Body-First Subtype of Parkinson's Disease with Probable REM-Sleep Behavior Disorder Is Associated with Non-Motor Dominant Phenotype.

BACKGROUND: The hypothesis of body-first vs. brain-first subtype of PD has been proposed with REM-Sleep behavior disorder (RBD) defining the former. The body-first PD presumes an involvement of the brainstem in the pathogenic process with higher burden of autonomic dysfunction. OBJECTIVE: To identify distinctive clinical subtypes of idiopathic Parkinson's disease (iPD) in line with the formerly proposed concept of body-first vs. brain-first subtypes in PD, we analyzed the presence of probable RBD (pRBD), sex, and the APOEɛ4 carrier status as potential sub-group stratifiers. METHODS: A total

Heart rate variability impairment during sleep in Veterans with REM sleep behavior disorder, traumatic brain injury, and posttraumatic stress disorder: An early potential window into autonomic dysfunction?

UNLABELLED: Individuals with comorbid REM sleep behavior disorder (RBD) and neurotrauma (defined by traumatic brain injury and post-traumatic stress disorder) have an earlier age of RBD symptom onset, increased RBD-related symptom severity and more neurological features indicative of prodromal synucleinopathy compared to RBD only. An early sign of neurodegenerative condition is autonomic dysfunction, which we sought to evaluate by examining heart rate variability during sleep. Participants with overnight polysomnography were recruited from the VA Portland Health Care System. Veterans without n

Empatica E4 wristband assessment of probable REM sleep behavior disorder in people with Parkinson's disease. Results from the DIGI.PARK study.

BACKGROUND: Parkinson's disease is frequently accompanied by Rapid Eye Movement (REM) sleep behavior disorder (RBD), causing individuals to physically act out their dreams. The REM Sleep Behavior Disorder Questionnaire (RBDSQ) is a 13-items self-report tool to identify individuals with probable REM Sleep Behavior Disorder (pRBD). While self-report of symptoms is limited by inaccuracies in recall and subjective interpretation, some of the RBDSQ items concerns nocturnal motor behavior that could be suitable for digital assessment. Therefore, we examined the potential of the Empatica E4 wristband

Elevated plasma neurofilament light chain in isolated REM sleep behavior disorder: Associations with neurogenic orthostatic hypotension and implications for phenoconversion.

BackgroundIsolated REM sleep behavior disorder (iRBD) is a prodromal stage of α-synucleinopathies, including Parkinson's disease (PD), dementia with Lewy bodies (DLB), and multiple system atrophy (MSA). Evaluating trajectories of phenoconversion is crucial in this population. Plasma neurofilament light chain (NfL), a marker of axonal injury, has emerged as a promising candidate for tracking disease progression.ObjectivesTo assess plasma NfL levels in iRBD patients, examine their associations with clinical features, particularly neurogenic orthostatic hypotension (nOH), and explore their potent

The GABAB receptor agonist baclofen inhibits the reconsolidation of methamphetamine reward memory in adolescent and adult REM sleep-deprived rats.

Methamphetamine (METH) is a highly addictive psychostimulant drug. A key behavior of addiction is the relapse to drug-seeking and self-administration after abstinence. Like small amounts of a drug, does sleep deprivation increase the risk of relapse. Thus, identifying the mechanisms of relapse, and inhibiting them, is one strategy to develop a better treatment of addiction. In this study, adolescent and adult male rats underwent a 7-day episode of REM sleep deprivation (RSD). Following this, they were trained to establish a METH (2 mg/kg, i.p.)-induced conditioned place preference (CPP). After

Montreal Cognitive Assessment and the Clock Drawing Test to Identify MCI and Predict Dementia in Isolated REM Sleep Behavior Disorder.

BACKGROUND AND OBJECTIVES: Patients with isolated/idiopathic REM sleep behavior disorder (iRBD) are at high risk for developing mild cognitive impairment (MCI) and dementia with Lewy bodies (DLB). However, there is a lack of scientific knowledge regarding the accuracy of cognitive screening tools to identify these conditions in iRBD. This study aimed to determine in iRBD the psychometrics of 2 screening tests to discriminate patients with MCI and those at risk of DLB. METHODS: We retrospectively selected and followed 64 patients with polysomnography-confirmed iRBD seen in sleep clinic between

Elevated pain sensitivity is associated with reduced rapid eye movement (REM) sleep in females with comorbid temporomandibular disorder and insomnia.

OBJECTIVE: Patients with chronic pain disorders, including Temporomandibular Disorders (TMDs) endorse high levels of sleep disturbances, frequently reporting reduced sleep quality. Despite this, little is known about the effect that daytime pain has on the microstructure and macro-architecture of sleep. Therefore, we aimed to examine the extent to which daytime pain sensitivity, measured using quantitative sensory testing (QST), is associated with objective sleep parameters the following night, including sleep architecture and power spectral density, in women with TMD. METHODS: 144 females wi

Consistent skin α-synuclein positivity in REM sleep behavior disorder - A two center two-to-four-year follow-up study.

OBJECTIVE/METHODS: Phosphorylated alpha-synuclein (p-syn) in dermal nerves of patients with isolated REM sleep behavior disorder (iRBD) is detectable by immunofluorescence-labeling. Skin-biopsy-p-syn-positivity was recently postulated to be a prodromal marker of Parkinson's disease (PD) or related synucleinopathies. Here, we provide two-to four-year clinical and skin biopsy follow-up data of 33 iRBD patients, whose skin biopsy findings at baseline were reported in 2017. RESULTS: Follow-up biopsies were available from 25 patients (18 positive at baseline) and showed consistent findings over ti

Neuroprotective effect of exogenous melatonin on the noradrenergic neurons of adult male rats' locus coeruleus nucleus following REM sleep deprivation.

Melatonin is primarily secreted by the pineal gland in dark. In addition to its role as an internal sleep facilitator, melatonin acts as an antioxidant, anti-inflammatory and neuroprotective agents. melatonin has been introduced as a therapeutic strategy for sleep disorders. Hence, in the present study, we studied the neuroprotective effects of pre- and post-treatment of melatonin in locus coeruleus nucleus (LC) of rapid eye movement (REM) sleep deprived (REM-SD) male adult rats. Adult male rats of control, sham and trial groups were used Exogenous melatonin (ExMe) was intraperitoneally inject

Continued Presence of Period Limb Movements During REM Sleep in Patients With Chronic Static Pediatric Acute-Onset Neuropsychiatric Syndrome (PANS).

STUDY OBJECTIVES: A major component of pediatric acute-onset neuropsychiatric syndrome (PANS) is disruption of sleep. These disturbances have been reported in the acute phase of diagnosis but it is unknown if these sleep disruptions persist, especially in patients with chronic static symptoms. This retrospective chart review sought to review polysomnography (PSG) tests of patients in whom PANS has been clinically diagnosed in order to assess sleep architecture, periodic limb movements, and presence of rapid eye movement (REM) sleep without atonia (RSWA) after a chronic static course of symptom

Higher glymphatic system activity is linked to longer prodromal stage in isolated REM sleep behavior disorder: a possible protective factor.

Isolated rapid eye movement sleep behavior disorder (iRBD) is recognized as a prodromal stage of alpha-synucleinopathies. While glymphatic dysfunction has been investigated in Parkinson's disease, its role in iRBD remains incompletely understood. This study evaluated the association between glymphatic function and the speed-modulating effect to alpha-synucleionpathy conversion. Fifty-six patients (67.2 ± 7.1 [mean age ± SD]) with an iRBD and 48 control subjects (61.5 ± 10.0) underwent brain magnetic resonance imaging. Glymphatic function was evaluated using the diffusion tensor imaging along t

Olfactory impairment and probable REM sleep behavior disorder in Parkinson's disease: clinical relevance to the single- and dual-hit hypotheses.

BACKGROUND: Rapid eye movement (REM) sleep behavior disorder (RBD) and olfactory impairment are common prodromal features of Parkinson's disease (PD), but the implications of their coexistence remain uncertain. The dual-hit hypothesis proposes concomitant olfactory and enteric involvement, whereas the revised single-hit framework proposes predominantly brain-first or body-first routes. We examined whether probable RBD (pRBD)/anosmia-defined subgroups exhibit distinct clinical profiles. METHODS: In this cross-sectional study, 201 individuals with PD underwent olfactory, cognitive, motor, non-m

Disrupted brain functional network topology and potential network reorganization in early-stage Parkinson's disease with probable REM sleep behavior disorder.

PURPOSE: Parkinson's disease with rapid eye movement sleep behavior disorder(RBD) often represents a more aggressive subtype, presenting more serious clinical manifestations even in early-stage disease. However, this subtype's specific whole-brain functional network alterations in the early stages of the disease remain unclear. Research was designed to explore the unique functional network alterations patterns in early PD with RBD and underlying abnormal neural network development mechanisms. METHOD: We gathered resting-state fMRI data of35 healthy controls (HC), 33 early PD with probable RBD

Elevated levels of colonic interleukin-1beta and interleukin-8 in isolated REM sleep behavior disorder without associated changes in permeability.

Gastrointestinal inflammation could contribute to the early development of Parkinson's disease (PD). This study investigated gut inflammation, intestinal barrier function, and integrity in patients with isolated REM sleep behavior disorder (iRBD), a prodromal stage of PD. Sigmoid colon biopsies from patients with iRBD (n = 20), PD (n = 34), and controls (n = 20) were analyzed by qPCR to measure the expression levels of proinflammatory cytokines and enteric glial markers, along with fecal calprotectin. Gut permeability was evaluated in biopsies mounted in Ussing chambers, and the integrity of t

Association of DTI-ALPS Glymphatic Index With Differential Phenoconversion in Isolated REM Sleep Behavior Disorder: A Multi-Cohort MRI Study.

BACKGROUND AND OBJECTIVES: Isolated REM sleep behavior disorder (iRBD) is the strongest prodromal marker of synucleinopathies, including Parkinson disease (PD) and dementia with Lewy bodies (DLB). Identifying brain biomarkers that predict progression and distinguish phenoconversion trajectories remains a challenge. The glymphatic system is involved in interstitial waste clearance, and its dysfunction has been associated with pathologic protein accumulation and neurodegeneration. Diffusion tensor imaging along the perivascular space (DTI-ALPS) has been proposed as a noninvasive proxy for glymph

Gut-Initiated Alpha Synuclein Fibrils Drive Parkinson's Disease Phenotypes: Temporal Mapping of non-Motor Symptoms and REM Sleep Behavior Disorder.

Parkinson's disease (PD) is characterized by progressive motor as well as less recognized non-motor symptoms that arise often years before motor manifestation, including sleep and gastrointestinal disturbances. Despite the heavy burden on the patient's quality of life, these non-motor manifestations are poorly understood. To elucidate the temporal dynamics of the disease, we employed a mouse model involving injection of alpha-synuclein (αSyn) pre-formed fibrils (PFF) in the duodenum and antrum as a gut-brain model of Parkinsonism. Using anatomical mapping of αSyn-PFF propagation and behavioral
