Control is a key issue for patients with amyotrophic lateral sclerosis, and caring for them in their own homes is often an essential part of remaining in control. This goal can usually be achieved if all resources are correctly used.

OBJECTIVES: Palliative care phases (stable, unstable, deteriorating, terminal and bereavement) are useful in describing the palliative care situation of patients/relatives and their care needs as well as the suitability of care plans. Little is known about care setting-specific differences of the phases and their association with burden of symptoms/problems and functional status. We aimed to describe the presence and association of symptom/problem burden and functional status with the palliative care phase at the beginning of care episodes in specialist palliative care units, specialist home c

AIMS AND OBJECTIVES: To explore palliative care nurses' work experiences caring for patients at the end of life in private homes.
BACKGROUND: The home continues to be a desired place for care and dying; however, professional competence is needed and specialist palliative care nurses play a crucial role, often highly valued by patients and their family members. The private home as a physical work environment for nurses is explored, with both positive and negative aspects being described. To ensure future high-quality end-of-life care, there is a need to describe how palliative care nurses work

Crisis at the end of life are exceptional challenges for patients, relatives and therapists. With respect to the individual treatment goals and the patients' autonomy, therapeutic action should be commenced in an adequate manner in order to preserve the patients' quality of life as much as possible. Advance care planning for specific critical scenarios may be helpful in order to treat patients according to their wishes and values even if they are not capable to express themselves. Furthermore, a crisis plan can define the scope of action for therapists in emergency situations. Dyspnea, pain an

Rates of morbidity and mortality are significantly higher in homeless populations. Homeless people experience many barriers to receive adequate palliative care. This qualitative study examines how a major urban city's palliative care resources can be improved to increase access and better serve the homeless. Audiotaped interviews were preformed with 7 homeless care providers in Toronto, Canada, and their transcripts were analyzed using thematic analysis. The findings of the study suggest that in order to increase access and to serve the city's terminally ill homeless better, the following 4 ar

BACKGROUND: Significant knowledge gaps persist in understanding how to support patients receiving palliative and end-of-life care (PEoLC) who wish to die at home. This study aimed to identify factors associated with remaining at home and home death for patients receiving PEoLC.
METHODS: A sequential mixed-methods explanatory study (QUANT + QUAL) was conducted. Administrative data from a not-for-profit at-home palliative care organization in Québec Canada from 2015 to 2024 (n = 5931) and 73 semi-structured interviews with patients receiving PEoLC, caregivers, service providers and decision-mak

BACKGROUND: Palliative support has been shown to improve the quality of end-of-life care provided in long-term care facilities (LTCF). In this context, a practice improvement plan, which included creating a withholding and withdrawing life-sustaining treatment (WLST) protocol, has been developed and implemented in our LTCF. We aimed to compare professional practices before and after the implementation of this improvement plan.
METHODS: The study population included all individuals who died in 2018 and in 2021. A clinical audit noted if WLST was decided and which palliative interventions were

BackgroundMuslim patients have historically received aggressive end-of-life care, though palliative acceptance is growing. The factors that influence ICU (aggressive) vs ward (comfort-focused) deaths remain unclear, limiting value-concordant care.PurposeTo examine end-of-life care patterns and predictors of ICU vs ward death among Muslim patients, to guide culturally sensitive care.MethodsRetrospective cohort of adult Muslim decedents (2021-2024) at tertiary hospital in Muslim-majority country, stratified by death location: ward, ICU, and post-ICU ward. INTRODUCTION: Patients with cancer are at high risk of developing pressure ulcers at the end of life as a result of their underlying condition or cancer treatment. There are many guidelines which set out best practice with regard to end-of-life skin care. However, the complexity of palliative cancer care often means that it is challenging for nurses to make the appropriate person-centred decisions about end-of-life skin care. This study seeks to explore the perceived importance that nurses place on different factors in their end-of-life skin care for patients with cancer. The utility, face val

A minority of patients with end-stage disease are referred to palliative medicine for consultation in advanced heart failure. Educating stakeholders, including primary care, cardiology, and critical care of the benefits of hospice and palliative medicine for patients with poor prognosis, may increase appropriately timed referrals and improve quality of life for these patients. This article reviews multiple tools useful in prognostication in the setting of advanced heart failure.

We held a case study for an integrated consultation support system that enabled the senior citizen's continuous home recuperation. We had two purposes: One was to clarify a problem and then solve the problem for the senior citizens to continue a home recuperation. An effective support method was suggested. The other was to consider an ideal way to assign nursing staffs. Through the case study and its analysis, the effective support system should have these six points: (1) Construct a mutual trust with senior citizens,(2) Help them to improve a self-management ability of daily life,(3) Improve t

Blood transfusion's repercussions on quality of life are less well studied in cancer research, and rarer in palliative situation. It is necessary to look for studies dealing with anaemia to estimate its effects. In curative palliative situation, the situation is similar to that of curative stage patients. It is necessary landing quickly for anaemia to assure the patient's quality of life. Blood transfusion and more recently erythropoïesis-stimulating agents are effective treatments. In advanced palliative stage, transfusion improves symptoms (weakness and dyspnoea bound anaemia) and the patien

Prolonged grief disorder (PGD) is a recognized distinct mental health disorder in the DSM-5-TR and ICD-11, characterized by core symptoms such as longing for and preoccupation with the deceased that persist and worsen over time. Family members and caregivers of patients that receive end-of-life care are confronted with imminent bereavement, which emphasizes the importance of bereavement care as a preventive measure and the need for continued support after death beyond the hospital setting. This review discusses bereaved family members' and caregivers' risk factors of developing PGD, screening

The aging of society is also leading to a growing number of elderly prisoners in the prison system. The challenges associated with this relate not only to the treatment and medical care of prisoners in accordance with legal requirements with the prospect of release from prison. The prison system must also find an appropriate way of dealing with prisoners who are spending the end of their lives in a correctional facility. So far, the prison system has only begun to address this issue. This primarily concerns the material and personnel requirements for a prison system that is suitable for the el

The setting in which patients face the end of life (EOL) is shaped by complex, individualized needs beyond physical care. Mr. X, a man in his mid-50s with glioblastoma, endured treatment complications and progressive decline. Supportive care was engaged to manage symptoms and support his wife, Mrs. X, who was simultaneously the family's financial provider and caregiver for both her husband and their autistic adult son. As his condition worsened, she resisted hospice and code status changes, citing emotional, logistical, and financial barriers, while fiercely advocating for his wish to die in t

BACKGROUND: Older adults with dementia including Alzheimer's disease may have difficulty communicating their treatment preferences and thus may receive intensive end-of-life (EOL) care that confers limited benefits.
OBJECTIVE: This study compared the use of life-sustaining interventions during the last 90 days of life among Medicare beneficiaries with and without dementia.
METHODS: This cohort study utilized population-based national survey data from the 2000-2016 Health and Retirement Study linked with Medicare and Medicaid claims. Our sample included Medicare fee-for-service beneficiaries
