Isoniazid-induced severe hepatotoxicity: an infrequent but preventable cause of liver failure in children treated for latent tuberculosis infection.

Isoniazid (INH) monotherapy has gained widespread acceptance as an efficacious therapy for latent tuberculosis infection (LTBI) especially in low-prevalence settings. Although INH related hepatotoxicity is well recognized, progression to severe liver dysfunction requiring care at a transplant center remains unpredictable. We report the management of a five year-old girl who developed progressive liver failure due to INH prophylaxis. This highlights the potential severity of INH related hepatic injury and underscores the significance of vigilant clinical monitoring throughout the duration of th

Causes of minimal hepatic periportal fibrosis present in Egypt.

This study aimed to elucidate the common causes of minimal periportal fibrosis in Egypt. Out of 50 patients having minimal periportal fibrosis, 22 (44%) had schistosomiasis as a sole aetiology, 33 (66%) had combined schistosomiasis with both HBV and HCV. Rare causes were tuberculosis in 4% of the patients and HIV in 2%. No cause could be elicited in 14% of the patients. It is concluded that although schistosomiasis is the commonest cause of minimal hepatic fibrosis in Egypt, yet it is not the sole aetiological factor.

HIV, prisoners, and human rights.

Worldwide, a disproportionate burden of HIV, tuberculosis, and hepatitis is present among current and former prisoners. This problem results from laws, policies, and policing practices that unjustly and discriminatorily detain individuals and fail to ensure continuity of prevention, care, and treatment upon detention, throughout imprisonment, and upon release. These government actions, and the failure to ensure humane prison conditions, constitute violations of human rights to be free of discrimination and cruel and inhuman treatment, to due process of law, and to health. Although intervention

Recombinant Mycobacterium smegmatis vaccine candidates.

Mycobacterium smegmatis is a species of rapidly growing saprophytes with a number of properties that make it an effective vaccine vector. Recombinant M. smegmatis expressing protective antigens of different pathogens and molecules modulating the immune responses offers some potential for reduction of the burden of tuberculosis, HIV and hepatitis B infections. This paper discusses the molecular methods used to generate recombinant M. smegmatis and the results obtained with some of these recombinants.

Reprimun--an antibiotic with large spectrum and immunomodulatory properties.

Reprimun contains an oxyminomethyl rifamycin-SV derivative as the active substance and has a large spectrum antibiotic activity, on Mycobacterium tuberculosis too. Reprimun also shows an inhibitory activity on viral and human lymphocyte reverse transcriptases, an antiproliferative effect on retrovirus, as well as a selective immunomodulator action at the level of TCD4+ lymphocyte. In animal, the toxicity tests demonstrated a good tolerance. In human subject, the drug is quite well tolerated: no severe adverse reactions were noted during and after its administration for 10-12 months. In man, Re

Prevalence of viral hepatitis infection in India: A systematic review and meta-analysis.

BACKGROUND: Nowadays, Viral Hepatitis can be comparable to the big three communicable diseases: tuberculosis, HIV/AIDS, and malarial infections. The main purpose of this study was to summarize the prevalence of viral Hepatitis in India from peer-reviewed articles published from February 2000 to February 2021. MATERIALS AND METHODS: We conducted a systematic search on Science Direct, Scopus, Medline, PubMed, Web of Science, Google Scholar, and other open access journals. We evaluated all relevant papers that looked into the prevalence of viral Hepatitis systematically. Finally, 28 studies on v

Changes in epidemiological features of vaccine preventable infectious diseases among three eras of national vaccination strategies from 1953 to 2018 in Shanghai, China.

BACKGROUND: Recurring outbreaks of infectious diseases highlight the importance of population vaccination strategies. We aimed to assess the impact of national vaccination strategies on vaccine-preventable infectious diseases (VPDs) in Shanghai, China and to identify vulnerable groups that may benefit from future vaccination policies. METHODS: Infectious disease data from 1953 to 2018 was obtained from Xuhui District Center for Disease Control and Prevention, Shanghai China. We used joinpoint regression to show incidence, mortality and fatality trends and to determine annual percent change in

Clozapine and tuberculosis treatment: a case report and literature review.

INTRODUCTION: To date, clozapine is the only antipsychotic approved by the United States Food and Drug Administration (FDA) for the management of treatment-resistant schizophrenia. People with serious mental illness are at higher risk of developing tuberculosis and have worse tuberculosis recovery outcomes compared to the general population. First-line regimens for acute tuberculosis often include rifamycins and isoniazid, both of which impact clozapine metabolism and levels through induction or inhibition of the hepatic cytochrome P450 (CYP450) enzyme system. There is limited evidence, mostly

Idiopathic hepatic granuloma: a case report.

BACKGROUND: Granulomatosis of the liver may present a diagnostic enigma. Granulomas are organized aggregates of macrophages that form as a chronic inflammatory response to various stimuli. CASE PRESENTATION: We report a case of a 69-year-old white male patient presenting with fever and lower extremity pain but no other symptoms of infection. Despite extensive diagnostic evaluations, no infectious source was identified. Initial empirical antibiotic therapy failed, prompting treatment with methylprednisolone and methotrexate, which improved clinical and laboratory parameters. Subsequent imaging

The first report of two cases of fatal liver injury due to anti-tuberculosis drugs in the presence of alpha-1 antitrypsin deficiency.

Tuberculosis (TB) is a major global health problem. Awareness of liver injury due to anti-TB therapy is vital because fulminant hepatic failure is a devastating and often fatal condition without liver transplantation. Here, we report for the first time, two patients of fatal liver injury due to anti-TB drugs in the presence of alpha-1 antitrypsin deficiency. Based on the triad of rapid loss in hepatocyte function, the onset of hepatic encephalopathy, and absence of a prior history of liver disease, the diagnosis of acute liver failure was established. Both patients had low levels of serum alph

A Taxonomy of medical comorbidity for veterans who are homeless.

Homeless veterans have numerous medical and behavioral health problems. Grouping homeless people based on comorbidity patterns may assist in determining severity of illness and triaging health care more effectively. We sought to determine if a finite number of profiles could be identified related to demographic characteristics, living situation, length of homelessness, and referral areas using interview data from 2,733 veterans who were presently or recently homeless. We considered 12 disorders: eye problems, hypertension, cardiovascular problems, COPD/emphysema, tuberculosis, gastrointestinal

Inhibitory Effects of Selected Antituberculosis Drugs on Common Human Hepatic Cytochrome P450 and UDP-glucuronosyltransferase Enzymes.

The comorbidities of tuberculosis and diseases such as HIV require long-term treatment with multiple medications. Despite substantial in vitro and in vivo information on effects of rifampicin and isoniazid on human CYPs, there is limited published data regarding the inhibitory effects of other anti-TB drugs on human CYPs and UGTs. The inhibitory effects of five first-line anti-TB drugs (isoniazid, rifampicin, pyrazinamide, ethambutol, and rifabutin), and the newly approved bedaquiline, were evaluated for six common human hepatic UGT enzymes (UGT1A1, 1A4, 1A6, 1A9, 2B7 and 2B15) in vitro using

Pulmonary infiltrates as part of drug reaction with eosinophilia and systemic symptoms syndrome in a patient with pulmonary tuberculosis: a clinical conundrum.

Drug reaction with eosinophilia and systemic symptoms (DRESS) syndrome is a rare but potentially fatal hypersensitivity reaction characterised by rash, fever, eosinophilia and multiorgan involvement. Although antiepileptics are common triggers, antitubercular therapy (ATT) is increasingly recognised as a cause in tuberculosis-endemic regions. Pulmonary involvement is uncommon and can mimic disease progression or superadded infection, creating diagnostic confusion. We report a diabetic woman in her late 40s with pulmonary tuberculosis who developed a diffuse erythematous rash, fever, eosinophil

Tuberculosis Beyond the Lungs: A Pictorial Review of Key Diagnostic Imaging Insights.

Tuberculosis (TB) remains a significant global health challenge, traditionally associated with pulmonary manifestations. However, extrapulmonary tuberculosis (EPTB) accounts for a substantial portion of TB cases, particularly in immunocompromised patients. EPTB can affect virtually any organ system and often mimics other infectious, inflammatory, or neoplastic conditions, making diagnosis particularly challenging. This pictorial review aims to illustrate the broad spectrum of imaging findings in EPTB using selected, confirmed cases involving hepatic, splenic, adrenal, pancreatic, genitourinary

[Pancytopenia in disseminated tuberculosis: Think of macrophage activation syndrome].

INTRODUCTION: The macrophage activation syndrome (MAS), also known as haemophagacytosis, is a non-specific clinical syndrome, which includes, among other things general debility, fever and hepatospleneomegaly. The most frequent pathological abnormalities are pancytopenia, hypertryglyceraemia, and hyperferrititinaemia. These clinical and biological criteria, which determine the severity of MAS, must be associated with a histological criterion - haemophagocytosis (with histiocytosis), which is usually medullary but can more rarely be splenic, hepatic or ganglionic. MAS can be primary or secondar

Causes of hepatomegaly at King Edward VIII Hospital, Durban. A prospective study of 240 black patients.

In this prospective study of 240 black patients with liver enlargement admitted to the medical wards of King Edward VIII Hospital, Durban, a cause for the hepatomegaly was found in 92.5% of cases (63.8% without recourse to biopsy, 28.7% after liver biopsy). The commonest cause was congestive heart failure (36.7%), followed by amoebic liver abscess (7.1%), hepatocellular carcinoma (5.8%) and cirrhosis (5.4%). Liver biopsy provided the diagnosis in 90.8% of patients with initial unexplained hepatomegaly. The diagnostic yield of liver biopsy was increased by submitting 3 biopsy specimens for hist

Hepatic granulomas: experience over a 10-year period in the West of Scotland.

A survey of all patients in whom liver biopsy showed epithelioid granulomas was undertaken at two major teaching hospitals in Glasgow for the period 1970-1979. Seventy-seven patients with hepatic granulomas were studied retrospectively. In 53 cases (69 per cent) a clear-cut clinical diagnosis was established, which included sarcoidosis (eight cases), tuberculosis (eight), extrahepatic biliary obstruction (seven), primary liver diseases (11), neoplasm (six), bacterial infection (five) and miscellaneous (eight). In 24 patients (31 percent) no cause was found. Seventeen patients from this idiopat
