Flexor tenodesis procedure in the treatment of lesser toe deformities.

In this technical report study, we describe the use of a flexor tenodesis procedure in the treatment of lesser toe deformities (LTD). Using a specific implant, both the flexor digitorum longus and brevis tendons are attached to the plantar aspect of the proximal phalanx, allowing dynamic correction of flexible deformities of metatarsophalangeal and interphalangeal joints. Good clinical results and absence of complications were observed in a series of 3 patients, with considerable correction of the LTD, and absence of substantial residual floating toe or metatarsophalangeal joint stiffness. Functional restoration of elbow flexion in brachial plexus injuries: results in 167 patients (excluding obstetric brachial plexus injury).

From 1985 to 1990, 167 patients were treated for impaired elbow flexion caused by brachial plexus injury. Surgical procedures were divided into two categories: nerve reconstruction (128 patients) and muscle or tendon transfers (39 patients). Surgery in the nerve reconstruction group included direct suturing, nerve grafting of portions of the brachial plexus responsible for elbow flexion, or nerve transfer (intercostal, phrenic, or spinal accessory nerve) to the musculocutaneous nerve. The second category included tendon or muscle transfer or a functioning free muscle transplantation for biceps…

Tendocutaneous free flap transfer from the dorsum of the foot.

Tendocutaneous free flap transfer has been used to treat troublesome wounds involving skin and tendons, with the dorsum of the foot being the most frequent donor site. Free flaps consist of the dorsalis pedis artery, saphenous vein, branch of the superficial peroneal nerve, long extensor tendons, and overlying skin. We analyzed 13 cases of skin and tendon loss from the dorsum of the hand and wrist that were treated with tendocutaneous free flap from the dorsum of the foot. The size of the flap was from 6 x 5 cm to 11.5 x 7.1 cm (average, 9.2 x 5.9 cm), and the transferred tendons ranged from 1

Surgical repair of a tibialis anterior tendon rupture : a case report.

A 61-year-old man who stretched in the morning presented a rupture of the tibialis anterior tendon treated by extensor hallucis longus transfer. Rupture of the tibialis anterior tendon is rare. Surgical treatment seems to be more efficient in improving the function.

Primary augmentation of percutaneous repair with flexor hallucis longus tendon for Achilles tendon ruptures reduces tendon elongation and may improve functional outcome.

PURPOSE: Achilles tendon ruptures (ATR) result in loss of strength and function of the gastrosoleus-Achilles tendon complex, probably because of gradual tendon elongation and calf muscle atrophy, even after surgical repair. Flexor hallucis longus (FHL) augmentation not only reinforces the repair and provides new blood supply to the tendon, but also protects the repair, internally splinting the repaired Achilles tendon, maintaining optimal tension. We prospectively compared the clinical outcomes of patients with acute ATR, managed with either percutaneous repair only or percutaneous repair and

Pollicisation of the index finger without interosseous muscle or extensor tendon repositioning in isolated thumb hypoplasia/aplasia.

As a part of the pollicisation procedure, previous authors describe interosseous muscle and/or extensor tendon repositioning. The benefit of these tendon transfers has never been investigated despite the fact that other authors have used different techniques or do not perform some of these transfers routinely. In this paper, 16 children (17 hands) with isolated aplasia/hypoplasia of the thumb underwent the pollicisation procedure without any intrinsic tendon transfers. Surgery was performed at a mean age of 22 months and the mean follow-up time was 5 years. No early operative complications wer…

What Do You Do With The Achilles if You Have No Fancy Toys?

Surgical management of Achilles disorders warrants excision of the degenerated tendon and removal of impinging bone. Resulting defects can be bridged by various methods. Although FHL is the most commonly used tendon for transfer, large defects in cases of chronic Achilles ruptures may be bridged by use of a distant donor tendon. Bony anchorage of a lengthened or transferred tendon into the calcaneus can be done with suture anchors or with interference screws. In developing countries, such implants may not be available or affordable, necessitating the adoption of innovative ways to anchor tendo

A modification of the palmaris longus-to-extensor pollicis longus transfer for radial nerve palsy.

PURPOSE: The standard palmaris longus (PL)-to-rerouted extensor pollicis longus (EPL) transfer was modified by taking the PL with an extension of the palmar aponeurosis (PA) and performing the transfer at the level of the thumb metacarpal. Our purpose was to evaluate whether this modified transfer could restore both the extension and the radial abduction of the thumb without rerouting the EPL. METHODS: We restored thumb function of 5 patients with unrecovered radial nerve palsy (4 men and 1 women; mean age at surgery, 47 years; mean duration between onset of palsy and surgery, 13 months; and

[Management of soft tissue defect after achilles tendon repair].

OBJECTIVE: To investigate the management of the soft tissue defect after the Achilles tendon repair. METHODS: From April 1996 to April 2006, 24 patients (17 males, 7 females; aged 16-59 years), who suffered from postoperative Achilles tendon exposure caused by local soft-tissue necrosis after the Achilles tendon repair, were treated and evaluated. Of the 24 patients, 8 had an original open injury (machine-crush injury in 2 patients, heavy-object press injury in 3, motor-cycle wheel crush injury in 3) and 16 patients had a closed injury (sports injury). In their treatment, the transfer of the

Functional Recovery Following an L-Lengthening Local Tendon Flap for Extensor Pollicis Longus Chronic Ruptures.

PURPOSE: Extensor indicis proprius tendon transfer has been considered the reference standard technique for extensor pollicis longus (EPL) chronic ruptures, but relevant complications have been reported. We describe a new reconstructive technique for chronic EPL ruptures and assess functional recovery after this procedure. METHODS: We observed 31 patients who fulfilled inclusion criteria an average of 6.8 months after treatment. An L-shaped local tendon flap was prepared by making a transverse incision to the middle of the tendon and then longitudinally toward the end of one of the tendon stu…

Late reconstruction of median nerve palsy.

The median nerve provides sensory innervation to the radial aspect of the hand, including the palm, thumb, index, long, and half of the ring fingers. It provides motor innervation to most of the volar forearm musculature and, importantly, to m ost of thenar musculature. The main goal of median nerve reconstructive procedures is to restore thumb opposition. There are a variety of transfers that can achieve this goal but tendon transfers must recreate thumb opposition, which involves 3 basics movements: thumb abduction, flexion, and pronation. Many tendon transfers exist and the choice of tendon…

Biomechanical Study Comparing Pulvertaft, Double Side-to-Side, and Locking Side-to-Side Tendon Suture Techniques.

PURPOSE: In this study, we present a single-sided locking side-to-side (STS) suture technique as a possible alternative for the Pulvertaft weave in tendon transfers. The aim of the study was to compare the biomechanical characteristics of the single-sided locking STS reconstruction with Pulvertaft and double-sided nonlocking STS reconstructions. METHODS: Twenty-four human cadaveric extensor digitorum communis tendons and 24 flexor digitorum superficialis tendons were randomly assigned to 1 of 3 groups, resulting in a total of 8 flexor tendons and 8 extensor tendons per group: Pulvertaft, doub…

Radial half of extensor carpi radialis longus tendon as graft to elongate muscle tendon unit for correction of finger clawing.

In 12 patients, the extensor carpi radialis longus muscle tendon unit was elongated using the radial half of the parent tendon so that it could reach the site of new insertion, the A1-A2 pulley of flexor sheath or lateral bands, after routing the transfer through the carpal tunnel. The tendon was of appropriate thickness and could be split into two halves to be used as a graft. Further splitting of the tendon into four tails was possible. The transferred slips retained adequate strength to activate the fingers after the operation. It is suggested that splitting of the extensor carpi radialis l

Dynamic stabilization of winging scapula by direct split pectoralis major transfer: a technical note.

A new technique of split pectoralis major tendon transfer (sternal head) for symptomatic scapular winging is shown. Whereas other authors use a lengthening with autogenous grafts, we prefer a direct attachment of the split pectoralis major tendon. With the use of an anterior and a posterior incision, the tendon of the sternal head of the pectoralis major is mobilized and transferred directly to the inferior angle of the scapula. An anatomic study shows that the pectoralis major muscle usually seems to be suitable for this procedure. Direct transosseous fixation of the transferred split pectora…

Long-term follow-up of patients undergoing tibialis posterior transfer: Is acquired pes planus a complication?

In this retrospective study, a series of 10 elective patients treated with transfer of the tibialis posterior (TP) tendon for pes cavus and … lgus deformity. The authors conclude that planovalgus deformity is not an inevitable sequelae of TP Tendon transfer and that established theory underestimates the role of static soft tissue restraints such as spring ligament in hindfoot stability.

Anatomic evaluation of the subcoracoid pectoralis major transfer in human cadavers.

Subcoracoid transfer of the pectoralis major has recently been described as a reconstruction for subscapularis insufficiency. The purpose of this study was to examine the surgically relevant anatomy of this transfer. The importance of understanding this anatomy was recently highlighted to us following our encounter with musculocutaneous neuropraxia in 2 patients after transfer of the entire pectoralis major, one deep to the musculocutaneous nerve. Dissections were performed on 20 fresh, whole human cadavers in which the entire pectoralis major muscle, medial and lateral pectoral nerves, and mu
