[Sleep apnea syndrome].

Apneas during sleep are physiologic events. If they occur with some frequency and cause health problems, the disturbance is called sleep apnea syndrome. Most commonly the apneas are mainly obstructive and caused by intermittent upper airway collapse during sleep. The resultant pathophysiologic phenomena and their clinical aspects are the consequences of this. The diagnostic aspects and differential diagnosis of related disorders are discussed, and the various types of treatment are reviewed.

[Finger-free speech following total laryngectomy. Instrumentation and technic of surgical voice rehabilitation].

Surgical voice restoration during and after total laryngectomy using Blom-Singer puncture and the glottoplasty techniques provide good results. There is no doubt that the proportion of patients with a good voice is higher after surgical voice restoration than with the use of the esophageal speech or electronic speaking aids. Nevertheless there are problems due to the shunt or the necessary daily changing of the prosthesis with its risks, and failures due to the tracheostomy, the prosthesis and the pharynx. In order to reduce these surgical problems we developed new instruments, a personal surg

Acute epiglottitis in childhood, Christchurch 1970-84.

The case records of 31 children presenting to Christchurch Hospital with acute epiglottitis over the last 15 years have been reviewed. There has been a significant increase in incidence over this period (p less than 0.05). Despite generally classical presenting features the disease was recognised in only 26% of children when seen by general practitioners prior to admission. The increasing problem of ampicillin resistance to Haemophilus influenzae type b locally has resulted in a change in first line antibiotic therapy in recent years. Changes have also occurred in methods of airway management

[Surgical treatment of giant thymoma in a patient with myasthenia gravis].

We report a case report of giant thymoma (17 cm×14 cm×4 cm) in a 32-year-old male patient with myasthenia gravis. Examinations revealed that the tumor invaded the mediastinum, blood vessels and pericardium, and was histologically classified as WHO type B1 and Masaoka stage III. The patient developed myasthenia gravis crisis after radical resection of the thymoma and appropriate treatments with tracheotomy were administered. The patient recovered and was discharged after a hospital stay for 252 days with a mechanical ventilation time of 102 days.

A case of laryngeal paraganglioma and literature review.

Paragangliomas are common in carotid body, jugular tympanic cavity and cervical vagus nerve, but uncommon in larynx. We report clinical and pathological features of a 40-year-old woman with laryngeal paraganglioma. Tracheotomy combined with vein anesthesia was performed to isolate tumors. The patient was followed up for 3 months after surgery treatment and recovered well. We also provide a better understanding of this disease through a review of relevant literature. Largnx paraganglioma is rare, but should require long-term follow-up for possible recurrence.

Utility of a partial autopsy: a case report of congenital laryngeal atresia.

Laryngeal atresia is a rare congenital anomaly that is fatal if not quickly diagnosed at birth. Here we report a case of unexpected congenital laryngeal atresia and esophageal atresia in a preterm male infant, in whom prenatal ultrasonic findings were limited to polyhydramnios and duodenal atresia. Attempts at intubation and emergent tracheotomy were unsuccessful. A diagnosis of congenital laryngeal atresia with esophageal atresia was made during a restricted autopsy (limited to the neck). In an era of declining autopsy rates nationwide, this case emphasizes the value of limited autopsy examin…

Nasogastric tube syndrome: a case report.

Nasogastric tube syndrome (NTS) is an uncommon complication of an indwelling nasogastric tube. Ulceration and infection in the posterior cricoid region causes dysfunction in the abduction of the vocal cords and may seriously compromise patients' airway. This pathology should be considered in patients with prolonged nasogastric intubation who start with moderate pharyngeal pain. We report a case of a 70 year-old woman, admitted to the Neurology department of our centre due to an ischaemic cerebral accident who presented acute dyspnoea requiring emergency tracheotomy following prolonged nasogast…

Unusual outcome of Ludwig's angina.

This is a report of 2 cases of Ludwig's angina. An Indonesian young female patient developed severe stridor after oral examination. Then she underwent tracheostomy and developed post decannulation dyspnea due to huge surgical emphysema. The second case regards an Indian young male who developed disseminated intravascular coagulation and died from hemorrhage. The objectives of our cases presentation are to avoid mouth examination of Ludwig's angina if we are not ready for performing tracheotomy and to be aware of the possible development of disseminated intravascular coagulation and post decann…

Giant laryngoceles: a cause of upper airway obstruction.

Laryngoceles and saccular cysts, which are abnormal dilatations of the laryngeal saccule, are uncommon. The etiology is unknown but is probably related to both congenital and acquired factors. These structures are usually asymptomatic and are incidentally discovered through radiographic studies for unrelated symptoms. We describe two patients with upper airway obstruction, one caused by a giant laryngocele and the other by a large saccular cyst. In the former patient, acute tracheotomy had to be performed. The laryngocele and saccular cyst were removed surgically, which relieved patients' symp…
