Post-traumatic thoracic scoliosis with rib head dislocation and intrusion into the spinal canal: a case report and review of literature.

The objective of the study was to report a rare occurrence of dislocation and intrusion of two rib heads into the spinal canal at the convex apex of a post-traumatic thoracic scoliosis in an adult in the absence of any neurological impairment. A 47-year-old male presented with a slowly progressive, post-traumatic thoracic scoliosis and a mild aching sensation over the posterior chest wall. The lower limb neurology and bowel and bladder function were normal. There was no clinical evidence of neurofibromatosis. CT scans showed that the 8th and 9th ribs on the convex apex of the scoliotic curve h…

[HAC-titanium as an implant for interbody fusion in spinal canal stenosis of the cervical spine. Six-year clinical trial].

A total of 102 patients were operated on in 134 levels because of an isolated spondylotic cervical stenosis in 6 years. A titanium implant coated with hydroxyapatite ceramic (HAC) was inserted. The average age of the patients was 53 years. The final clinical and radiological follow-up was performed after 15 months. The functional outcome was classified as good to excellent (Odoms's score) in 83.8% of the patients. Implant-related complications requiring revision surgery included two dislocated dowels and one vertebral collapse in a bisegmental operation. After 6 weeks beginning of bony growth

Lumbago and radicular complaints: not always a disc hernia or degenerative stenosis of the spinal canal. A differential diagnosis of infrequent diseases.

Low back ache and pain in the legs are not always due to disc displacement and lumbar spinal degenerative changes. Some infrequent, but really not very rare diseases are presented in order to avoid mistakes which can have serious consequences for the patients. Degenerative changes of the lumbar spine are very common, not only in aged people. A superficial examination of the patient with back ache and/or pain in the legs can lead to a fallacious tie-up between such lesions and disturbances and complaints which are not connected to them.

A review of biomechanics of the central nervous system--Part I: spinal canal deformations resulting from changes in posture.

OBJECTIVE: To discuss how the spinal cord deforms as a result of changes in posture or biomechanical alterations of the spine. DATA COLLECTION: A hand search of available reference texts and a computer search of literature from the Index Medicus sources were collected, with special emphasis placed on spinal canal changes caused by various postural rotations and translations of the skull, thorax, and pelvis. RESULTS: All spinal postures will deform the spinal canal. Flexion causes a small increase in canal diameter and volume as the vertebral lamina are separated. Extension causes a small dec…

[Spontaneous "regeneration" of the spinal canal in traumatic bone fragments after fractures of the thoraco-lumbar transition and the lumbar spine].

This is a report about four patients in whom spontaneous remodelling of the spinal canal was observed after thoracolumbar fractures with intraspinal bone fragments leading to narrowing by 33-93%. One of the patients was female and three male; two patients had partial neurologic deficits and two had none. The ages of the patients were 33, 16, 65 and 16 years, and the fractures were localized in one case in segment T-12 in two in L-1 and in one in L-4. Active functional treatment was performed with no repositioning or plastering. The changes in spinal canal narrowing and the bone union were moni…

[Results after conservatively managed vertebral fractures of the thoracolumbar level with special reference to bony stenosis of the spinal canal].

Seventy-two patients with compression fractures of the lower thoracic and lumbar spine who were treated by the Böhler method from 1972 to 1987 were followed up to compare the clinical signs and the results of conventional radiography and spinal computed tomography. There were 33 men and 39 women with an average age of 44.3 years (23-69 years). Treatment by the Böhler method is indicated for fractures between T12 and L5, which constitute 60% of all vertebral fractures. The main distribution in our patients was T12 (n = 26) and L1 (n = 33). Four patients primarily had neurological symptoms. The

[Sonographic determination of spinal canal width. Comparison of computed tomography, myelography and conventional roentgen study of the lumbar spine].

Sonographic evaluation of the width of the spinal canal is inferior compared to CT or myelography, concerning accuracy and especially quantity of information received. The advantage of sonography is the lacking of X-ray exposure, the non-invasive procedure and the low cost of apparatus and operational time. For these reasons sonography is favoured as a screening method in detecting spinal canal stenosis in a group whose handicap was largely occupational or sport-related.

Smaller thoracic canal diameters are associated with thoracic radiculopathy and abdominal pain after spinal cord stimulator paddle lead placement.

INTRODUCTION: It is not uncommon for patients to experience postoperative neurologic deficit, thoracic radiculopathy, abdominal pain, or lower extremity paresthesia after the implantation of thoracic spinal cord stimulator (SCS) paddle leads. Smaller thoracic canal diameters have previously been associated with postoperative neurologic deficits. OBJECTIVE: This imaging study examined whether postoperative SCS neurologic complaints other than neurologic deficit may be correlated with thoracic spinal canal diameter. METHODS: Patients who underwent thoracic laminotomy for SCS paddle lead placem…

A Nomogram to Predict Intra-Spinal Canal Cement Leakage Among Elderly Patients with Spine Metastases: An Internal-Validated Model.

PurposeThis study aimed to assess the risk variables for predicting intra-spinal canal cement leakage, especially among elderly patients with spine metastases after being treated with percutaneous vertebroplasty (PVP). Furthermore, we proposed and validated a nomogram to stratify risks of intra-spinal canal cement leakage.MethodsWe retrospectively analyzed 163 elderly patients (age ≧65 years) with spine metastases who underwent PVP. Patients were randomly divided into a training cohort (n=100) and a validation cohort (n=63). The multivariate logistic regression analysis was used to screen pote…

Quantitative and qualitative analyses of spinal canal encroachment during cervical laminectomy using the kerrison rongeur versus High-Speed burr.

BACKGROUND: Several cervical laminectomy techniques have been described. One commonly used method involves making bilateral trough laminotom … in less canal encroachment compared to 1 mm or 2 mm Kerrison rongeurs. In the setting of a stenotic spinal canal, spine surgeons should consider using the burr to perform laminectomy to minimize the degree of canal encroachment.
