Catastrophic failure of a titanium locking plate in a proximal humeral fracture: case report and literature review.

BACKGROUND: Angular stable locking plates have shown good clinical results in treating proximal humeral fractures, but complications are not uncommon. This study reported a rare case of catastrophic failure of a titanium locking plate. A retrieval analysis of the implants was performed using an optic microscope and a scanning electron microscope. CASE PRESENTATION: A 69-year-old male reported a right proximal humeral fracture at the surgical neck and was treated by open reduction and internal fixation with a locking plate system. Ninety-six days after surgery, the patient came to clinic for a

The pillars of the scapula.

Total shoulder replacement has been shown to provide predictable pain relief and functional improvement in patients with glenohumeral arthritis. Loosening of the glenoid component remains the most frequent indication for revision surgery at long-term follow-up. The component most widely used is an all-polyethylene keeled or pegged design cemented to the glenoid cavity of the scapula. The glenoid is small and its cup-shaped morphology allows only a restricted site for limited fixation devices. This is particularly so in revision surgery where there are often large bony defects of the glenoid. I

Risk factors of excessive sliding in elderly patients with intertrochanteric fractures treated with PFNA-II: a retrospective observational study.

PURPOSE: Excessive sliding of cephalic components of cephalomedullary nails has been established to be significantly associated with the development of mechanical failures and unfavorable results in the surgical treatment of intertrochanteric fractures. This study aims to elucidate the risk factors that contribute to excessive sliding in elderly patients treated with PFNA-II devices for the fixation of intertrochanteric fracture. METHODS: We conducted a retrospective analysis of patients aged 65 and older who presented with intertrochanteric fractures and underwent surgical treatment using PF

Titanium Plug Closure after HeartWare Ventricular Assist Device Explantation in a 15-Year-Old Girl: First U.S. Experience.

We describe the case of a teenage girl with anthracycline-induced cardiomyopathy who received a HeartWare ventricular assist device and underwent successful device explantation after cardiac recovery. During device support, the patient's cardiac function returned to normal. Twelve months after implantation, we explanted the device via repeat median sternotomy. To close the hole in the left ventricular apex and preserve the sewing ring in case future device support is needed, we used a German-manufactured titanium plug, developed specifically for this purpose. To our knowledge, this is the firs

Outcome of Bilateral C1 Laminar Hooks Combined With C2 Pedicle Screw Fixation for the Treatment of C1-C2 Instability: A Report of 18 Cases From a Single Chinese Center.

STUDY DESIGN: A retrospective technical report. OBJECTIVE: To assess the effect of bilateral C1 laminar hooks combined with C2 pedicle screw fixation for the treatment of C1-C2 instability. SUMMARY OF BACKGROUND DATA: Various posterior atlantoaxial fixations for C1-C2 instability have been developed. However, due to anatomic anomalies of the vertebral artery, the smallness of the pedicle, trajectories of broken screws, or a lack of surgical experience, a simple atlantoaxial fixation technique with good safety and effectiveness is urgently needed. MATERIALS AND METHODS: From January 2007 to

Bone realignment with use of temporary external fixation for distal femoral valgus and varus deformities.

BACKGROUND: Correction of a distal femoral deformity may prevent or delay the onset of osteoarthritis or mitigate its effects. Accurate correction of deformity without production of a secondary deformity depends on precise localization and quantification of the deformity. We report a technique to correct distal femoral deformities in the coronal plane. METHODS: Fourteen femora in thirteen skeletally mature patients with a distal femoral deformity underwent operative reconstruction. The preoperative deviation of the mechanical axis ranged from 90 mm laterally (genu valgus) to 120 mm medially (

Proximal Femoral Nail Unlocked versus Locked (ProFNUL): a protocol for a multicentre, parallel-armed randomised controlled trial for the effect of femoral nail mode of lag screw locking and screw configuration in the treatment of intertrochanteric femur fractures.

INTRODUCTION: Intertrochanteric fractures are common fragility injuries in the elderly. Surgical fixation using intramedullary devices are one of the widely used management options. To date, evidence demonstrating the effects of lag screw configuration and the mode of lag screw locking in these devices is lacking. The purpose of this study is to investigate whether the lag screw configuration (single vs integrated dual interlocking screw) and the mode of lag screw locking (static vs dynamic) of a femoral nail device result in differences in clinical and functional outcomes. METHODS AND ANALYS

A Novel Intramedullary Locked Fixation Device for Treatment of Clavicle Shaft Fractures.

INTRODUCTION: An intramedullary nail (the Sonoma CRx) with a unique flexible anatomical design and locking system is a viable alternative for treatment of displaced and shortened clavicle shaft fractures. STEP 1 POSITIONING: Place the patient in the beach-chair position and drape the arm free so that you can obtain an exaggerated anteroposterior and an axial fluoroscopic view of the clavicle by manipulating the position of the arm. STEP 2 SKIN INCISION: The surgical technique necessitates opening the fracture site in order to implant the device. STEP 3 PREPARING THE MEDIAL FRAGMENT: The pre

Arthroscopy-assisted Treatment of Displaced Distal Clavicle Fractures Utilizing Curved Buttons and Suture Tape With a Single Coracoclavicular Tunnel.

BACKGROUND: Distal third clavicle fractures are associated with high rates of nonunion. Acceptable healing rates have been reported; however, complications and the need for additional procedures are a concern. The purpose of this study was to evaluate the functional outcomes and healing rates of an arthroscopy-assisted technique for fixation of unstable distal third clavicle fractures. METHODS: Seven patients presenting to a single surgeon with displaced distal clavicle fractures were included. A curved suspensory button and suture tape construct with a single coracoclavicular tunnel was used

Examining risk factors for posterior migration of fusion cages following transforaminal lumbar interbody fusion: a possible limitation of unilateral pedicle screw fixation.

OBJECT: Because the authors encountered 4 cases of hardware migration following transforaminal lumbar interbody fusion, a retrospective study was conducted to identify factors influencing the posterior migration of fusion cages. METHODS: Patients with lumbar degenerative disc disease (125 individuals; 144 disc levels) were treated using transforaminal lumbar interbody fusion and followed for 12-33 months. Medical records and pre- and postoperative radiographs were reviewed, and factors influencing the incidence of cage migration were analyzed. RESULTS: Postoperative cage migration was found

Complications of intramedullary Hagie pin fixation for acute midshaft clavicle fractures.

The purpose of this report was to evaluate patient outcomes after treatment of acute midshaft clavicle fractures with an intramedullary Hagie pin, including clinical results and the incidence of postoperative complications. Between 1993 and 2003, 16 patients who underwent intramedullary Hagie pin fixation of a midshaft clavicle fracture were identified. The medical records of each patient were reviewed to ascertain the mechanism of injury, indication for surgical intervention, and treatment course. Clinical outcomes were evaluated with respect to time to fracture union, postoperative shoulder

Republication of "First Metatarsophalangeal Joint Arthrodesis: Functional Outcomes Using 2 Different Fixation Techniques. Is More Expensive Better?".

BACKGROUND: First metatarsophalangeal (MTP) joint arthrodesis can be fixed using either a dorsal plate or crossed screws. However, there is considerable difference in the cost of these implants, and it is not known if there is sufficient difference in outcome that might justify this cost difference. Our aim was to compare the functional results and patient satisfaction rates after first MTP joint arthrodesis in a group of patients using the same surgical technique except for the fixation devices. METHODS: A prospective cohort of 27 patients who underwent first MTP joint fusion by the same sur

Intrabody Cage Augmentation in Kümmell Disease and Osteoporotic Burst Fractures: Technical Insights and Narrative Review of Current Evidence.

Intrabody cage augmentation has emerged as a minimally invasive technique for anterior column reconstruction in Kümmell disease and osteoporotic burst fractures. These osteoporotic conditions lead to progressive vertebral collapse, kyphosis, and instability. While cement augmentation provides rapid pain relief, it often fails to reliably restore sagittal balance or ensure biological integration in advanced stages of collapse. Although conventional anterior corpectomy with long-segment posterior fusion can achieve satisfactory deformity correction, these procedures are associated with substanti

[Reduction techniques for minimally invasive stabilization of proximal humeral fractures].

OBJECTIVE: The aim of surgical stabilization of proximal humerus fractures is to restore the anatomical relations between the greater and lesser tubercle, to reconstruct the joint and preserve the vascular supply of the fragments. INDICATIONS: Approximately 80% of proximal humeral fractures can be treated conservatively. Surgical treatment is indicated based on the fracture pattern, patient-related factors and the risk of avascular head necrosis. Two-part fractures with a metaphyseal comminution zone and 3/4-part fractures can benefit from near to anatomic reconstruction depending on the pati

Osseodensification for enhancement of spinal surgical hardware fixation.

Integration between implant and bone is an essential concept for osseous healing requiring hardware placement. A novel approach to hardware implantation, termed osseodensification, is described here as an effective alternative. 12 sheep averaging 65kg had fixation devices installed in their C2, C3, and C4 vertebral bodies; each device measured 4mm diameter×10mm length. The left-sided vertebral body devices were implanted using regular surgical drilling (R) while the right-sided devices were implanted using osseodensification drilling (OD). The C2 and C4 vertebra provided the t=0 in vivo time p

Mechanical closure devices for atrial fibrillation.

Atrial fibrillation (AF) is the most commonly sustained arrhythmia and is a major cause of stroke and systemic embolism. Chronic oral anticoagulation reduces this risk, but at the cost of increased bleeding. In addition, a substantial proportion of AF patients who are at moderate-to-high risk for stroke are undertreated due to real or perceived contraindications. The major source of thromboembolism in AF appears to be the left atrial appendage (LAA). Therefore, device closure of the LAA represents a mechanical approach to stroke prevention in AF patients. In this review, we describe the ration

The severely deviated septum--the way I solve the problem.

Surgical correction of severely deviated or "saddle" noses poses specific problems that are often difficult to resolve because of the complexity of the deformities, particularly those of the septum. Four steps are necessary: (1) the deformations must be skeletonized and visualized by a perfect extramucosal dissection and maxillary-premaxillary approaches; (2) the septum must be mobilized by resection of the osteocartilaginous chondrovomerine callus, after which it is possible to carry out the septum as long as the mucosa is not torn; (3) the angulations must be corrected carrying out discontin
