Asymptomatic bacteriuria among pregnant women with sickle cell trait in Enugu, South Eastern Nigeria.

CONTEXT: Asymptomatic bacteriuria (ASB) in pregnancy is a major risk factor for developing acute cystitis and pyelonephritis, especially, among women with sickle cell disease. This study compared the prevalence, pattern, and microbiological characteristics of ASB in pregnancy between sickle cell trait (HbAS) and normal hemoglobin AA (HbAA) genotype subjects. MATERIALS AND METHODS: Culture and sensitivity of mid-stream urine samples were collected from 300 HbAS women and 300 matched HbAA control at the antenatal clinic of University of Nigeria Teaching Hospital Enugu, Nigeria from August 2010 …

Giant oral tumor in a child with malnutrition and sickle cell trait: Anesthetic challenges.

Pediatric oral tumors have always been challenging for the even most skilled anesthesiologists. The conventional method of awake intubation is not realistic in this age group. The management is to chart out a plan to intubate the child post induction. We describe successful management of a case of giant of ossifying fibroma in a child with sickle cell trait where non-conventional innovate approach helped us to secure the airway pre-operatively and avoid possible medical complications.

Rhabdomyolysis and acute renal failure in a sickle cell trait athlete: a case study.

This case study reports the clinical details and pathologic mechanisms of a nonfatal case of rhabdomyolysis secondary to heat exhaustion and sickle cell trait (SCT) resulting in acute renal failure. A 19-year-old African American male college football player collapsed after running 5 intervals of 300 m during a preseason conditioning test. After 17 days of treatment, the athlete was released from the hospital to a short-term noncritical care facility for further treatment and dialysis. Scientific literature reports that at least 15 college football players with SCT have died as a result of a s…

ACSM and CHAMP summit on sickle cell trait: mitigating risks for warfighters and athletes.

INTRODUCTION: An estimated 300 million people worldwide have sickle cell trait (SCT). Although largely benign, SCT has been associated with exertional rhabdomyolysis and exercise-related sudden death in warfighters/athletes (WA). The National Collegiate Athletic Association's policy to confirm a student athlete's SCT status during their preparticipation medical examination prompted reaction from some organizations regarding the rationale and ethical justification of the policy. METHODS: On September 26 and 27, 2011, a summit, composed of military and civilian experts in sports medicine and SC…

Effort required to contact primary care providers after newborn screening identifies sickle cell trait.

People with heterozygous status for sickle cell disease (also called sickle cell trait) are essentially healthy, but evidence of rare health problems has increased interest in screening adolescents and young adults prior to enlisting in athletics or military service. Ironically, almost everyone with sickle cell trait is already identified during routine newborn screening for sickle cell disease, but this identification may never reach the parents. As part of a larger statewide study of communication after newborn screening, we decided to document the amount of labor required to connect sickle …

Gonioaspiration for refractory glaucoma secondary to traumatic hyphema in patients with sickle cell trait.

The authors describe a new surgical method for treating disproportionately high intraocular pressures secondary to small traumatic hyphemas in three patients with the sickle cell trait. A novel technique was performed in all three cases to clear the hyphema from the anterior chamber angle. A Lewicky anterior chamber maintainer cannula was placed at the superior limbus to maintain infusion into the anterior chamber. Under direct visualization using a Koeppe goniotomy lens, a 23-gauge single-port cannula was used to aspirate the hyphema from the anterior chamber angle until it was free of blood.…

Central retinal vein occlusion in a sickle cell trait carrier after a cycling race.

A 26-yr-old man with sickle cell trait (SCT) suddenly lost visual acuity in the left eye after a cycling race in hot tropical environment. The cause was massive central retinal vein occlusion (CRVO) with hemorrhaging that rapidly worsened to neovascular glaucoma. Although medically treated, the eye is now marked by total retinal detachment. Cardiovascular function assessment shows no electrocardiographic abnormalities, no anomaly in the supra-aortic tree, and no evidence of structural heart disease. Although normal coagulation markers values (i.e., activated partial thromboplastin time, prothr…

Accelerated clearance of Plasmodium-infected erythrocytes in sickle cell trait and annexin-A7 deficiency.

The course of malaria does not only depend on the virulence of the parasite Plasmodium but also on properties of host erythrocytes. Here, we show that infection of erythrocytes from human sickle cell trait (HbA/S) carriers with ring stages of P. falciparum led to significantly enhanced PGE(2) formation, Ca(2+) permeability, annexin-A7 degradation, phosphatidylserine (PS) exposure at the cell surface, and clearance by macrophages. P. berghei-infected erythrocytes from annexin-A7-deficient (annexin-A7(-/-)) mice were more rapidly cleared than infected wildtype cells. Accordingly, P. berghei-infe…

Hormonal contraception, sickle cell trait, and risk for venous thromboembolism among African American women.

OBJECTIVE: We evaluated the effect of oral and other hormonal contraceptive (HC) use on venous thromboembolism risk among African American women and investigated whether the association was modified by the sickle cell trait. STUDY DESIGN: We report the findings of a case-control study that included 60 African American women with an idiopathic, first episode of venous thromboembolism and 196 African American controls. RESULTS: The odds of current HC use compared with noncurrent use contrasting cases and controls is 3.8 (95% confidence interval [CI], 1.7-8.1; P < .001). Among subjects with sic…

Red blood cells from donors with sickle cell trait: a safety issue for transfusion?

Sickle cell trait (SCT) affects approximately 8% of the population in Martinique (FWI) and about the same rate is found among the African Americans. In several regions of tropical Africa, up to 40% of individuals are also affected. SCT has been characterized as a benign condition and patients are currently asymptomatic or run a benign course. However, life-threatening complications may occur. SCT can be detected in patients hospitalized for various complaints, and SCT likely causes serious morbidity and mortality in some people. Moreover, these clinical observations have been supported by seve…

Sickle cell trait and the risk of Plasmodium falciparum malaria and other childhood diseases.

BACKGROUND: The gene for sickle hemoglobin (HbS) is a prime example of natural selection. It is generally believed that its current prevalence in many tropical populations reflects selection for the carrier form (sickle cell trait [HbAS]) through a survival advantage against death from malaria. Nevertheless, >50 years after this hypothesis was first proposed, the epidemiological description of the relationships between HbAS, malaria, and other common causes of child mortality remains incomplete. METHODS: We studied the incidence of falciparum malaria and other childhood diseases in 2 cohorts …

Lactic response in sickle cell trait carriers in comparison with subjects with normal hemoglobin.

OBJECTIVE: The objective of this study was to assess lactic response (mmol/L) during incremental exercise in a specific population of men and women with sickle cell trait. SUBJECTS: Athletic men and women from the French West Indies showing no anemia and/or alpha thalassemia. METHODS: Anemia was ruled out by hemoglobin level, and alpha thalassemia was ruled out by hemoglobin S percentage with concomitant hemoglobin level and mean corpuscular volume. Comparison was made with control subjects with normal hemoglobin, matched for physical fitness, anthropometric data, and sex. All subjects perfo…

Acute renal insufficiency due to oral acyclovir in a man with sickle cell trait.

Several published reports have suggested that oral acyclovir can cause renal insufficiency, but baseline renal function was either abnormal or unclear in those reports. We describe a patient with oral acyclovir-induced acute renal failure and a normal serum creatinine level documented just before exposure to the drug. Conceivably, competition with a cephalosporin for renal tubular elimination predisposed our patient to nephrotoxic serum levels of acyclovir. In addition, the patient had sickle cell trait, which might have contributed to a disproportionate degree of hyperkalemia and acidosis see…

Haemorheological changes and fibrinolytic activity in adult asthmatic patients with sickle cell trait (HbAS).

Haemorheological parameters and euglobulin lysis time (ELT) were assessed in eighty-one adult asthmatic patients, 27 in stable state (19 with genotype AA and 8 heterozygous AS), 27 during attack (13 homozygous AA and 13 heterozygous AS) and 27 normal healthy individuals as control group (16 homozygous AA and 11 heterozygous AS). Packed cell volume (PCV), plasma fibrinogen concentration (PFC) and plasma viscosity (PV) were significantly higher in asthmatic patients during attack and stable state (p < 0.001), respectively, than controls. ELT was higher in controls (p < 0.001) than in asthmatic p

The clinical manifestations of cerebral malaria among Nigerian children with the sickle cell trait.

In order to describe the interaction between haemoglobin type and the clinical manifestations of cerebral malaria, we studied 60 children aged between 6 and 60 months at University College Hospital, Ibadan, Nigeria. Haemoglobin AS individuals with cerebral malaria did not exhibit major differences in clinical and laboratory characteristics when compared with their haemoglobin AA counterparts. There were no deaths among the Hb AS children compared with an 18% mortality in the Hb AA group. Blood transfusion rates were higher in the AS than in the AA children (86% vs 44%). The higher transfusion

Influence of HbS levels upon the hematological and clinical characteristics of sickle cell trait.

The variable concentration of HbS in individuals with sickle cell trait led us to study the relationship between HbS level and selected vascular events in 355 hospitalized black men with sickle cell trait. There were significant differences in hemoglobin concentration and mean corpuscular volume found in four groups divided by their HbS level, the lowest proportion of HbS (less than 30%) being associated with the lowest hemoglobin concentration (12.6 g/dl) and MCV (77 fl). The percent HbS did not influence the incidence of pulmonary embolism, thrombophlebitis, myocardial infarction, cerebrovas…
