Combined endoscopic transmaxillary-transnasal approach to the pterygoid region, lateral sphenoid sinus, and retrobulbar orbit.

OBJECTIVES: The sublabial transmaxillary approach to the pterygoid region was a popular one during the Caldwell-Luc, pre-endoscopic era. It was the procedure of choice for management of lesions of the pterygopalatine space, for internal maxillary artery ligation, and for vidian neurectomy. With the introduction of endoscopic instrumentation and techniques, the Caldwell-Luc procedure is rarely performed today. Also, because vidian neurectomy is performed less frequently, and because internal maxillary artery ligation for severe epistaxis has been replaced with transnasal endoscopic sphenopalati

The relationship between the sphenoid and the posterior ethmoid sinuses and the optic nerves in Turkish patients.

The sphenoid and the posterior ethmoid sinuses are surrounded by more vital structures than any other sinus. With the widespread acceptance and expanding role of endoscopic sinus surgery, a proper understanding of the anatomy of the sphenoid and the posterior ethmoid sinuses is achieved. We reviewed 100 CT studies of the paranasal sinuses belonging to patients examined for a chronic inflammatory disease of the nasal cavity and the paranasal sinuses. The relationship between the optic nerves and the paranasal sinuses is classified into four discrete categories. Type 1 with a proportion of 64% i

Solitary plasmacytoma of the sphenoid sinus involving the pituitary fossa: a case report and review of the literature.

A rare case of solitary plasmacytoma of the sphenoid sinus involving the pituitary fossa is reported. A 50-year-old woman with a history of diplopia and a mass in the sphenoid sinus and the sellar region, documented by computed tomography, was referred to our department with a presumed diagnosis of nonfunctioning pituitary adenoma. The clinical and biochemical characteristics were unrevealing, but magnetic resonance imaging examination demonstrated the extrapituitary origin of the lesion. The patient was operated on by the transsphenoidal approach, and the lesion was histologically diagnosed a

Intrasphenoidal encephalocele associated with cerebrospinal fluid fistula and subdural hematomas: technical case report.

OBJECTIVE AND IMPORTANCE: Intrasphenoidal encephalocele is a rare clinical entity that is often complicated by rhinorrhea, recurrent meningitis, and headache, but in no case has the association of rhinorrhea with subdural hematomas been described. A surgical procedure to stop persistent cerebrospinal fluid leakage is reported. CLINICAL PRESENTATION: A 59-year-old man sought care for intractable rhinoliquorrhea of 6 months' duration. Cranial computed tomographic and magnetic resonance imaging scans revealed a basal posterior frontal bony defect and an evocative image suggesting intrasphenoidal

Endoscopic transsphenoidal biopsy of the sphenoid and clival mass.

Abnormalities of the clivus/skull base occur most commonly as a result of disease spread from adjacent structures, but can also occur from primary involvement of the clivus. Traditionally, definitive diagnosis was made by CT guided biopsy or craniotomy. However, lesions can occur in this area that are not amenable to CT guided biopsy or craniotomy. Endoscopic transsphenoidal surgery can provide a safe method for obtaining a clival biopsy, debulking of tumor tissue, or definitive treatment without the morbidity and mortality of a craniotomy. This study was designed to describe our experience wi

Blindness after Le Fort I osteotomy: a possible complication associated with pterygomaxillary separation.

INTRODUCTION: Visual loss after Le Fort I osteotomy is a devastating complication the mechanism of which is not always clear. METHODS: A case report of blindness following Le Fort I osteotomy is presented. The literature on the various skull base complications associated with Le Fort I osteotomies is reviewed and the mechanisms of these complications discussed. CONCLUSION: The radiological findings in this case are similar to those previously reported. They strongly support the hypothesis that an adverse transmission of forces associated with pterygomaxillary separation via the sphenoid bone…

Proptosis secondary to subdural hematoma.

A subdural hematoma extending into the orbit through a fracture of the sphenoid bone appeared as unilateral proptosis with no neurologic abnormalities. Repeated percutaneous needle aspirations of the orbital portion of this mass provided only temporary relief of proptosis, but decompressed the intracranial portion of the hematoma adequately for prevention of neurologic manifestations. Ultrasonography correctly diagnosed the nature of the lesion and identified the bony dehiscence. Computed tomographic (CT) evaluation suggesting that a tumor might be present led to the performance of curative ne…

Atypical Intracranial Compound Simil Odontoma.

Odontoma is the most common mixed epithelial and mesenchymal tumour-like malformation which affects maxillary bones. Clinically, Odontomas present as lesions of benign behavior, causing little or no deformity, are asymptomatic, with self-limited growth and usually detected on routine radiographs. Radiographically, they are radiopaque lesions, well-demarcated surrounded by a thin soft tissue capsule. Compound odontomas consist of numerous tooth-like structures, whereas in complex odontomas it consists of a disorganized mass of calcified tissue. The aim of this article was to report an atypical

Primary chondrosarcoma of ethmoid bone in a 6-year-old child.

Chondrosarcoma (CS) is a distinctly uncommon tumor in young patients and tends to be located in the extremities. Cartilaginous tumors in young patients are benign tumors. We report a case of a childhood CS in a 6-year-old boy who presented with right-sided proptosis. Radiological examination revealed a large destructive mass in right ethmoidal sinus extending into the surrounding sphenoidal sinus, intracranially, and the into right orbit, causing proptosis. Biopsy of the mass showed characteristic features of a well-differentiated CS. There are no reports of CS presenting as proptosis and aris

Benign fibroosseous lesions involving the skull base, paranasal sinuses, and nasal cavity. Report of two cases.

The authors report two patients with benign fibroosseous lesions involving the center of the skull base: a 15-year-old boy with repetitive meningitis and pneumocephalus and a 11-year-old boy with nasal obstruction and headache. The clinical diagnoses were ossifying fibroma and aneurysmal bone cyst, respectively. Lesions in both patients extended to the nasal cavity, the sphenoid and posterior ethmoid sinuses, and the skull base, where the planum sphenoidale, the sella turcica, the upper two-thirds of the clivus, and the medial portion of the middle cranial base were involved. The lesions were

CT features of hyperostosing meningioma en plaque.

Hyperostosis of the sphenoid ridge or convexity of the skull associated with meningioma en plaque (MEP) is often confused with other hyperostosing conditions, such as fibrous dysplasia or osteoma. The authors present nine cases, six of them proven, of hyperostosing MEP with specific attention to the CT features. All nine cases had one or more CT features that were characteristic or suggestive of MEP. These included periosteal pattern of hyperostosis, inward bulging of the vault lesion, surface irregularity of the hyperostotic bone, and intracranial changes. The role of CT in evaluating MEP is

Orbital neurofibromatosis: involvement of the orbital floor.

Neurofibromatosis has been reported to involve the eye and orbit by causing irregularities of the sphenoid and other facial bones and enlargement of the foramina. To our knowledge, abnormalities of the orbital floor, however, have not been attributed to this disease in the literature until now. Two patients with neurofibromatosis had orbital floor involvement. Multiple neurofibromas eroding the orbital floor threatened vision in one patient. The other case, seen after trauma, had a condition simulating that of a blow-out fracture. Diagnostic workup with adequate roentgenography, especially tom

Congruence of individual cranial bone morphology and neutral molecular affinity patterns in modern humans.

Recent studies have demonstrated that the shape of the human temporal bone is particularly strongly correlated with neutral genetic expectation, when compared against other cranial regions, such as the vault, face, and basicranium. In turn, this has led to suggestions that the temporal bone is particularly reliable in analyses of primate phylogeny and human population history. While several reasons have been suggested to explain the temporal bone's strong fit with neutral expectation, the temporal bone has never systematically been compared against other individual cranial bones defined using

Single staged transorbital neuroendoscopic (TONES) approach of managing pulsatile proptosis using patient specific implant (PSI) in orbitopalpebral neurofibromatosis.

Sphenoid wing dysplasia and plexiform neurofibromas are among the diagnostic criteria for neurofibromatosis 1 (NF1), an autosomal dominant neurocutaneous disorder. It has been called by different names based on the involved anatomic areas such as orbito-palpebral, orbito-temporal, and orbito-facial plexiform neurofibromatosis. Management of the deformity in plexiform neurofibromatosis is challenging given the ill-defined, infiltrative, and vascular nature of the lesion, associated with a defect in the greater wing of sphenoid, resulting in pulsatile proptosis. A staged multidisciplinary approa

Adult Burkitt lymphoma originating in the sphenoid sinus: case report and review of the literature.

Burkitt lymphoma is a high-grade B-cell non-Hodgkin lymphoma. The endemic form of this malignancy occurs primarily in children aged 5 to 7 years, and it presents with jaw and facial bone involvement. The sporadic form affects older children (mean age: 12.2 yr) and often manifests as an abdominal mass; it rarely involves the head and neck. The presence of any type of lymphoma in the paranasal sinuses is rare. We report a case of Burkitt lymphoma that originated in the sphenoid sinus in a 66-year-old white woman. The patient presented with hypoesthesia in the left V1 and V2 distributions and fre

Tension pneumocyst after transsphenoidal surgery for Rathke's cleft cyst: case report.

OBJECTIVE AND IMPORTANCE: Tension pneumocephalus is a rare but well-described complication of transsphenoidal surgery. It is usually associated with postoperative cerebrospinal fluid fistulae causing lower intracranial pressure, with air located in the subdural, subarachnoid, or intraventricular space. We report a case of suprasellar tension pneumocyst that caused visual deterioration to develop after an operation for a Rathke's cleft cyst. Only one similar case has been reported previously. CLINICAL PRESENTATION: A 54-year-old woman with a cystic sellar-suprasellar mass compressing the chias

[Endoscopic treatment of sphenoid aspergilloma].

In recent years there has been an increase in fungal infections of the nose and paranasal sinuses, even though it is difficult to ascertain the incidence and prevalence of such disorders. The only information which can be drawn from the literature is that such infections are diagnosed in 6% to 13% of surgically treated chronic sinusitis. Aspergillus is the etiologic agent in approximately 80% of cases. Fungal infections are classified into two groups: non-invasive and invasive. The former includes mycetoma and allergic fungal sinusitis. The latter comprises the chronic indolent form, which pre
