Minimal access approach for the repair of atrial septal defect: the initial 135 patients.

BACKGROUND: From May 1996 to August 1998 a minimal access approach was used for 135 of 200 consecutive surgical atrial septal defects closures in children through young adults ranging in age from 6 months to 25 years (median 5 years). METHODS: A 3.5- to 5-cm midline incision was centered over the xiphoid with division of the xiphoid alone (transxiphoid) or of the lower sternum (ministernotomy); both groups underwent bicaval venous cannulation through the incision. Cardioplegia and aortic cross-clamping were administered through the incision. Cephalad retraction of the sternum with a fixed-arm…

Cervico-pectoral ectopia cordis in two Holstein calves.

Two Holstein calves affected with cervico-pectoral ectopia cordis, a male (No. 1) delivered stillborn and a female (No. 2) died 1 hour after birth, were examined macroscopically and radiographically to assess the severity and elucidate the teratogenesis of the anomaly. The heart of one calf was covered by the intact pericardium and skin and displaced to the caudalmost portion of the ventrocervical region, just cranial to an enlarged thoracic inlet. The cranial vena cava and the vena azygos were duplicated. The sternum was bilaterally divided into two parts fused only at the xyphoid process and…

[Sternocostoclavicular hyperostosis: a space-occupying lesion of the superior thoracic aperture].

Sternocostoclavicular hyperostosis is a rare disease characterized by soft-tissue ossifications and hyperostosis of the clavicles, anterior segments of the upper ribs and sternum. These changes may imitate other disorders and thus may be misinterpreted. We present the radiologic appearances of this entity and discuss the diagnostic value of computerized tomography, magnetic resonance imaging and scintigraphy.

[Diagnostic and treatment principles in primary tumors of flat bones].

Tumors of ribs, the scapula, sternum and clavicle make 5.6% of all bone tumors. To diagnose the former it is necessary to use a complex method, which includes along with clinico-roentgenological also special (radioisotope, morphological) methods of examination. The treatment of these lesions depends on the character of tumor process, its stage, histogenesis and localization.

Cardiac Transplantation and Consecutive Minimally Invasive Pectus Excavatum Repair.

Pectus excavatum is a common chest wall deformity with inward deviation of sternum and accompanying ribs. The depression can cause symptomatic cardiac compression, although the cardiopulmonary impact remains controversial. We present 2 cases of cardiac transplantation followed by modified minimally invasive pectus excavatum repair due to the hemodynamic consequences of the pectus deformity.

Anomalous anterior midline cervical tube.

Anterior midline cervical anomalies, excepting thyroglossal tract abnormalities, are rare. Two unusual and almost identical cases of an anomalous congenital tube connecting the chin to the upper sternum are described; only one previous similar report exists in the literature. In all three cases, the tubes occurred as isolated anomalies in otherwise healthy children.

[Recurrent thoracic abscess due to Salmonella typhi].

Abscess formation by Salmonella typhi is an unusual manifestation. A woman aged 62 suffered from recurrent abscess in the anterior thorax for 35 years, which was released by spontaneous pus drainage each time. Pus cultures grew S. typhi, and the CT scan revealed a large abscess enclosing the sternum and left ribs.

Galen: The First Cardiac Surgeon?

It is widely believed that the first and the second pericardium surgeries were done in the nineteenth century by Francisco Romero and Dominique Jean Larrey, respectively; however, Galen was the first surgeon who proceeded with pericardiectomy. This ancient case report of sternum osteomyelitis and pericardiectomy illuminates some dark part of the history of medicine and the ability of physicians in that era.

A newborn infant with sternal malformation/vascular dysplasia association.

Sternal malformation/vascular dysplasia complex was described by Hersch et al. in 1985. The principle findings include cleft of the sternum covered by an atrophic skin, a midline abdominal raphe and hemangiomatosis. The inheritance pattern seems to be sporadic. We report a newborn baby with sternal defect, cleft lip and palate, supraumbilical raphe and hemangiomas.

Retropharyngeal abscess associated with tuberculosis of the cervical spine.

A rare case of retropharyngeal abscess in association with tuberculosis of the upper cervical spine is presented. The disease was associated with tuberculosis of the sternum. The onset was insidious. Fever, neck pain, dysphagia and hoarseness of voice were the presenting features. Conservative treatment with anti-tuberculosis drugs resulted in good outcome. The case is discussed and pertinent literature is reviewed.

Presternal bronchogenic cysts.

A presternal subcutaneous cyst was removed from a 21-month-old male infant. This is a rare congenital lesion that typically appears as a pinpoint opening over the sternum that periodically drains small amounts of mucinous material. Histologically, these lesions have the features of their intrathoracic and mediastinal counterparts. Surgical excision is advocated because of the potential for infection.

Absent mesosternum in congenital heart disease.

The association of developmental abnormalities of the sternum with congenital heart disease is well known. These abnormalities include hypoplasia, multiple centers, and premature fusion. The absence of the sternal bodies in patients with congenital heart disease unrelated to a specific syndrome complex has not been previously described. Possible association with congenital pulmonary vascular disease was noted.

Sternal cleft with surgical repair and 3-year follow-up: case report and literature review.

Sternal cleft is a rare congenital condition associated with unprotected viscera and respiratory failure. We present a case of a sternal cleft, absent right clavicle, and congenital diaphragmatic paresis in a neonate. The sternum was closed primarily, and the patient was taken back for plication of the diaphragm. At 3 years follow-up, the patient is free from complications.

Chest reconstruction in thoracic dystrophy.

Enlargement of the thoracic cage by splitting of the sternum and fixation with bone grafts proved a life-saving measure in a child with thoracic dystrophy and severe respiratory failure whose condition was deteriorating in spite of artificial ventilation. Supported ventilation has, however, remained necessary chiefly because of difficulty in achieving adequate stability of the thoracic cage.

Right Anterior versus Right Transaxillary Access for Minimally Invasive Aortic Valve Replacement: A Propensity Matched Competitive Analysis.

(1) Background: Right anterior thoracotomy (RAT-AVR) has been the sole established sternum-sparing technique for minimally invasive aortic valve replacement (MICS-AVR) thus far. Nevertheless, transaxillary access, known as Minimally Invasive Cardiac LATeral Surgery (MICLATS-AVR), represents the latest and innovative advancement in sternum-sparing MICS-AVR access routes. In this study, procedural and clinical outcomes of a substantial transaxillary MICS-AVR cohort are compared to those of a RAT-AVR control group; (2) Patients and Methods: This retrospective study included 918 consecutive patien…

Surgical management of traumatic sternal body nonunions: Technical tricks and clinical experience.

BACKGROUND: Rarely, traumatic sternum fractures may result in nonunion, which can have drastic, negative implications. Literature on traumatic sternal nonunion reconstruction outcomes is limited to case reports. We present the surgical principles and report clinical outcomes for seven patients following surgical reconstruction of a traumatic sternal body nonunion. METHODS: Consecutive adult patients with a nonunion after a traumatic sternum fracture who underwent reconstruction using locking plate technology and iliac crest bone graft at a Level I trauma center from 2013 to 2021 were identifi…
