Disparities and outcomes of patients living with Down Syndrome undergoing healthcare transitions from pediatric to adult care: A scoping review.

Down Syndrome (DS) is one of the most common chromosomal disorders worldwide, and people with DS experience more co-morbidities and have poorer health outcomes compared to the general population. An area that is not well understood is how patients with DS transition from pediatric to adult care, as well as the details, barriers, and difficulties of these transitions for patients. Hence, we aimed to provide a scoping review of the literature in PubMed, Scopus, and CINAHL on the topic of healthcare transitions (HCTs) for patients with DS. Findings suggest patients with DS who continued receiving

How Transitions in Dementia Care Trajectories Affect Health Problems in Partners: A Longitudinal Analysis With Linked Health and Administrative Data.

OBJECTIVES: To evaluate whether the prevalence of health problems in cohabiting partners of people with dementia differs between the year before and the year after 3 transitions: the diagnosis of dementia, institutionalization, and the death of the person with dementia. METHODS: Individuals with dementia and their partners were identified on the basis of data in the electronic health records (EHRs) of 451 Dutch general practices. EHRs were also the data source on their health problems, which were linked to the Dutch population registry and health administration data with demographic character

An Integrated Health-System Specialty Pharmacy Model for Coordinating Transitions of Care: Specialty Medication Challenges and Specialty Pharmacist Opportunities.

Adherence and persistence to specialty medications are necessary to achieve successful outcomes of costly therapies. The increasing use of specialty medications has exposed several unique barriers to certain specialty treatments' continuation. Integrated specialty pharmacy teams facilitate transitions in sites of care, between different provider types, among prescribed specialty medications, and during financial coverage changes. We review obstacles encountered within these types of transitions and the role of the specialty pharmacist in overcoming these obstacles. Case examples for each type

Role of Pulse Oximetry Screening for Term Healthy Newborns During Transitional Period to Detect Critical Congenital Heart Disease (CCHD): A Prospective Observational Study in a Tertiary Care Hospital.

BACKGROUND: Early detection of critical congenital heart disease (CCHD) remains a challenge, especially in settings with early postnatal discharge. Pulse oximetry screening is a simple, non-invasive method that can aid in early diagnosis, but optimal timing in resource-limited settings requires evaluation. OBJECTIVE: This study aims to assess the effectiveness of pulse oximetry screening at 15 minutes and six hours of life in identifying persistent hypoxemia and detecting CCHD or other early echocardiographic cardiac findings among term healthy newborns. METHODS: This prospective observation

Postsurgical opioid prescribing among veterans using community care for orthopedic surgery at non-VA hospitals compared to a VA hospital with a transitional pain service: a retrospective cohort study.

BACKGROUND: The USA provides medical services to its military veterans through Veterans Health Administration (VHA) medical centers. Passage of recent legislation has increased the number of veterans having VHA-paid orthopedic surgery at non-VHA facilities. METHODS: We conducted a retrospective cohort study among veterans who underwent orthopedic joint surgery paid for by the VHA either at the Salt Lake City VHA Medical Center (VAMC) or at non-VHA hospitals between January 2018 and December 2021. 562 patients were included in the study, of which 323 used a non-VHA hospital and 239 patients th

College mental health and the digital care transition: national trends in distress, help-seeking, and technology-mediated access, 2020-2025.

BACKGROUND: College mental health burden increased during the COVID-19 era while university mental health services rapidly adopted remote and digital modes of care. The population-level co-occurrence of these changes has rarely been described across the pandemic and post-pandemic period. METHODS: We performed a secondary ecological trend analysis of seven publicly available, weighted Healthy Minds Study national reports based on successive independent cross-sectional samples from Winter/Spring 2020 through 2024-2025. The reports contained a nominal 553,785 response records across waves; they

Overcoming Barriers to Health Care during Reentry with the North Carolina Formerly Incarcerated Transition (NC FIT) Program: Patient Perspectives.

Transitions Clinic Network (TCN) programs facilitate continuity of care for people with chronic conditions recently released from incarceration. Despite widespread implementation of TCN programs and their association with positive reentry outcomes, studies have yet to examine clients' perspectives on the programs' most helpful traits for overcoming reentry barriers to health care. Therefore, we interviewed 19 clients from the TCN-affiliated North Carolina Formerly Incarcerated Transition (NC FIT) Program. Respondents' identified reentry barriers included financial challenges, difficulties with

Lean on me: Social factors and diabetes outcomes among young adults with type 1 diabetes prior to transitioning to adult care.

AIMS: Little is known about how different aspects of social relationships relate to diabetes self-management and glycaemic outcomes in young adults with type 1 diabetes (T1D). We examined relationships of general and diabetes-specific social variables with diabetes self-management and HbA1c. We hypothesized diabetes-specific factors would more strongly associate with self-management and HbA1c than general social factors. METHODS: We analysed baseline data from 100 young adults with T1D (Mage = 19.9 ± 1.3 years) enrolled in a randomized controlled trial of a paediatric to adult care transition

Do early intervention services for psychosis maintain their effects after transition to usual/modular care? A systematic review and meta-analysis.

Early intervention services (EIS) for psychosis have demonstrated superiority to usual care/modular care (UC/MC) until the end of their delivery. However, the maintenance effects of EIS care after transition to UC/MC are less clear. We aimed to compare these effects vs. UC/MC at least one year after the end of EIS care. This PRISMA and MOOSE-compliant systematic review searched PubMed, EMBASE, PsycINFO and Web of Science databases and Cochrane Central Register of Reviews, without time or language restrictions. We included studies initially designed as randomized controlled trials (RCTs) compar

A call for personalized transition of care in congenital surgical anomalies: A population-based cohort study on healthcare utilization from birth to adulthood.

BACKGROUND: Many children with congenital surgical anomalies (CSA) face complex, lifelong healthcare challenges. We aimed to evaluate long-term healthcare utilization patterns to guide effective transition-of-care strategies. METHODS: Our retrospective population study compared CSA patients (n = 768) to matched controls (n = 7635). Cox and Poisson regression models assessed time-to-event and frequency of ambulatory visits and hospitalizations, adjusting for sex, socioeconomic status (SEFI), family size and rural/urban residency. RESULTS: CSA survivors had earlier (HR = 1.22, 95%CI: 1.12-1.32

A Transition of Care Service to Reduce Hospital Readmissions for High-Risk Cardiology Patients (RECARD): Protocol for a Pre-Post Interventional Trial.

BACKGROUND: Transition of care (TOC) is a high-risk period for medication errors as patients move between multiple health care professionals, settings, and changing medication regimens. Ineffective communication systems further increase the likelihood of medication harm and hospital readmission. A TOC service with tailored, patient-specific support may help reduce these risks. Cardiovascular medicines significantly contribute to medication harm, and patients recovering from acute myocardial infarction (AMI) or cardiothoracic surgery (CS) are vulnerable due to complex regimens and frequent medi
