Hydrogen sulfide in placental development and pregnancy disorders: mechanisms, therapeutic potential, and translational challenges.

The placenta plays a crucial role in maintaining pregnancy stability, regulating fetal growth, and facilitating maternal-fetal exchange. Its proper development relies on placental vasculogenesis and angiogenesis, along with adaptive remodeling of the maternal uterine vasculature. However, the mechanisms underlying placental development and the successful establishment of the maternal-fetal interface remain incompletely understood. As the third gaseous signaling molecule identified after nitric oxide (NO) and carbon monoxide (CO), hydrogen sulfide (H₂S) has been demonstrated to regulate vascula

Transforming corneal alkali burn treatment: unveiling mechanisms and pioneering therapies from bench to bedside.

Corneal alkali burns are severe ocular injuries characterized by extensive tissue damage, inflammation, oxidative stress, and neovascularization, which often lead to long-term visual impairment and corneal fibrosis. This review comprehensively examines the mechanisms underlying alkali burn injuries, including the roles of inflammatory mediators, oxidative stress, and cellular responses, while highlighting current and emerging therapeutic approaches. Traditional treatments, such as corticosteroids and surgical interventions, often have limited efficacy and significant side effects. Recent advan

Microplastics as an emerging driver of osteoarthritis: a translational synthesis of environmental exposure, patho-mechanisms, and public health implications.

BACKGROUND: Osteoarthritis (OA), a leading global cause of disability, has been increasingly associated with environmental microplastic (MP) exposure. MPs bioaccumulate in human tissues via the food chain and may disrupt joint homeostasis through inflammatory and oxidative pathways. However, their role in OA pathogenesis remains underexplored in translational contexts. METHODS: We conducted a synthesis of environmental microplastic contamination data from global aquatic ecosystems, integrated with molecular pathway analyses and epidemiological evidence. Microplastic sampling and polymer ident

Fibrous dysplasia/McCune-Albright syndrome: state-of-the-art advances, pathogenesis, and basic/translational research.

Fibrous dysplasia/McCune Albright syndrome (FD/MAS) is a rare genetic disease caused by postzygotic activating variants in the GNAS gene, encoding the α subunit of stimulatory G protein (Gαs). Although multiple organs may be involved, skeletal lesions usually represent the most severe and least treatable expression of the disease, leading to bone deformities, spontaneous fractures, and chronic pain that severely reduce patients' quality of life. The recognition of the causative Gαs variants and the consequent ligand-independent activation of the adenylyl cyclase/cAMP/PKA pathway has provided a

Translating Biomarker Discovery: From Bench to Bedside in Dry Eye Disease.

Dry Eye Disease (DED) is a complex, multifaceted ocular disease characterized by tear film instability and inflammation. It can sometimes be elusive to identify the type of DED in patients, given the overlapping symptoms with other conditions like allergies and the multitude of stimuli that might trigger DED onset. There is also difficulty due to limitations on the diagnostic testing available to clinicians, as poor reliability and a lack of standardization plague accurate diagnoses. Identified biomarkers can help identify DED pathophysiology and category, and these include molecular biomarker

Mesenchymal stromal cell-derived extracellular vesicles therapy openings new translational challenges in immunomodulating acute liver inflammation.

Inflammation plays a critical role in conditions such as acute liver failure, acute-on-chronic liver failure, and ischemia-reperfusion-induced liver injury. Various pathogenic pathways contribute to liver inflammation, involving inflammatory polarization of macrophages and Küpffer cells, neutrophil infiltration, dysregulation of T cell subsets, oxidative stress, and activation of hepatic stellate cells. While mesenchymal stromal cells (MSCs) have demonstrated beneficial properties, their clinical translation is limited by their cellular nature. However, MSC-derived extracellular vesicles (MSC-

The Potential of CAR-T cells for Treating Heart Diseases: Current Status and Hurdles in Clinical Translation.

Traditional treatments for heart disease, including pharmacotherapy and surgical interventions, are effective in managing symptoms and preventing complications but often fail to fully restore cardiac function or halt the progression of the disease. Additionally, these approaches are frequently associated with significant adverse effects. Inspired by the success of CAR-T cell therapy in oncology, this review examines the potential of CAR-T cell technology for treating heart diseases, detailing how CAR-T cells, engineered by merging antibody-derived targeting domains with T-cell signaling domain

Magnetoencephalography in Psychiatry: A Perspective on Translational Research and Applications.

Magnetoencephalography (MEG) is a neuroimaging technique that has excellent temporal as well as good spatial resolution for measuring neural activity and has been extensively employed in cognitive neuroscience. However, MEG has only been more recently applied to investigations of brain networks and biomarkers in psychiatry. Besides providing new insights into the pathophysiology of major psychiatry syndromes, especially in schizophrenia, a major objective of current research is the identification of biomarkers that could inform early intervention and novel treatments. This chapter will provide

A novel translational model of atherosclerosis, the ex vivo pump-perfused amputated human limb model.

The preclinical study of atherosclerosis has traditionally centred around the use of small animal models, translating to large animal models, prior to first-in-man studies. We propose to disrupt this paradigm by designing an ex vivo pump perfused human limb model. The novel model consists of taking a freshly amputated limb and incorporating it into an ex situ pump-perfused bypass system (akin to extracorporeal membrane oxygenation), circulating warmed, oxygenated blood. The circuit incorporates an introducer sheath and guiding catheter for intravascular imaging and X-ray angiography. Regular m

Drug delivery strategies for local immunomodulation in transplantation: Bridging the translational gap.

Drug delivery strategies for local immunomodulation hold tremendous promise compared to current clinical gold-standard systemic immunosuppression as they could improve the benefit to risk ratio of life-saving or life-enhancing transplants. Such strategies have facilitated prolonged graft survival in animal models at lower drug doses while minimizing off-target effects. Despite the promising outcomes in preclinical animal studies, progression of these strategies to clinical trials has faced challenges. A comprehensive understanding of the translational barriers is a critical first step towards

A novel translational bioinformatics framework for facilitating multimodal data analyses in preclinical models of neurological injury.

Pediatric neurological injury and disease is a critical public health issue due to increasing rates of survival from primary injuries (e.g., cardiac arrest, traumatic brain injury) and a lack of monitoring technologies and therapeutics for treatment of secondary neurological injury. Translational, preclinical research facilitates the development of solutions to address this growing issue but is hindered by a lack of available data frameworks and standards for the management, processing, and analysis of multimodal datasets. Here, we present a generalizable data framework that was implemented fo

A framework for translating tauopathy therapeutics: Drug discovery to clinical trials.

The tauopathies are defined by pathological tau protein aggregates within a spectrum of clinically heterogeneous neurodegenerative diseases. The primary tauopathies meet the definition of rare diseases in the United States. There is no approved treatment for primary tauopathies. In this context, designing the most efficient development programs to translate promising targets and treatments from preclinical studies to early-phase clinical trials is vital. In September 2022, the Rainwater Charitable Foundation convened an international expert workshop focused on the translation of tauopathy ther

Comprehensive quantitative sensory testing shows altered sensory function in women with chronic pelvic pain: results from the Translational Research in Pelvic Pain (TRiPP) Study.

Chronic pelvic pain (CPP), despite its high prevalence, is still relatively poorly understood mechanistically. This study, as part of the Translational Research in Pelvic Pain (TRiPP) project, has used a full quantitative sensory testing (QST) paradigm to profile n = 85 women with and without CPP (endometriosis or bladder pain specifically). We used the foot as a control site and abdomen as the test site. Across 5 diagnostically determined subgroups, we found features which are common across different aetiologies, eg, gain of function in pressure pain threshold (PPT) when assessing responses f

Does the pursuit of scientific excellence serve or hamper translational medical research: an historical perspective from hematological malignancies.

Despite enormous global investment, translational medical research faces considerable challenges and patients, and their doctors are frequently frustrated by the apparent lack of research activity or progress. Understanding the factors that prevent innovative research discoveries from making it to clinical trials is a multifaceted problem. However, one question that must be addressed is whether the nature of current research activity and the factors that influence the conduct of pre-clinical research, permit, or hamper the timely progression of laboratory-based observations to proof of concept

Translational research around five categories of CI.

Five categories of cochlear implants are introduced: The 'classic CI', the 'combined CI' - which can be combining a CI based on electric stimulation with acoustic stimulation (EAS) or with mechanical stimulation (EMS) or with electrical stimulation of the vestibular system (VICI) -, the 'individualised CI', the 'augmented CI' and the 'totally implantable CI'. The translational research activities leading to and within these categories have been, are and will be numerous and are the subject of the compendium for which this paper is the concluding chapter. Early translational research has result

Non-Coding RNA in Systemic Sclerosis: A Valuable Tool for Translational and Personalized Medicine.

Epigenetic factors are heritable and ultimately play a role in modulating gene expression and, thus, in regulating cell functions. Non-coding RNAs have growing recognition as novel biomarkers and crucial regulators of pathological conditions in humans. Their characteristic feature is being transcribed in a tissue-specific pattern. Now, there is emerging evidence that lncRNAs have been identified to be involved in the differentiation of human skin, wound healing, fibrosis, inflammation, and immunological response. Systemic sclerosis (SSc) is a heterogeneous autoimmune disease characterized by f

Rabbit models of dry eye disease: Current understanding and unmet needs for translational research.

Dry eye disease (DED) is emerging as an eye health pandemic, affecting millions worldwide. The development of novel drugs, drug delivery systems, and targeted therapies for addressing the inflammation in DED necessitates progress in experimental models of DED. Animal models of DED have been created for simulating the two clinically described forms of DED: lacrimal insufficiency and the evaporative DED models. Although most DED models have relied upon rodents, the larger eye size and longer life span of rabbits and the closer resemblance to human lacrimal glands, render rabbits a promising near

Improving translatability of preclinical studies for neuromuscular disorders: lessons from the TREAT-NMD Advisory Committee for Therapeutics (TACT).

Clinical trials for rare neuromuscular diseases imply, among other investments, a high emotional burden for the whole disease community. Translation of data from preclinical studies to justify any clinical trial must be carefully pondered in order to minimize the risk of clinical trial withdrawal or failure. A rigorous distinction between proof-of-concept and preclinical efficacy studies using animal models is key to support the rationale of a clinical trial involving patients. This Review evaluates the experience accumulated by the TREAT-NMD Advisory Committee for Therapeutics, which provides
