Sleep disorders in pregnancy.

Sleep complaints are a common occurrence in pregnancy that are in part due to pregnancy-associated anatomic and physiological changes but may also be due to pathological causes. In the non-pregnant population, sleep deprivation has been associated with physical and cognitive issues; poor sleep may even be associated with adverse maternal outcomes. Maternal obesity, one of the most prevalent risk factors in obstetric practices, together with physiologic changes of pregnancy predispose to the development of sleep disordered breathing. Symptoms of sleep disordered breathing have also been associa…

[Sleep-related breathing disorders].

BACKGROUND: Sleep-related breathing disorders (SRBD) is a term that includes several different diagnoses. This paper describes aetiology, diagnostics, treatment options and consequences of untreated disease, in children and adults. MATERIAL AND METHOD: The paper is based on own clinical experience and literature identified through a non-systematic search of PubMed and various books. RESULTS: The prevalence of SRDB in children and adults is about 10-15% and that for the most serious type, OSAS (Obstructive Sleep Apnoea Syndrome), is about 2-4%. A sleep study is required to confirm the diagnos…

Benign neonatal sleep myoclonus.

Benign neonatal sleep myoclonus is a non-epileptic disorder. This phenomenon of the first weeks of life is characterized by erratic myoclonic jerks occurring only during sleep and with no electroencephalographic changes. It is not associated with perinatal complications, disappears spontaneously within two to four months, and it does not compromise future development. We illustrate with a video this relatively frequent condition, which is often misdiagnosed as epileptic in nature, and discuss the clinical characteristics and differential diagnosis.

Sleep and allergic rhinitis.

Patients with chronic diseases, including chronic respiratory diseases, usually have considerably impaired sleep quality that may increase the frequency of exacerbations and severity of symptoms, lead to difficulty in patient management, and reduce quality of life (QOL). During the last few decades, several studies have shown that, in addition to the classic signs of sneezing, nasal itching, rhinorrhea, and nasal obstruction, allergic rhinitis has an important impact on the QOL of adults and children. In 2001, the ARIA (Allergic Rhinitis and its Impact on Asthma) report based its new severity …

[Central sleep apnea syndrome].

Central sleep apnea (CSA) is characterized by a lack of drive to inspire for at least 10 sec. In the CSA-syndrome accompanying arousals and desaturations of the arterial blood cause sleep disturbances and sympathetic nerve activations which lead to excessive daytime sleepiness and increase the risk for cardiovascular morbidity. There are six manifestations of CSA: a rare primary or idiopathic form, often in hypocapnic patients with an increased hypercapnic ventilatory drive; Cheyne-Stokes respiration, characterised by periodic CSA and a crescendo/decrescendo breathing pattern, often in patient…

[Obstructive sleep apnea syndrome].

Recurrent total or partial upper airway collapse occurring during sleep characterizes obstructive sleep apnea syndrome (OSAS). Although the disease is affecting more than 5% of the general population, it remains largely undiagnosed. The disease is associated with augmented cardiovascular risk, reduction in vigilance and attentional abilities, being responsible for traffic or occupational accidents. There is also glucose metabolism impairment likely to occur during OSA i.e. resistance to insulin and diabetes. Simplified tools are available in order to achieve the diagnosis either in the sleep l…

[Normal and disordered sleep].

Normal sleep is a complex and reversible state of brain functioning, including reduced inputs and outputs, blunted reflexes, and metabolic and cognitive changes. Evidence supports a role for sleep in the consolidation of an array of learning and memory tasks. Sleep deprivation and fragmentation result in executive dysfunction, increased appetite/weight and cellular stress. Sleep is a vital, complex but plastic function that can be modulated depending on individual heritage and motivation. The major role of sleep in attention and memory raises about concern the reduction in sleep duration recen…

[Classification of sleep disorders].

Sleep disorders are frequent processes, both as a symptom associated with other diseases and as independent disorders. However, only in the last 4 decades has Sleep medicine gained its position among the medical specialties. In fact, it was only in these years that significant advances were obtained in the study of the etiology and treatment of these disorders. Similarly, the different classifications have been evolving over the years. First, they were based upon the clinical symptom; later on, more emphasis was given to the diseases. Finally, in 2005, the new classification was once again bas…

Cytokines and pathological sleep.

Cytokines are proteins produced by leukocytes and other cells that function as intercellular mediators acting on several target tissues, resulting in multiple biologic actions. Over the last decade, medical research has explored the interaction between cytokines and sleep disorders. The aim of this review is to illustrate recent advances in knowledge about the relationship between cytokines and disorders of excessive sleepiness. Cytokines may have an important role in mediating excessive daytime sleepiness in sleep loss or insomnia. Alterations of the immune system have also been associated wi…

[Psychiatric aspects of sleep].

Sleep disturbances are frequently the consequence of physiological and psychological problems and may be classified in three groups: diminished total quantity of sleep, loss of the quality and inadequate timing of the sleep process. It is considered that one third of the population has a sleep disturbance some time in the life. Insomnia is the most frequent sleep disturbance but timely and careful diagnostic procedure is fundamental for the specific treatment. Factors connected with the increased prevalence of sleep disturbance are female gender, presence of mental or somatic diseases and adva…

[Complex sleep apnea syndrome].

Some patients with obstructive sleep apnea (OSA) develop persistent central sleep apnea activity when started on continuous positive airway pressure (CPAP). This recently recognized syndrome, complex sleep apnea syndrome (CompSAS), is thought to be due to dysfunction of the respiratory center in a setting of an obstructive sleep apnea. We present the current knowledge on epidemiology, pathophysiology, and treatment options in CompSAS. We review the current literature on CompSAS, and present our own data on this interesting condition.

Circadian rhythm sleep disorders.

Humans exhibit endogenous circadian rhythms that are regulated by the master circadian clock of the body, the suprachiasmatic nucleus. These endogenous circadian rhythms are aligned to the outside world by social and environmental cues. Circadian rhythm sleep disorders (CRSD) occur when there is an alteration of the internal timing mechanism or a misalignment between sleep and the 24-h social and physical environment. CRSD are often underrecognized yet should be considered in the differential of patients presenting with symptoms of insomnia and/or hypersomnia. Because behavioral and environmen…

Sleep and vascular disorders.

It is not surprising that cardiovascular diseases such as congestive heart failure and coronary insufficiency can give rise to varying degrees of sleep impairment; it is less readily appreciated that certain physiologic events occurring during sleep-as well as long-term unsatisfactory sleep-may cause or increase the risk of cardiovascular conditions such as hypertension, atherosclerosis, stroke, and cardiac arrythmias. Heart rate abnormalities during sleep in normotensive subjects predict later cardiovascular disease, and their early identification alerts the physician to undertake preventive …

[Obstructive sleep apnoea syndrome].

Sleep apnoea syndrome is a frequent disease, occurring in men aged more than 30 years and in postmenopaused women. Its prevalence is more than 1% in men and between 0,3%-0,5% in women. It is an important public health problem. Diagnosis is made by polysomnography, which is considered as a gold standard, or by polygraphy. Sleep apnoea syndrome must be looked for by clinicians in presence of symptoms such as sleepiness, snoring, or behavioural problems, because these patients have a high risk not only of driving crashes, but also of arterial hypertension, brain strokes, and myocardial infarction…

Sleep and medical disorders.

Extensive evidence links cardiovascular disease and sleep disordered breathing. OSA has adverse effects on blood pressure, cardiovascular status, and mortality. Effective CPAP therapy can improve blood pressure and cardiac function in patients who have OSA. Patients who have congestive heart failure have a high prevalence of sleep-disordered breathing, with OSA occurring in 30% of such patients and Cheyne-Stokes respiration in 40%.CPAP is the preferred mode of therapy for both types of sleep-disordered breathing in patients who have coexistent congestive heart failure. Nocturnal worsening of as…

Suicidality and sleep disturbances.

STUDY OBJECTIVES: A growing body of research indicates that sleep disturbances may be specifically linked to suicidal behaviors. It remains unclear, however, whether this link is largely explained by depressive symptoms. The present study investigated the relationship between suicidality, depression, and sleep complaints in a clinical outpatient setting. DESIGN AND SETTING: Upon admission, 176 outpatients completed measures on sleep disturbances, suicidal symptoms, and depression. Several sleep disturbances were evaluated with regard to suicidal ideation, including insomnia, nightmares, and s…
