OBJECTIVE: To investigate in-vitro thyroid stimulatory activity in the serum of patients with hyperemesis gravidarum and thyrotoxicosis.
DESIGN: Serum from hyperthyroid patients was incubated with cultures of human thyroid cells. Attempts were made to neutralize stimulatory activity with antisera to hCG.
PATIENTS: Five patients presenting in early pregnancy with hyperemesis and thyrotoxicosis.
MEASUREMENTS: Serum concentrations of thyroid hormones (total and free), TSH and hCG. Accumulation of extracellular cAMP in response to serum.
RESULTS: All five patients had biochemical hyperthyroidi

The objectives to be achieved by a medical therapy of constipation are: 1) to protect the patient from excessive use of dangerous drugs (laxatives), 2) to help the patient to understand what is a "normal" intestinal function, 3) to reduce or eliminate pain and 4) to avoid complications. The first step consists in general measures (reduced stress, regular meals and physical exercise) and some modifications in diet habits (greater than 1.5 1 of water a day, vegetables, fruits, whole wheat bread). The pharmacological therapy is based on drugs which act in different ways: a) some contain unabsorba

Inhaled capsaicin in humans causes cough and transient increase in airways resistance: both these effects appear to be through stimulation of sensory nerves in the airway. It has therefore been possible to study the effect of pharmacological agents on these two reflexes. The induced cough can be modulated by using opiates and local anaesthetics and the increase in airways resistance by anticholinergic agents. The use of inhaled capsaicin has therefore proved a useful human model for the study of novel treatments of cough.

1. We investigated the effect of trypsin (Tryp) on basal, stimulated and fluphenazine (FPZ)-inhibited net water flow (Jw) through isolated toad skin (Bufo arenarum). 2. Epidermal Tryp (20 min) promoted an increase in basal Jw which was dose-dependent (maximal with 0.5 mg/ml) and was prevented by a Tryp inhibitor (SBTI). 3. Tryp treatment inhibited the subsequent response to substances known to act before (oxytocin, Oxy) or after cyclic AMP (cAMP) generation (theophylline). 4. Tryp-induced Jw was not additive with the maximal response to Oxy or theophylline and did not modify FPZ's inhibitory e

We have studied the effects of acetorphan, an inhibitor of enkephalinase, on the electrical activity of human rectum. Acetorphan activated the rectum of healthy volunteers by inducing bursts of spike potentials, this activation might become cyclical. In contrast, this drug failed to modify the motility of the aganglionic portion of the rectum in one case of Hirschsprung's disease, indicating that the effect of acetorphan involves the intramural plexuses. Activation of the rectum persisted in spite of the blockade of the cholinergic excitatory pathways by atropine, which confirms the existence

The effect of 5-hydroxytryptamine (5-HT) on the crypt cell production rate in normal human duodenal mucosa cultured in vitro was studied using a stathmokinetic technique with crypt microdissection. The addition of 5-HT (2 mg/ml) to the culture medium significantly increased the crypt cell production rate (p less than 0.001), suggesting that this amine may have a trophic effect on crypt cells in vitro. Increased synthesis and release of 5-HT from enterochromaffin cells in patients with coeliac disease may increase the crypt cell production rate and lead to a flattened small intestinal mucosa.

An odor identification test and an odor threshold test offered satisfactory quantitative information on olfactory function in patients with chemosensory complaints. The threshold test used various concentrations of butanol presented in an ascending sequence. On each trial, the participant had to choose between stimulus and blank (two-alternative forced-choice procedure). The identification test used common odorous items (e.g., baby powder and ground coffee) and pungent items (e.g., ammonia) to test trigeminal function. A list of odor names and use of corrective feedback during testing overcame

The effects of somatostatin on gastrin release and total gastrin immunoreactivity were examined under in vitro conditions in rat antral organ culture experiments. Basal antral gastrin release was inhibited by somatostatin. Total culture gastrin contents (culture medium gastrin plus extracted antral mucosal gastrin) at 6 h were reduced significantly by 10(-5) M (p less than 0.02) and 10(-4) M (P less than 0.01) somatostatin. Gastrin release into the culture media stimulated by the cholinergic agent, carbachol (10(-5) M), was suppressed by somatostatin: at 30 min and 6 h of culture 10(-8) M soma

The effect of variations of intra-antral pH on the intraluminal release of somatostatin was studied. Acute pouches were created in anaesthetized cats, and the pouches were perfused with solutions differing in pH. Somatostatin levels were then measured in the perfusates. In this model phosphate buffer was a potent stimulator of intra-antral somatostatin release, whereas perfusion with 0.1 M HCl failed to release somatostatin by itself. Since phosphate buffer also releases gastrin, the releasing effect ought to be exerted beyond the mechanism that can be blocked by somatostatin. (Thus the stimul

Different peptides of natural origin were studied for their stimulant activity on the stomach of the anaesthetized rat. The group of the tachykinins (substance P and its analogues) showed a noticeable spasmogenic activity on the whole stomach from the fundus to the pylorus. Threshold doses ranged between 0.1 and 5 microgram/kg by i.v. route and the order of potency was: eledoisin greater than phyllomedusin greater than physalaemin greater than uperolein greater than substance P. A good correlation between the dose and the duration of the spasmogenic effect was always observed and tachyphylaxis

The gastric acid secreted per unit of serum gastrin concentration or per unit of gastrin delivered to the gastric mucosa was significantly greater during electrical vagal stimulation than during stimulation by meat extract in the pyloric antrum. There was no significant difference in the concentrations of big, big gastrin or Components I and II in gastric venous serum during the two forms of stimulation. There were significantly greater concentrations of Components II and IV in gastric venous serum during meat extract stimulation than during vagal stimulation. The gastrin in gastric venous ser

To assess whether or not a persistent antral response to meat extract stimulation is an indication of incomplete vagotomy, 29 patients with a duodenal ulcer who underwent operation had preoperative and postoperative acid studies as well as postoperative Hollander tests. In the preoperative tests, two groups of patients were delineated--antral and nonantral responders. Postoperative Hollander tests revealed three positive responses; in two patients with an early positive response, meat extract led to a significant increase in acid and gastrin response. In the patient with a late positive Hollan
