Achilles tendon injuries in athletes.

Two-thirds of Achilles tendon injuries in competitive athletes are paratenonitis and one-fifth are insertional complaints (bursitis and insertion tendinitis). The remaining afflictions consist of pain syndromes of the myotendineal junction and tendinopathies. The majority of Achilles tendon injuries from sport occur in males, mainly because of their higher rates of participation in sport, but also with tendinopathies a gender difference is probably indicated. Athletes in running sports have a high incidence of Achilles tendon overuse injuries. About 75% of total and the majority of partial ten

Tendon Disorders of the Foot and Ankle.

Attritional and traumatic injuries to the tendons around the foot and ankle are not uncommon. Treatment of overuse-type injuries (tendinitis) remains straightforward. However, surgical treatment of peroneal subluxation, Achilles tendon ruptures, and posterior tibial tendon insufficiency remains somewhat controversial. Generally speaking, soft-tissue reconstruction of the superior peroneal retinaculum is superior to bony procedures for peroneal dislocation. Open repair of a torn Achilles tendon is more predictable than closed treatment. Good clinical judgment is needed in determining the best t…

Acute flexor tendon injuries.

Current surgical techniques and controlled postoperative mobilization have significantly decreased the amount of intertendinous adhesions that plagued earlier attempts at repair. Nevertheless, the ability to recover normal function after flexor tendon injury remains a challenging and often elusive goal. In the civilian population, good to excellent results may be expected in 69% to 90% of patients. A thorough understanding of tendon anatomy and physiology, attention to atraumatic surgical technique, and a well-designed postoperative therapy regimen will ensure satisfactory results in the major

Muscle, tendon, and ligament evaluation.

Magnetic resonance imaging is the technique of choice for soft tissue evaluation. Muscle injuries occurring during physical activities are very frequent and are very well appreciated by MR imaging. Associated with 31P spectroscopy, MR imaging allows better understanding of physiopathologic phenomena and inflammatory lesions. An early and correct diagnosis is possible using MR imaging in cases of soft tissue pseudotumors, such as myositis ossificans. The quadriceps and Achilles tendons, which are superficial and thick, can be well evaluated using ultrasonography. Nevertheless, MR imaging is sup…

[Dislocation of the tibialis posterior tendon].

The authors report a case of traumatic dislocation of the tibialis posterior tendon on the medial malleolus. They describe the treatment applied and made a review of the literature about this rare pathology.

Tendon transfers for elbow flexion.

This article reviews the causes of loss of elbow flexion. It outlines the necessary preoperative considerations for elbow flexorplasty. The techniques, advantages, and disadvantages of flexorplasty, including the Steindler, pectoralis major, triceps, sternocleidomastoid, and latissimus dorsi, are discussed.

[Habitual peroneal tendon dislocation].

Between 1945 and 1985 a total of 54 patients were treated for recurrent dislocation of the peroneal tendon. 30 cases were operated on employing five various surgical techniques. 31 patients denied trauma as being the cause, which therefore made constitutional weakness of the retention apparatus of the peroneal tendon responsible. The pathogenesis is described in detail. The period of follow-up is 13.2 years (2-40). 80% of the operated patients became absolutely trouble-free. Only in one single case did a relapse occur.

Tenosynovitis: tendon and nerve entrapment.

The pathophysiology associated with tendon and nerve entrapment due to idiopathic tenosynovitis of the hand and wrist is discussed. The basis of clinical symptoms, nonoperative and operative treatment, complications, and prognosis relating to stenosing tenosynovitis and carpal tunnel syndrome are reviewed. A systematic approach to developing a philosophy for operative treatment is based upon severity of symptoms and findings at surgery. Deformities of the median nerve are classified into grades that correlate with symptoms and influence selection of the surgical procedure.

Delayed reconstruction of the calcaneal tendon.

Thirteen patients with ruptures of the calcaneal tendon diagnosed more than four weeks after injury were reviewed. Eleven patients had operative reconstruction with tendon shortening and the postoperative follow-up ranged from one to seven years. Isometric and isokinetic measurements, as well as the strength of the triceps surae, all compared favourably with the normal contralateral leg. Only one tendon re-ruptured. Eight of the eleven patients were satisfied with the results and the two patients who had refused reconstruction had worse functional results. Late reconstruction of a ruptured cal

Tendon injuries--extensor surface.

Although discussed much less than flexor tendon injuries, achievement of satisfactory function is difficult with these injuries. Under proper circumstances in clean wounds, and in a well equipped emergency department, they can be primarily repaired if the examining physician has the experience. If these conditions are not possible, the patient should be referred to a qualified hand surgeon. The author outlines treatment according to anatomic zones.

Tendon injuries--flexor surface.

Because of the difficulties in reconstructing chronic, unrecognized tendon injuries, early diagnosis and proper management are of the utmost importance. The initial examining physician is key to the treatment pathway the patient will take. A high index of suspicion for possible injuries to deep structures beneath the laceration followed by appropriate examination of each of the involved parts will usually result in an accurate diagnosis.

Tendon adaptation to bone shortening.

The ability of tendon to adapt its length to imposed conditions was tested in rat soleus. Shortening of one tibia left tendon insertions intact, but reduced the distance between them. Tendon lengths were found to decrease after a short period of recovery (1 or 2 months) whether surgery was performed in young or adult animals. Comparison of tibia and tendon length correlation in control rats of different ages and in experimental animals showed that adaptation was more complete in young rats than in adult rats. A long period of recovery seemed to improve adaptation only in young rats. Hydrotherm

Flexor tendon repair in the neonate.

Primary or delayed primary repair of flexor tendon lacerations in the digital sheath has become the accepted practice among surgeons expert in the treatment of hand injuries. The youngest patient reported in the literature we reviewed was 3 months old. We report a case of flexor tendon laceration at the time of delivery by emergency cesarean section. Delayed primary repair was carried out ten days later. Five months following surgery, functional results were judged to be good to excellent. The operating microscope is a valuable adjunct for tendon repair in the neonate. The procedure should be

If there is any doubt after observing the child using his hand, the wound must be explored and tendons and nerves identified to ensure their integrity. Even though the results of tendon repair in children are better than in adults, the structures are small, technique must be meticulous, and magnification must be used. Specially trained hand surgeons …
