techniques, such as magnetic resonance imaging (MRI) and computed tomography (CT), bring certain advantages and disadvantages in assessing joint structure. Before a surgical procedure, bone loss can be observed and measured using these imaging techniques in both 2-dimensional and 3-dimensional (3D) views. The ability to visualize the shoulder joint in a 3D manner, as commonly done with CT scans, is helpful in asse…

Neuroarthropathy: an overuse injury of the shoulder in quadriplegia.

The shoulder joint of the spinal cord injured patient is the primary joint used for transfer and wheelchair propulsion. In addition to ordinary use, the shoulder is subject to overuse injury with resultant functional impairment. Frequent overuse injuries of the shoulder include subdeltoid bursitis, bicipital tendonitis, rotator cuff tear and secondary degenerative arthritis. Neuroarthropathy of the shoulder, an overuse injury of an insensate shoulder girdle, has been rarely described in the literature. The case presented is that of an active 47 year old, right hand dominant, C7 complete quadri…

Editorial Commentary: Magnetic Resonance Arthrogram Is More Accurate and Precise Than Conventional Magnetic Resonance Imaging for Evaluating Labral Tears After First-Time Shoulder Dislocation.

Recent research shows that magnetic resonance arthrogram offers better definition of certain shoulder pathology, especially labral tears, compared with magnetic resonance imaging (MRI). Conventional MRI accuracy and precision deteriorate after 2 weeks, and few health delivery environments achieve MRI within 14 days of a shoulder dislocation. Moreover, further loss of MRI accuracy is time dependent. Although magnetic resonance arthrogram should be considered a first-line imaging study when evaluating shoulder instability more than 2 weeks after the patient's injury, one must also consider clini

The Short, 5-Item Shoulder Instability-Return to Sport After Injury Score Performs as Well as the Longer Version in Predicting Psychological Readiness to Return to Sport.

PURPOSE: To reduce the length of the Shoulder Instability-Return to Sport After Injury (SIRSI) scale and determine the predictive validity of the short version compared with the original form. METHODS: This study included patients who underwent an arthroscopic Bankart repair or open Latarjet procedure between 2017 and 2019. One group was used for the SIRSI scale-reduction process, and a second group was used to test the predictive validity of the proposed short SIRSI scale. The Cronbach α value was used to evaluate internal consistency. Validity was determined by calculating the Pearson corre

An algorithm for successfully managing anterior shoulder instability.

The most common form of shoulder instability involves the anterior glenohumeral joint. Often it is associated with labral and bony injuries with subsequent recurrent instability. To determine optimal management, clinicians should perform a detailed history and physical examination, including appropriate diagnostic imaging to assess for concomitant humeral and glenoid bony deficiencies and other soft-tissue pathologies. Early surgical intervention may reduce risk of recurrence, particularly in young, active athletes. This article highlights the relevant anatomy, pathoanatomy, diagnostic examina

Glenoid concavity has a higher impact on shoulder stability than the size of a bony defect.

PURPOSE: Surgical treatment of shoulder instability caused by anterior glenoid bone loss is based on a critical threshold of the defect size. Recent studies indicate that the glenoid concavity is essential for glenohumeral stability. However, biomechanical proof of this principle is lacking. The aim of this study was to evaluate whether glenoid concavity allows a more precise assessment of glenohumeral stability than the defect size alone. METHODS: The stability ratio (SR) is a biomechanical estimate of glenohumeral stability. It is defined as the maximum dislocating force the joint can resis

Long-term results of arthroscopic Bankart repairs for anterior instability of the shoulder in patients aged thirty years or older.

INTRODUCTION: Bankart repair is a popular treatment for anterior shoulder instability. However, long-term failure rates of arthroscopic Bankart repair remain higher than Latarjet procedures. The purpose of this study was to report long-term results of arthroscopic Bankart repair in patients greater than 30 years old and analyze risk factors of failure following arthroscopic Bankart repair that are independent of younger age. MATERIALS AND METHODS: Between January 1999 and December 2003, 41 patients aged 30 years or older treated with arthroscopic Bankart repair for anterior shoulder instabili

Editorial Commentary: Delphi Expert Consensus Clarifies Evidence-Based Medicine for Shoulder Instability and Bone Loss.

Anterior glenohumeral instability with glenoid bone loss is a difficult problem and often requires open procedures with bone block augmentation. The current evidence suggests glenoid bone loss of 20% or more as a cutoff value indicating augmentation. Expert consensus-based techniques, such as the Delphi, clarify evidence-based medicine and allow pooling of expert opinion in a scientific fashion. These methods suggest that 3-dimensional computed tomography should be used to evaluate bone loss, previous dislocations, or failed soft-tissue surgery; Hill-Sachs lesions are poorly quantified by stan

Editorial Commentary: Personalized Medicine for Shoulder Instability May Result in Best Outcomes With the Lowest Complication Rates.

Personalization is a type of medical care in which the treatment is customized for an individual patient. When treating shoulder instability, we need to consider not only soft-tissue damage but also the bony lesion and patient characteristics. Of particular importance is the consideration of whether there is anterior glenoid bone loss, together with the presence of a Hill-Sachs lesion, on or off-track, as well as whether the patient is hyperlax and/or is an athlete, in which case in what type of sport. In hyperlax, nonoverhead sport athletes with recurrent anterior instability and glenoid bone

Editorial Commentary: Surgical Treatment of Shoulder Instability With Subcritical Glenoid Bone Loss Requires Innovation: Bankart May Risk Significant Recurrence and Latarjet May Risk Significant Complications.

The patient with a history of shoulder dislocation and subcritical (10%-15%) glenoid bone loss presents a complicated scenario. The "safest" procedure (arthroscopic Bankart repair) may result in a high rate of failure and risk of further surgery. The most successful procedure for avoiding recurrence (Latarjet) comes with potentially high complication rates (of up to 20%), a steep learning curve, risk of permanent nerve injury (up to 15%), and substantial risk of subscapularis deficit. Innovation is most needed in surgery when current treatments lack success or risk significant complications. A

Shoulder problems in the weekend athlete.

With more competitive and recreational athletic pursuits now being undertaken by an aging population, the painful shoulder is seen frequently in the weekend athlete. Episodic stresses to soft-tissue restraints and musculotendinous units around the shoulder lead to a variety of overused pathologic entities. The two most common shoulder problems in the weekend athlete are irritation, swelling, and mechanical impingement; and tearing of the rotator cuff with traumatic shoulder instability (typically of an anterior direction).
