An unusual presentation of strongyloidiasis in a patient on chronic dialysis causing clinical deterioration.

BACKGROUND: Strongyloidiasis is a parasitic infection caused by the helminth Strongyloides stercoralis. Strongyloidiasis is endemic in tropical regions, including Mali. Diffuse forms of the disease occur in seriously immunocompromised patients and those undergoing immunosuppressant therapies, such as corticosteroids. We report an unusual case of disseminated strongyloidiasis in a patient undergoing chronic hemodialysis in Mali. CASE PRESENTATION: A 62-year-old woman from Mali with a past medical history of diabetic nephropathy was followed in our clinic. She has been undergoing chronic hemodia…

International Survey on Screening and Management of Strongyloidiasis in Solid Organ Transplant Patients.

BACKGROUND: Strongyloides stercoralis infection can be associated with high mortality in solid organ transplant (SOT) recipients, yet global practices regarding screening and management remain poorly characterized. We conducted an international survey to evaluate knowledge, screening, and management practices among healthcare workers (HCWs) involved in SOT care. METHODS: We performed a cross-sectional, web-based survey of HCWs caring for SOT candidates and recipients. The survey assessed provider characteristics, knowledge of strongyloidiasis, as well as screening and management practices. De…

Serodiagnosis of strongyloidiasis in a low-endemic setting - a two-tiered test approach.

BACKGROUND: Previous studies have demonstrated that Strongyloides ELISAs frequently exhibit cross-reactivity with infections caused by other helminths. This may result in false-positive results, especially in low-endemic areas, potentially leading to unwarranted treatment. This study aimed to present a new Western blot for the detection of anti-Strongyloides antibodies such as to enhance the accuracy of distinguishing true-positive from false-positive ELISA results. METHODS: We developed and validated a Western blot to detect anti-S. stercoralis specific antibodies using reference sera from c

A Rare Case of Cutaneous Strongyloidiasis in an Immunocompromised Patient: Clinical Insights and Implications.

BACKGROUND: Strongyloides stercoralis (S. stercoralis) is a soil-transmitted nematode endemic to tropical and subtropical regions. While infections often remain asymptomatic in immunocompetent individuals, they can progress to hyperinfection or disseminated disease in immunocompromised hosts, particularly those receiving corticosteroid therapy. Disseminated strongyloidiasis is associated with a high mortality rate if not promptly diagnosed and treated. CASE PRESENTATION: We present the case of a 45-year-old woman from northern Iran with a long-standing history of corticosteroid use for rheuma…

Clinical presentation and diagnosis of imported strongyloidiasis at a tertiary hospital, Stockholm, Sweden.

BACKGROUND: Since Strongyloides can persist in its host for decades, and cause life threatening infections data on prevalence, the burden and risk factors for infection is crucial in migrant populations. METHODS: In this observational retrospective cohort study, we describe the epidemiological, clinical, and microbiological characteristics of imported strongyloidiasis diagnosed at the Karolinska University Hospital, Stockholm, Sweden, during 2010-2021. RESULTS: We identified 98 individuals with strongyloidiasis, 89 (90.8%) born in endemic and 9 (9.2%) in non-endemic countries. Sub-Saharan Af…

Pulmonary strongyloidiasis and hyperinfection in a Patient with idiopathic inflammatory myopathy:A case report.

Pulmonary strongyloidiasis is a rare infection in patients with autoimmune diseases, and immunosuppression can lead to the development of hyperinfection syndrome with a high mortality rate. We present a case of a 78-year-old male with previous idiopathic inflammatory myopathy (IIM) with interstitial lung disease. He developed hyperinfection syndrome and respiratory failure, and diagnostic metagenomic next-generation sequencing (mNGS) of bronchoalveolar lavage fluid (BALF) confirmed the presence of Strongyloides stercoralis. After treatment with ivermectin, the patient's symptoms improved. Ther…

Immune complexes as a tool for strongyloidiasis immunodiagnosis in kidney and liver transplant candidate.

Strongyloidiasis is a chronic and asymptomatic infection in immunocompetent patients. Immunocompromised patients, such as organ transplant candidates, can develop severe forms of this disease, and the best way to prevent progression to these forms is early diagnosis. Serological techniques using specific IgG and immune complexes (IC) detection can help in the diagnosis of these patients. This study aimed to detect specific anti-Strongyloides IC and IgG antibodies in kidney transplant (KT) and liver transplant (LT) candidates. A total of 100 blood samples was collected from transplant candidate…

Prevalence of strongyloidiasis in the general population of the world: a systematic review and meta-analysis.

Strongyloides stercoralis is a neglected soil-transmitted helminth affects approximately 100-370 million people globally. The life cycle is unusual as only larvae can be found in stool specimens. Thecurrent review and meta-analysis represented the distribution of strongyloidiasis in general population of the world based on published papers. Five English databases (Science Direct, Scopus, PubMed, Web of Science, and Google Scholar(were explored for literature published before October 2019.Altogether 235 studies (862243 participants) was eligible. Regarding diagnostic method, the overall prevale…

Strongyloidiasis in Children Outside the Tropics: Do We Need to Increase Awareness?

Strongyloidiasis belongs to the group of neglected tropical diseases, due to diagnostic difficulties and the lack of systematic screening. Studies on strongyloidiasis prevalence are often heterogenous and mainly performed in adults in endemic countries. We retrospectively enrolled 2633 children referred to a tertiary care hospital in Italy between 2009 and 2020 and tested for S. stercoralis infection. Sixty-one (2.3%) had a positive serology and for 55 of them, clinical and epidemiological information were available. Thirteen cases (24%) were diagnosed in Italian children without history resid…

Prevalence of strongyloidiasis and schistosomiasis among migrants: a systematic review and meta-analysis.

BACKGROUND: Global migration from regions where strongyloidiasis and schistosomiasis are endemic to non-endemic countries has increased the potential individual and public health effect of these parasitic diseases. We aimed to estimate the prevalence of these infections among migrants to establish which groups are at highest risk and who could benefit from screening. METHODS: We did a systematic review and meta-analysis of strongyloidiasis and schistosomiasis prevalence among migrants born in endemic countries. Original studies that included data for the prevalence of Strongyloides or Schisto…

The Health Effects of Strongyloidiasis on Pregnant Women and Children: A Systematic Literature Review.

Strongyloidiasis is a helminth infection that remains under-researched despite its ability to cause significant illness. Women and children may be at particular risk of health consequences from this parasite. This systematic literature review aims to examine research on the long-term health effects that strongyloidiasis has in pregnant women and children. We conducted a structured search using multiple databases to collect all primary studies discussing health effects of strongyloidiasis in the aforementioned groups. The review included 20 results: 16 primary studies and four case reports. The…

Epidemiological Characteristics of Strongyloidiasis in Inhabitants of Indigenous Communities in Borneo Island, Malaysia.

Epidemiological study on strongyloidiasis in humans is currently lacking in Malaysia. Thus, a cross-sectional study was carried out to determine the prevalence of Strongyloides stercoralis infection among the inhabitants of longhouse indigenous communities in Sarawak. A single stool and blood sample were collected from each participant and subjected to microscopy, serological and molecular techniques. Five species of intestinal parasites were identified by stool microscopy. None of the stool samples were positive for S. stercoralis. However, 11% of 236 serum samples were seropositive for stron…

Notes from the field: Strongyloidiasis in a rural setting--Southeastern Kentucky, 2013.

Strongyloidiasis is caused by Strongyloides stercoralis, a parasitic nematode (worm). Initial symptoms can include abdominal pain, diarrhea, or rash. Infection is often asymptomatic in the chronic phase but can be life-threatening in immunosuppressed persons. Transmission typically occurs when larvae from stool-contaminated soil penetrate skin; intraintestinal autoinfection is also possible, sometimes allowing infection to persist for decades. Serologic studies are often used in prevalence estimates because intermittent shedding can make stool-based testing insensitive. Strongyloidiasis is mos…
