[Man is not just a number].

Man is not just a number, but a unique blend of body, personality and social being. Everyone will become ill at some stage of his or her life, because nobody enjoys perfect health. To understand fully the nature of this illness, the doctor should be aware of (1) stress, and the ways in which a human being can respond to it; (ii) the concepts of incompetence and psychosocial life crises; (iii) the postulates of Halliday; and (iv) the importance of good communication with the patient.

Life experiences and the decision to become a mental patient.

This research focuses upon life events influencing the decision to adopt or reject the mental patient role. Studied were 30 mental health center patients who defined themselves as mentally ill and aggressively sought treatment. These were contrasted with 21 persons who consistently denied that they were mentally disordered, and persistently resisted the treatment forced upon them. Patients in both groups had been exposed to the mental health system on the average of 7 years each. Findings showed that those identifying with the mental patient role, in contrast to those resisting it, were more l

[Tinnitus - Clinical Symptoms and Therapy].

Tinnitus is a phantom sound that can be a symptom of various disorders such as hearing loss, cardiovascular diseases, neurological disorders, diabetes or tumors. Because of the inability to divert the auditory attention from tinnitus sound, patients suffer with insomnia, problems with concentration and other psychological conditions, in some extreme cases including major depression symptoms. The auditory character of tinnitus induces the patients to seek help predominantly at the otolaryngologist practice. The first aim of a physician is to establish the cause of tinnitus and to measure its au

Use of ICF in assessing the effects of Meniere's disorder on life.

OBJECTIVES: We sought to determine the value of the World Health Organization's International Classification of Functioning, Disability and Health (ICF) in subjects with Meniere's disorder in relation to their quality of life. METHODS: We asked 228 members of the Finnish Meniere Federation to report the effects that Meniere's disorder had on their lives. The replies were classified on the basis of the ICF classification and related to the EuroQol 5D score and disease-specific impact. Logistic regression and decision tree analyses were used to determine the relationships. RESULTS: Seventy per

The role of anger in adherence to highly active antiretroviral treatment in patients infected with HIV.

BACKGROUND: Adherence to highly active antiretroviral treatment (HAART) is critical to long-term treatment success in patients infected with human immunodeficiency virus (HIV). However, the relationship between psychological variables and medication adherence is still poorly understood. The aim of this study was to investigate how anger dimensions in subjects with HIV affect adherence to antiretroviral drugs. METHODS: One hundred and thirty outpatients with HIV who were nondepressed and receiving HAART were administered the State-Trait Anger Inventory and a compliance self-report questionnair

Are somatoform disorders changing with time? The case of neurasthenia in China.

OBJECTIVE: To examine how sociocultural factors shape the professional classification and transformation of neurasthenia (shenjing shuairuo in the Chinese language) in China. METHODS: We review the relevant literature as well as cite our own research experience on the topic. RESULTS: We identify three main periods of transformation, namely, the prereform period (before 1980) in which neurasthenia encompassed a wide range of anxiety and mood disorders; the reform period (1980s-1995) when the Diagnostic and Statistical Manual of Mental Disorders-III (DSM-III) and Kleinman's study in Hunan stea

[Functional gastrointestinal disorders].

The functional gastrointestinal disorders (FGID) are the most frequent clinical conditions seen in practice. The FGID are associated with significant work absenteeism, impaired quality of life and increased medical costs. Most patients also suffer from psychosocial problems. Therefore it is important to define the patient's complaints in terms of a biopsychosocial disorder rather than just a medical illness. Physicians must acknowledge the relevance of the psychosocial aspects to prepare the patient for a referral to a specialist (in psychosomatic medicine or a psychotherapist) and to get the

Somatization disorder: a practical review.

OBJECTIVE: This paper reviews the present state of knowledge on the etiology, prevalence, diagnosis, and treatment of somatization disorder (SD). METHOD: A comprehensive review of the literature on SD is described under the above headings. RESULTS: SD is a common condition that is not well managed by many physicians. Patients with SD get caught between the cracks of the health care system, with expensive consequences. SD is a psychiatric disorder, but patients are reluctant to see and be treated by psychiatrists. They frequently are managed by nonpsychiatric physicians who have limited under

[Psychological and psychosocial factors in chronic backache. Findings and social medicine consequences].

In the last ten years, chronic back pain has reached epidemic proportions and plays a major role in work absenteeism, medical rehabilitation costs and early retirement due to occupational disability. Studies into the development of chronic back pain prove that psychogenic and psychosocial factors are far more significant than organically determined phenomena and additional medical findings made using technical equipment. As well as the patient's history and early stress factors, other issues play a major role, such as workplace, job satisfaction and the individual's ability to handle conflict.

Clinical features of dysthymia and age: a clinical investigation.

A few authors have described the clinical picture of dysthymia in groups of elderly patients and pointed out differences from literature reports of dysthymia in younger adults. The present study, an attempt to analyze age effects on clinical characteristics of dysthymia throughout a lifetime, was performed in a sample of 106 patients, all aged > or =18 years, who were diagnosed according to DSM-IV. The patients were evaluated using: (1) a semistructured interview to assess clinical features, family history and previous treatments; (2) the Hamilton Depression Rating Scale; (3) the Interview for

[The competent cell - the immune system as a metaphor, sign, language and culture].

We investigated how the progressive function of the metaphor, the model world of signs, language and culture, link the cell's immunological knowledge with a perceivable concept of the body. Using ethnomethodologically oriented, partly interpretative text analysis, we examined the lymphocyte stimulation test, word clusters in immunology and other medical specialties, the linguistic presentation of the body in German, American and the internet, the presentation of the "self" of the cell in biology and immunology, and protocols of oncological out-patient interviews and chatrooms. The impact of a brief problem-solving training intervention for relatives of recently diagnosed breast cancer patients.

Previous studies have found high levels of psychological distress in women who have a family history of breast cancer. We evaluated a brief Problem-Solving Training (PST) intervention designed to reduce distress among women with a first-degree relative recently diagnosed with this disease. Participants were randomly assigned to either the PST group (N = 144) or a General Health Counseling (GHC) control group (N = 197). At baseline, these groups did not differ on any sociodemographic, risk factor, or psychological distress variables. We evaluated the impact of PST, relative to GHC, at the three

[Future occupational plans of patients after orthopedic rehabilitation: how are they expressed? What influences them?].

Perceptions about one's occupational future are regarded as the initial phase in an ongoing process of occupational resettlement towards achieving gainful activity. Actual gainful activity, however, tends to be increasingly dependent on a number of labour market variables. At the end of an orthopaedic rehabilitation measure, 341 patients with pain syndromes of the locomotor system were interviewed concerning their vocational perspectives and notions, and the data obtained were studied in association with anamnestic and psychosocial factors. It is assumed that both plain expectations (such as r

Presidential address: Gastrointestinal illness and the biopsychosocial model.

OBJECTIVE: To review the evidence supporting the biopsychosocial model in understanding patients with gastrointestinal disorders (GI). METHOD: Essay of personal experience and review of related literature through a MEDLINE search. RESULTS: Through clinical examples of three common gastrointestinal disorders, a case is made to refocus our understanding from a biomedical or disease-based model of illness to a biopsychosocial model. With the latter model, the psychosocial and biological predeterminants are seen to interact in the clinical expression of illness and disease. With gastroesophageal

"Growing pains": clinical and behavioral correlates in a community sample.

Parents of 183 children identified them as having "pain in arms, legs, or joints during the previous 12 months." This group was compared with a group of children without pains selected randomly from the rest of a 1605-member community-based cohort in a study of chronic illness. The pains were most likely to be deep seated, to involve predominantly the lower limbs, and to be described in vague, nonspecific terms. These children were significantly more likely to have recurrent abdominal pain, a negative mood, and behavior problems, and to be aggressive, anxious, and hyperactive. There were no di

Anticipatory nausea and vomiting in pediatric cancer patients: an analysis of conditioning and coping variables.

We examined the influence of cognitive factors and conditioning variables on anticipatory nausea and vomiting symptoms (ANV) in 32 pediatric cancer patients with postchemotherapy nausea and vomiting (PNV) who received ondansetron. The patients were compared with 18 patients without ANV (NANV). Age, gender, history of motion sickness, aggressiveness of the chemotherapy, PNV severity, number of chemotherapy treatments, number, type, and efficacy of self-reported coping strategies, and expectations of severity of postchemotherapy symptoms failed to differentiate the ANV and NANV groups. Psychiatric aspects of cardiovascular disease.

The co-occurrence of psychiatric and cardiac disease in many patients exemplifies the complexity of psychosomatic medicine, with both psychosomatic and somatopsychic effects. Psychological factors including depression, anxiety, and hostility influence the development, clinical expression, and prognosis of coronary heart disease. Depression and anxiety are especially common problems complicating cardiac disease. The physician must approach the patient with an appreciation of the confounding aspects of diagnosis and a readiness to think flexibly about the nature of the problems encountered. SSRI

Coping strategies of children with faecal incontinence.

As part of a study concerning the psychosocial adjustment of 160 children treated for anorectal anomalies, the authors investigated the ways in which the children coped with faecal incontinence (FIC). At the time of assessment, the children were 6 to 18 years of age. Portions of the in-depth interviews with the children and their parents covered questions about methods of managing and coping with FIC at home, socially, and at school. In addition, information was obtained about child and family characteristics that have been shown to contribute to the ability to adapt to chronic health problems

Enhancing the health of somatization disorder patients. Effectiveness of short-term group therapy.

To identify an effective method of treating patients with somatization disorder (SD), the authors conducted a randomized controlled clinical trial of group therapy with 70 SD patients. Primary care physicians treating all patients in the study received a consultation letter offering treatment recommendations for SD. The experimental patients were invited to attend eight group therapy sessions in addition to the consultation provided to their physicians; 45% attended one or more sessions. The experimental patients reported significantly better physical (P < 0.05) and mental (P < 0.01) health in
