Extensor tendon injuries are common and require a complex treatment strategy to appropriately manage them, including initial repair, revision/reconstruction options, and postoperative protocols. Intrinsic and extrinsic components of the extensor mechanism contribute to a complex anatomic apparatus but also allow for numerous reconstructive options. Tenolysis, tendon grafting, and local tissue reconstruction are all options that can be used to treat complex extensor tendon injuries, but the type of repair is dependent on which of the eight extensor zones and accompanying structures are injured.…

Lacerations and ruptures of the flexor hallucis longus or extensor hallucis longus tendon are frequently managed with operative repair. Tendon injuries of the hallux are not all alike; careful consideration should be given to the mechanism and site of injury, the timing of presentation, and the presence of other injuries. Not all tendon injuries of the hallux require repair. The effectiveness of a repair will depend on the goals of surgery, which may include pain relief, active joint motion, or correction of deformity. When goals are clearly defined, a satisfactory result can be expected in mo…

Tendons connect muscle to bone and play an integral role in bone and joint alignment and loading. Tendons act as pulleys that provide anchorage of muscle forces for joint motion and stability, as well as for fracture reduction and realignment. Patients that experience complex fractures also have concomitant soft tissue injuries, such as tendon damage or rupture. Tendon injuries that occur at the time of bone fracture have long-term ramifications on musculoskeletal health, yet these injuries are often disregarded in clinical treatment and diagnosis for patients with bone fractures as well as in…

Unilateral quadriceps tendon ruptures are not uncommon. These injuries have been reported to occur spontaneously and after seemingly trivial trauma in elderly individuals, patients undergoing renal dialysis, and patients with metabolic derangements such as hyperparathyroidism. In young patients, unilateral quadriceps tendon ruptures have been reported as complications of burns, anabolic steroid abuse, and elective orthopedic surgery. Bilateral quadriceps tendon ruptures in young healthy patients are rare injuries. We present the case of a young, healthy, active duty soldier who sustained bilat

The optimal management of chronic Achilles tendon injuries continues to be debated. The tension-length relationship and intrinsic viscoelasticity of the native tendon that allows efficient propulsion during ambulation are difficult to restore once the tendon has been injured. Missed or misdiagnosed injuries or failure of surgical or nonsurgical management of acute Achilles tendon ruptures can lead to the tendon healing in an elongated position or lack of healing altogether. This condition results in persistent weakness, pain, and functional debilitation for the patient. Understanding the tendo…

BACKGROUND: Injuries to the Achilles tendon in men aged 20-40 years are most commonly caused by intensified physical activity leading to prolonged overload of the tendon. Most injuries affect the tendon's proximal third, which is less vascularised. Surgery for Achilles tendon injuries aims to obtain the best possible gait biomechanics and quickly restore the patient's functional performance. Clinical outcomes vary with the surgical technique used. This article presents the results of a study comparing the minimally invasive technique and the classical technique for primary repair of the Achill…

We reviewed 11 patients who had been treated between January 1986 and June 1994 for severe foot injuries by tendon transfer with microvascular free flaps. Their mean age was 5.6 years (3 to 8). Five had simultaneous tendon transfer and a microvascular free flap and six had separate operations. The mean interval between the tendon transfer and the microvascular free flap was 5.8 months (2 to 15) and the mean time between the initial injury and the tendon transfer was 9.6 months (2 to 21). The anterior tibial tendon was split in five of six cases. The posterior tibial tendon was used three times

PURPOSE: The purpose of this work was to demonstrate the MR findings of injuries to the distal gastrocnemius muscle.
METHOD: Twenty patients with clinically confirmed injuries to the distal gastrocnemius muscle underwent MRI. The injuries were evaluated with regard to abnormal morphology or signal abnormality, site, and degree of tearing.
RESULTS: Twenty-three injuries to the distal gastrocnemius occurred in 20 patients, with involvement of the myotendinous junction in 22 of 23 (96%) injuries. An interstitial tear of the proximal Achilles tendon was present in one instance. Myotendinous stra

Transposition of the extensor indicis seems to be the most common procedure for reconstruction of the ruptured or severed extensor pollicis longus (EPL) tendon. From 1959 to 1988, we performed 115 extensor indicis transpositions in 34 open EPL injuries and 81 subcutaneous EPL ruptures. The open injuries involved failed primary repair (n = 6) or untreated tendon injuries (n = 19). The subcutaneous ruptures occurred after distal radius fractures or other closed injuries of the wrist (n = 62), arthrosis or rheumatoid synovialitis (n = 8). Forty-seven patients returned for follow-up examination si…

INTRODUCTION: Flexor tendon injuries disrupt hand function and require precise surgical repair. Traditionally assessed through physical examination alone, ultrasound scanning (USS) is emerging as a valuable tool, offering real-time, non-invasive imaging to assess injury type and extent. This review evaluates the USS's accuracy in preoperative flexor tendon assessment.
METHODS: Following PRISMA guidelines, a systematic review was conducted using MEDLINE, EMBASE, and Web of Science. Studies on primary flexor tendon injuries assessed with preoperative USS were included, while non-clinical studie…

Eighteen patients were treated for finger pulling injuries between 1977 and 1986. There were two main types of injury, subcutaneous rupture of the flexor profundus tendon (n = 15) and fracture of the proximal phalanx of the finger (n = 3). Their mechanisms of injury were different. The prognosis in tendon ruptures is comparable to that in other open flexor tendon injuries. Age and distal localization were associated with a poorer prognosis in younger patients. Fractures healed well.

Ruptures of the extensor mechanism of the knee are serious injuries requiring prompt diagnosis and treatment. They can be divided into soft tissue and bony causes. Soft tissue tendon injuries can be either partial or complete. Rupture of the quadriceps tendon is an uncommon injury and is more frequent in patients over the age of 40 years. Patella tendon ruptures are even rarer and are more frequent in patients under the age of 40 years. Causes can be direct or indirect. Complete ruptures of the quadriceps tendon or patella tendon benefit from early surgical management, while partial ruptures m…

PURPOSE: The use of plates in the treatment of Leddy-Packer type 3-5 flexor digitorum profundus tendon avulsion injuries is becoming increasingly prevalent; however, potential complications, including disruption of the avulsed bone fragment and damage to the flexor digitorum profundus tendon, may be encountered. The objective of this study was to ascertain whether the buttress plate technique, without screw placement in the avulsed fragment, can be employed as a treatment option for patients with type 3, 4, and 5 flexor digitorum profundus tendon avulsion injuries.
METHODS: Type 3, 4, and 5 f

BACKGROUND: A virtual navigation system (VNS) can combine two imaging modalities for synchronous observation and can be beneficial for diagnosis and treatment. Ultrasound elastography (UE) can distinguish between soft and hard tissues. However, the application of UE to musculoskeletal structures is rare.
OBJECTIVE: To evaluate UE of the supraspinatus tendon using ultrasound-magnetic resonance imaging (US-MRI) virtual navigation.
METHODS: Sixty patients with an ache in the shoulder were diagnosed with supraspinatus tendon injuries using MRI. US-MRI virtual navigation was used to identify lesi…

Patellar tendon ruptures are the third most common injury to the extensor mechanism. These injuries are debilitating to the patient, and delays in treatment can present a challenge to the treating surgeon. Chronic patellar tendon injuries are rare and are more difficult to manage given proximal patellar retraction, scarring, and atrophy of surrounding tissues. There is no consensus on the optimal approach for treatment, and numerous reconstruction techniques have been described using a variety of graft choices and fixation methods. We describe our surgical technique for a 3-fold reconstruction…

The medial canthal tendon and the fragment of bone on which it inserts ("central" fragment) are the critical factors in the diagnosis and treatment of nasoethmoid orbital fractures. The status of the tendon, the tendon-bearing bone segment, and the fracture pattern define a clinically useful classification system. Three patterns of fracture are appreciated: type I--single-segment central fragment; type II--comminuted central fragment with fractures remaining external to the medial canthal tendon insertion; and type III--comminuted central fragment with fractures extending into bone bearing the
